Job summary
There is an exciting opportunity available within the in- patient Palliative care team at the Royal Free site. We are a dynamic and supportive team who offer various learning opportunities whilst in post.
The CNS contributes to the provision of specialist palliative care in the acute hospital setting for patients facing life-limiting illnesses (within the remit of the multi- disciplinary team) accepting patient referrals to agreed criteria and managing their own caseload with the support of the Lead Nurse and Consultant Nurse. They will provide advice, education and support to patients, their carers and other health and social care professionals. They will contribute to the development and smooth running of the service. The post holder will work to agreed professional, departmental and Trust boundaries as required to provide patient centred seamless care and will take part in the 7 day / week service to hospital patients and North Camden community patients.
Main duties of the job
To manage a caseload and provide an expert and autonomous nursing service for patients with life limiting illness and potentially facing end of life care issues, and support them and their families, using advance clinical judgement and knowledge in palliative care.To lead and innovate nursing practice in palliative care on the Royal Free Hospital site.To take part in a 7 day / week face to face clinical nurse specialist - specialist palliative care service.To contribute to the weekly specialist palliative care MDTTo design and deliver specialist palliative care education; and deliver palliative and end of life education. To support and mentor students in the team.To take part in the regular palliative care team programme of service development and review of clinical practice. To develop research skills and contribute to the development of multi-disciplinary research within palliative care.

Job summary
An opportunity has arisen permanent full or part time Band 2 Health Care Support workers within the Medical Directorate at Wrexham Maelor Hospital. Morris ward is a 27 bedded acute medical ward which specialises in dementia care. We aim to provide holistic care for all patients and heavily involve relatives and carers throughout their hospital journey.
We are looking for a hardworking compassionate team player to join our friendly ward.
Applicants must have an approachable manner necessary to deal with patients/relatives and desirable experience of a hospital environment, work as part of a team of nurses on the ward and undertake patient fundamentals of nursing care tasks under the direct or indirect supervision of a Registered General Nurse responsible for the patient's management.
Following an agreed level of training, NVQ level 2 & 3 in care, the successful candidate will be required to work internal rotation on the ward. You will have an induction programme arranged for their orientation into the Medical team.
Main duties of the job
The role involves assisting patients to meet their personal care needs, recording observations and completing bedside documentation. Health Care Support Workers are also required to carry out general cleaning in the clinical environment by preparing bed spaces and cleaning equipment.
In each shift, HCA are allocated to provide care for a group of patients. Duties include but are not inclusive to providing personal care; assisting with serving meals and drinks throughout the day; recording observations and accompanying patients for investigations.

The post holder will be a member of the Critical Care team working alongside the Nurse-in-Charge and Consultant to facilitate the effective use of beds within the Critical Care Unit and Surgical Special Care Unit, ensuring timely, proactive, appropriate admissions of patients from all admission portals and discharge of patients using a structured, timely approach and in accordance with trust policies.

To facilitate the effective use of beds within the Critical Care Unit and Surgical Special Care Unit, ensuring timely, proactive appropriate admissions of patients from all admission portals and discharge of patients using a structured,timely approach.
Provide senior support with any staffing issues within CCU/SSCU
Ensure pro-active planning to allow accommodation of both elective and emergency patients
Liaise closely with Theatres/PACU regarding elective and emergency admissions.
Liaise closely and regularly with ward nursing staff and Divisional Discharge Co-ordinators to ensure accurate up to date knowledge of patient movement is obtained.
Liaise as appropriate with the discharge Co-ordinators and multi-disciplinary team with regard to discharge issues. Identify problems affecting the efficiency and effectiveness of admitting and discharging patients, making these known to the senior nursing team.
Maintain up to date bed availability state.
Maintain up to date information regarding patients who can be transferred out of CCU/SSCU ensuring that the patients notes detail the decision to transfer out.
Maintain accurate reporting arrangements e.g. through a database, regarding the occupancy and demand levels, including patient acuity.
To liaise closely with the Site Matron and senior nurses if staffing issues are affecting bed capacity.
To participate fully in any project involving bed management to ensure its success.
Check, confirm and challenge estimated dates and times of discharge out of CCU.
Using managerial / operational knowledge, discuss/negotiate clinical need/time scales for admissions with medical teams.

Main duties of the job
The post is for a Locum Consultant Anaesthetist with a specialist interest in Peri-operative medicine. The post holder will provide high quality care for patients within the anaesthetic Department and provide perioperative care for elective and emergency patients across the range of specialities available within Hywel Dda UHB. The candidate will join a team of consultants with an interest in Peri-operative medicine, doing High Risk pre-assessment, CPET clinics and assisting with our Pre-habilitation service.

Job summary
Deliver care, therapeutic support and engagement to children attending the paediatric children's and young people emergency department or admitted to the children's ward with challenging behaviour and mental health concerns as directed by the ward nursing team, Trust mental health team or the Child and Adolescent Mental Health Service (CAMHS) Urgent Care Team.
Using excellent communication skills engage and gather information from patients and carers to support a multidisciplinary assessment of the patients' emotional needs
Contribute to risk assessments and risk planning by identifying the triggers for risky behaviours, the nature of the risky behaviours, any protective measures and report these findings and any other issues of concern to the child's allocated nurse or mental health team.
Build therapeutic relationships and ongoing continuous assessment and adjustment of interventions based on the patient's needs, develop and implement new activities in response to individuals needs and recording and escalating concerns relating to care accurately and promptly.
On occasions to be redeployed across the hospital to provide support to older adolescents admitted to adult wards and adult patients with mental health issues, delirium or dementia under supervision

Main duties of the job
Be key in instigating a ward environment that is pleasant, cheerful and welcoming.
Provide high quality care, therapeutic support and engagement to children attending the paediatric emergency department or admitted to the children's ward with challenging behaviour or mental health concerns.

Main duties of the job
The post holder is responsible in assisting in the delivery of healthcare services to the prison population. Supported by registered nursing staff within healthcare, they will deliver care within the boundaries of their role, focussing on supporting patients to be healthy; monitoring of long term conditions, health promotion and screening and responding to emergencies. Our NHS 111 (Integrated Urgent care) call centres are looking for experienced, enthusiastic and motivated clinicians to join our growing team of healthcare professionals.
We are currently only recruiting clinicians who are NMC RN1 and HCPC Paramedic registered.We do not accept NMC PRN Pin as it does notfulfillthe essential criteria for the role.

The Senior Clinical Advisor is responsible forthe assessment of health and clinical needs, utilisingevidence basedpractice,professional clinical judgement, supported by decision support software, to achieveappropriate safeclinical outcomes for our patients.
Our multi-disciplinary team of staff provide a high level of clinical care to patients across the Yorkshire and Humber region, using telephone communication systems. Our clinical staff are highly skilled in the field of remote clinical triage and assessment, supported by computer-based decision-supporting tools.
Training
You will commence your role on a training contract until successful completion of your training, following this you will then be moved onto a permanent contract. Training will be 7 weeks full time.
We do not have any homeworking opportunities.
Main duties of the job
Working as part of the Yorkshire Ambulance Service NHS Trust, we offer a high standard and professional service to our patients, supporting them by providing high level, evidence-based care, where necessary signposting to the most appropriate service or care provider including, ambulance dispatch and the wider community based services.
As part of working within the IUC team, you will be offered training and support into the field of remote clinical triage and assessment, with opportunity to progress and develop professionally through a verity of CPD opportunities. Telephone triage will support your on-going accreditation.
Unsocial hours payments
All time on Saturday (midnight to midnight) and any week day after 8pm and before 6am - time plus 30%
All time on Sunday and Public Holidays (midnight to midnight) - time plus 60%

Operating within our existing shift patterns, which includes rotational day, evening and night shifts, you will provide an outstanding service to the children and families we support.
As part of our nursing team, you will have the opportunity to develop your skills and be part of a Multidisciplinary Team providing a high quality of service supporting children and young people who have severe brain injuries requiring complex neuro-disability management including long term invasive and non-invasive ventilation, tracheostomy management, severe/complex epilepsy, and disorders of consciousness.
Main duties of the job
- Adheres to the Nursing and Midwifery Council Standards of Conduct, Performance and Ethics for Nurses and Midwives at all times.
- Maintains clear and identified professional boundaries at all times.
- Create environments which empower children and families to be fully involved intheir care.
- Undertakes assessment, care planning and evaluation of childrens care needscollaboratively with families, taking action to address any issues or complications.
- Ensures individualised high-quality care is delivered at all times, that is underpinned by evidence-based practice and guidance.
- Works in accordance with The Childrens Trust policies and operating procedures.
You will be a natural and enthusiastic individual who is able to provide, challenge and support a high performing team.

We are looking for enthusiastic and passionate Bank Children Support Assistants with varying experience to join ourSpecialist Centre based in Tadworth, Surrey.
