Gloucestershire Health and Care NHS Foundation Trust View all jobs
Posted 1 month(s) ago
Reference: 2345419
Job summary
This role is offered on a fixed term/secondment basis for 9months. The hours are part-time, 30 hours per week.
Internal applicants who wish to be considered for a secondment opportunity should discuss with their line manager the suitability of a possible secondment.
Due to the current post holder commencing maternity leave, we have an exciting opportunity for an enthusiastic and skilled Physiotherapist to join the Integrated Community Team (ICT) in the Stroud District on a fixed term contract until July 2027
The ICT consists of Community Nursing, Physiotherapy, Occupational Therapy and the Home First/Reablement Team. We work collaboratively with the wider MDT within our service and our Integrated Care System (ICS) partners to enable individuals to remain as healthy and independent as possible within their own homes.
This is a multifaceted role with a varied caseload incorporating both triage and urgent response of patients referred via the referral centre, and patients from the community waiting list. The post holder will also be responsible for the delegation of work to other members of the physiotherapy team within the ICT, as well as holding a caseload. This is an ideal opportunity for an existing Band 6 or experienced Band 5 looking for a new challenge who would like to work in new ways within the Stroud area.
As you will be driving around the beautiful Stroud & Berkeley Vale area delivering our service, being a car driver is essential to the role.
Main duties of the job
We can offer you the opportunity to work in a very supportive integrated team, with access to training, regular supervision, annual appraisal and the autonomy to be a lead professional in our service delivery.
To work as a Band 6 Physiotherapist participating as a member of the community team and independently managing a specialised clinical caseload. You will also be required to manage and triage work within the Stroud Referral Centre on a rota basis.
The post-holder will be required to assess, formulate, evaluate and monitor specialised rehabilitation care packages which will involve complex decision making within the community, including Home First services and referral centre. The role can require urgent responses to priority cases and will require negotiation and co-ordination of several multidisciplinary team members to deliver specialised rehabilitation care programmes.
To supervise, train, advise, support and manage physiotherapists, physiotherapy technicians/assistants, reablement workers, students and community care workers' performance who report to the post-holder.
To be responsible for the operational management of the team that reports to the post-holder; coordinating, allocating and organising the work of junior and assistant staff to meet service priorities.
The qualification, training & experience requirements for the role are underlined in the Job Description/Person Specification.
About us
We have a skilled and dedicated workforce of over 5000 colleagues working in a diverse range of services over 55 sites and within people's homes. We strive to enable a welcoming workplace culture that builds and celebrates civility, inclusivity and diversity, while providing a sense of belonging and trust.
Annual staff surveys, regular Pulse surveys and other engagement opportunities provide our people with lots of opportunity to tell us about their experiences of working with us. In the latest staff survey, 61% of colleagues gave us their views. It was great to hear that:
- 72% of colleagues would recommend the Trust as a place to work, ranking us 1st for Provider Trusts in the South West region on this question.
- 76% would recommend the standard of care provided in our services if a friend or relative needed treatment, also ranking us 1st in the South West region.
- 81% said that care of patients and service users is the Trust's priority, compared with an average in comparable NHS Trusts in England of 64%.
This high-level overview shows we are in a healthy position, with higher scores than average for comparable organisations, alongside a great response rate, indicating good staff engagement. However, we also know we have plenty of room for improvement in many areas. To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.

Make a real difference where it matters most helping people stay safely at home during times of urgent care. Are you a passionate Occupational Therapist looking for an exciting challenge in Urgent Care? Do you thrive in fast-paced environments and enjoy making a tangible difference to people's lives? If so, we'd love to hear from you. The Community Crisis Response Team (CCRT) is a dynamic, innovative multidisciplinary team providing urgent, person-centred care to people experiencing illness in their own homes, seven days a week. Our mission is simple: prevent avoidable hospital admissions and deliver outstanding care where people want to be most at home. Working within our highly respected team, you'll be at the forefront of urgent community care, using your OT expertise to support patients during periods of acute illness. We embrace shared learning, integrated working and positive risk-taking, ensuring patients receive the right care, at the right time, in the right place. An OT is a highly valued profession within CCRT, and you'll have the opportunity to work alongside trusted health and social care colleagues while developing advanced skills in urgent care, frailty assessment, clinical reasoning and holistic assessment. You'll also collaborate closely with our Acute Response Practitioners, broadening your physical assessment skills and enhancing your confidence in managing complex presentations.
Our service provides 7 day working from 08:00 to 20:00.
Main duties of the job
- The opportunity to work autonomously with a diverse and complex caseload of acutely unwell patients in the community
- Advanced experience in urgent and crisis response care.
- Exposure to holistic assessment and management of frailty and long-term conditions.
- Strong multidisciplinary working with experienced health and social care professionals.
- Ongoing training, competency development and access to professional courses
- Supportive leadership
As an OT within CCRT, you will manage your own caseload of complex patients, providing timely assessment and intervention to help avoid unnecessary hospital admissions. You will work both independently and as part of a large multidisciplinary team, delivering responsive, person-centred care in people's homes
Whether you are looking to build your expertise in holistic urgent care assessment, develop advanced frailty skills, or make the transition from an acute hospital setting to community practice, this role offers an exceptional platform for professional growth. If you are an OT who is enthusiastic, adaptable and committed to delivering outstanding care, this role could be the perfect next step in your career.
This role may not be eligible for sponsorship under the Skilled Worker route; please refer to the Direct Gov website for more information with regards to eligibility.
Please note that Livewell may close the job advertisement earlier than the specified deadline.
All staff are expected to be able and willing to work across a 7 day service.
