Boston Primary Care Network (PCN) is made up of six practices situated across Boston, Lincolnshire. With four town and two out of town practices, the PCN serves a population of approximately 77,000 patients.

This role will focus on:
Assessing and supporting our complex, frail elderly patients, supporting a proactive, patient-centred approach. Supporting our patients to avoid crises and hospital admissions and to stay well in either their own home or care/residential home wherever and whenever possible
Supporting patients with complex health care needs.
Carrying out assessments predominantly in a patient's place of residence
To provide clinically, effective, person-centred and evidence based occupational therapy assessment and intervention to our patients.
To provide support and share information with the relevant surgery staff with regard to occupational therapy interventions
The person in role will also work closely with Practice, PCN/Neighbourhood & Care Home teams to help create and get agreement on a long-term plan for the structure and processes to support the Multi-Disciplinary Team meetings and the ongoing patient case management.

JOB SUMMARY:
To assist qualified nursing staff and consultants to deliver direct/indirect quality clinical care to patients undergoing procedures within the specialty of Endoscopy. Working without direct supervision but within defined limits and competence for the post using own initiative.
Assessment, delivery and recording of direct and indirect care to patients undergoing diagnostic and therapeutic endoscopy procedures as delegated by registered nurse/consultant.
Contribute to care of patients in transition from one care setting to another. Work within defined limits and competence whilst using own initiative with remote supervision by the sister or other registered nurse.
The primary focus of this role is to provide an effective and high quality services in relation to the specified clinical procedures within the Endoscopy service. This involves undertaking a range of clinical procedures where there is a requirement to have knowledge of the procedure, and due to the nature of the procedure being undertaken, a requirement to have a knowledge of how to deal with related non-routine activities, such as dealing with complications, answering questions, or monitoring progress during/following the procedure.

We have an exciting opportunity for a Band 6 Specialist Physiotherapist to develop to join our General Intensive Care (GICU) therapy team at University Hospital Southampton (UHS).

We are seeking an enthusiastic and dedicated HCA with strong communication skills and relevant experience to join our ward team.
Ward ANU is a 23-bedded Acute Neurological Unit, with additional 3 single rooms for video telemetry seizure monitoring. We have recently opened a 4-trolley Neurological Ambulatory Care Unit (NACU), operating as a 9am-5pm day case unit. ANU forms part of the Greater Manchester Neurosciences Centre at Salford Royal.
This is a unique and exciting opportunity to join a team with vision, drive, and ambition. We are looking for a highly motivated nurse who shares our values and is committed to delivering high-quality care. You should have extensive post-registration experience, and ideally, previous nursing management experience. Experience in Neurosciences is desirable but not essential.
A passion for delivering excellent patient and family care, alongside a commitment to continuous improvement, is essential. Strong interpersonal and communication skills are key, as is the ability to demonstrate effective leadership throughout the recruitment process.
You must be able to motivate staff to achieve high standards and work collaboratively to ensure the organisation's goals are met.

Main duties of the job

We have an exciting opportunity in Critical Care at Royal Papworth Hospital with an excellent team of nurses and healthcare support workers providing expert care to patients and their families.
We are looking for resilient, highly motivated and compassionate individuals who thrive in a fast-paced environment join our large multi-disciplinary team. You will be involved in providing highly specialised care to critically unwell patients with complex nursing care needs. This includes supporting with transfers, collection of blood products, moving and handling procedures, assisting with extubating and chest drain removal as just a small sample of what is required in this interesting and varied role.
To support you in your career development we have a dedicated Critical Care Education team committed to supporting you. You will be expected to complete the Care Certificate if not already in possession supported by the clinical Education team. You will then commence the Critical Care Healthcare support worker competencies including supportive and assistive levels. To support your development in Critical Care you will be given supernumerary support, study days and our dedicated Critical Care Education Senior Healthcare Support worker will be there through every step of your journey with us.
Main duties of the job

Job summary
An exciting opportunity has arisen for a Receptionist to join our busy admin and clerical team within the Emergency Department. Full/Part time hours considered.
We are looking for a self-motivated individual who can work both well within a team and also can demonstrate the ability to use own initiative. You will be responsible for the registration of patients attending the department and assisting our clinical colleagues with administration duties. Attention to detail and flexibility to work across a 24 hour shift pattern is essential
Knowledge of EPR and previous experience of having worked in a similar environment desirable.
Various shifts - to include late shifts, early shifts, evenings, nights and weekends
Main duties of the job
The post holder will be responsible for delivering an effective and efficient support service to the Emergency Directorate, to assist in the provision of quality patient care. This will include comprehensive clerical and administrative duties that support the administrative elements of the emergency patient pathway, along with receptionist responsibility to the Emergency Department, both Adult and Paediatric areas.
The post holder is responsible for the first line of contact with families and professionals, and will provide assistance and reassurance to distressed or anxious patients/visitor. To co-ordinate and ensure the smooth registration of patients attending the Emergency Department and designated outpatient clinics. To assist in the preparation of the administration paperwork and attend the clinical board rounds throughout various times
Act as a mutli-skilled worker within the Emergency Care Directorate adapting to the competing demands of the service

To correctly record the address/contact details of all overseas patients in line with current protocols and inform the Trusts overseas team
Case notes are made up for newly registered patients and amalgamated where necessary
Follow up appointments are made for several Emergency Care clinics and Trust Outpatient Clinics on EPR
All tasks to be carried out in accordance with relevant departmental trust standards, policies and procedures.
All x-ray reporting work on a daily basis has been checked back through Firstnet and all outpatients appointments have been made correctly. Any discrepancy in the patients appointment must be amended and the patient informed of any changes
All documents required for the Emergency Outpatient clinics are complete and available for the start of the clinics and take appropriate remedial action if not.
Greet patients in a courteous and appropriate manner
