The Primary Care Team is based within the healthcare department but there is also wing work required.
We are happy happy to talk to Newley Qualified staff who ideally has done their placements in hospital environmentsHMP Garth offers:

- 37.5 hours per week
- Week 1,3,5 - Monday, Tuesday, Saturday and Sunday 7..30am - 7.45pm - Days

Main duties of the job
As a Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

- Registered nurse Adult- NMC

The West Middlesex Hospital Intensive care Unit comprises of a combination of level two and level three intensive care beds. We cater for a broad range of critically ill patients and offer a friendly and supportive working environment where education and staff development is seen as a priority.

Main duties of the job

As a Primary Care Dementia Practitioner you will be a part of a specialist team who are based across Cornwall and are passionate about providing support for people with dementia, their families, and carers.
We work closely with the Memory Assessment Service and Dementia Older People's Mental Health toprovide seamless support throughout a person's journey with dementia, helping people to live as well as possible, for as long as possible, in their own community.
Primary Care Dementia Practitioners listen, assess, problem solve and put solutions in place, thereby, avoiding crisis, preventing admissions to care home or hospital.
As a successful candidate you willwork closely with a designated group of GP surgeries and hold a case list of people who have been diagnosed with dementia and are registered to those GP surgeries.
This is a particularly exciting time from Primary Care Dementia Practitioners as we are embarking on a joint venture between Alzheimer's Society, Disability Cornwall and Memory Matters, strengthening our collaborative working to make a positive difference for people with dementia, their families and carers.

Main duties of the job
- Offer a skilled assessment to identify the needs of people with dementia and their family carers.

This is an exciting opportunity to join our team as a Band 6 Clinical Nurse Specialist within the Supportive and Palliative Care Team at University Hospital Southampton NHS Foundation Trust.
As part of a dynamic team, you will provide expert supportive and palliative care to patients with complex needs across a range of clinical settings, supporting patients and those important to them throughout their healthcare journey.
