Part Time GP RECEPTIONIST/CARE NAVIGATOR

26.25 Regular weekly hours to include 4 afternoon shifts until 6.30pm Mon to Friday plus one morning a week plus relief cover for four other team members.
A vacancy has arisen for a part-time Receptionist to join our busy and friendly GP practice team. We are looking for someone who is friendly and patient with good interpersonal skills. The ideal candidate should have previous GP receptionist experience, good organisational skills with the ability to multitask and prioritise workload. I.T. skills are essential preferably with previous scanning and coding. It is also essential applicants have a flexible approach to work and working hours as relief cover for 4 other team members both early morning and afternoon/evening is included in the role.

It is the duty of each receptionist to assist the smooth running of the Practice in general, and the Reception area in particular, and to project a positive and friendly image to our patients and other professionals who may contact us either in person or via telephone
PRINCIPLE DUTIES
1. Answering all incoming telephone calls. Diverting them as appropriate. And/or taking and passing on messages.
2. Dealing with all telephone queries/requests from patients efficiently, courteously and confidentially, and acting as liaison with the Doctors as necessary.
3. Liaise with hospital on behalf of GPs or patients.
4. Making appointments via the computerised system either on the day or in advance.
5. Greeting patients at reception, noting their arrival and dealing with any query or request as appropriate.
6. Scanning and coding medical information as necessary.
7. Managing the flow of patients at the desk and in the waiting room.
8. Processing prescriptions requests, passing them onto doctors for signing and filing for collection within the 48 hours specified by the practice. Dealing with any prompts on the EMIS Web system whilst doing so including medication reviews due.
9. Check Withnell health centre email for prescriptions requests/queries and deal with as appropriate
10. Preparing Doctors surgeries/closing down Doctors surgeries.
11. Taking visit requests.
12. Take samples at reception desk completing Pathology forms if necessary.
13. Open Building first thing in a morning including unlocking internal doors, switching on all computers or the opposite in the evening

10. Recording and informing others about RIP patients.

Main duties of the job
Healthcare Assistant Apprentices will receive appropriate training and guidance to undertake the full range of duties.
Understand the role and willingness to learn and develop theoretical knowledge and practical skills.
The role involves assisting trained staff in delivering care to patients at their most vulnerable times, ensuring dignity and respect is maintained throughout. Assisting patients to meet their personal care needs, assisting with meals and drinks throughout the day. Health Care Support Workers are also required to carry out general cleaning in the clinical environment by preparing bed spaces and cleaning equipment. The post holder needs to have good communication skills and be an effective team player. Staff will be supported to complete mandatory training.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

We are pleased to be able to offer an opportunity for an experienced mental health nurse and independent prescriber to take on an exciting new role in the Primary Care Mental Health Team.
This role will involve working in partnership with local GP practices to ensure reviews of prescribed medications are taking place for people who are under the Essential Shared Care Agreement (ESCA) with their GPs.
The role includes supporting GP practices to monitor prescribing and supporting joint working between the Integrated Mental Health Team (IMHT) and the GP Practices.
This is an extension of an existing programme of work providing joint physical and mental health reviews in local GP practices for people with a diagnosis of severe and enduring mental illness (SMI).
The role will include driving quality improvements through leading on areas such as patient satisfaction, data recording and reporting, clinical supervision, clinical audit and ensuring adherence to evidence-based practice.
Relationship building with Primary Care Networks and GP practices will form an important part of this role. An understanding of these relationships and an approach that supports collaborative working is crucial for the post holder.
Significant experience in mental health particularly working with people with severe and enduring mental health needs will be required to undertake this role.
Main duties of the job
- Work predominantly in GP practices in South East Staffordshire to provide assessment and review for individuals presenting with mental health needs.
- Review prescribing in collaboration with GP practices.
- Carry out assessments of service users following agreed practice guidelines.
- Implement and review care planning and risk management.

We are looking to recruit an efficient, and enthusiastic person 25 hours a week on a rota basis.
