Working closely with GPs Social Prescribing Link Workers Health and Wellbeing Coaches multidisciplinary teams and practice staff the post holder will help patients access the right support at the right time coordinate care and contribute to personalised care planning The role includes a range of operational and administrative duties such as appointment management patient record summarisation processing Subject Access Requests and supporting multidisciplinary team meetings
The Administrative Care Coordinator will help improve patient outcomes reduce health inequalities and support proactive care for people with complex needs long term conditions frailty cancer and care home residents ensuring patients receive coordinated compassionate and effective care across health and community services
Main duties of the job
Main Duties and Responsibilities
- Book appointments for patients requiring review through recall and screening systems.
- Process clinical documents, tasks, referrals and administrative workflows in line with practice procedures.
- Manage incoming and outgoing correspondence, drafting and initiating responses where appropriate.
- Maintain accurate and up to date patient records, ensuring all actions are recorded and completed in a timely manner.
- Summarise patient records and update clinical systems as required.
- Support the processing of Subject Access Requests and other information governance activities.
- Undertake general administrative duties to support the effective day to day operation of the practices and PCN.
- Support seamless pathways between personalised care services and wider health and community organisations.
- Coordinate patient care and ensure actions arising from multidisciplinary team discussions are completed and appropriately documented.
- Assist patients to access appropriate services and support through effective navigation and coordination.
- Contribute to reducing health inequalities by supporting patients with complex needs, long term conditions, frailty, cancer and care home residents.
- Work collaboratively with GPs, practice teams, multidisciplinary colleagues and external partners to ensure patients receive holistic and joined up care.

Job summary
An exciting opportunity has arisen for an experienced Health Care Assistant to join the specialist palliative care team working both within the community under hospice at home and the inpatient ward at the Macmillan Unit and Forest Holme. We provide palliative and End of Life Care to patients with life limiting disease. The successful candidate would need to be compassionate and demonstrate a keen interest in Palliative care. They would also demonstrate the ability to provide a high standard of quality care to our patients. The candidate needs to have excellent interpersonal skills and be able to work flexible hours. Previous NHS experience of working in palliative care environment is essential.
You will need to drive and have access to a car for this role.
The role covers shift work 7 days a week 24 hours a day.
New starters to the trust will have to undertake a numeracy and literacy test.
This post is being advertised as a Band 3, however applicants that do not meet the required care certificate competencies will initially be employed as a Band 2. Applicants who hold a care certificate accreditation will start in post at Band 3

We are looking for a Registered Nurse or Allied Health Professional to join us in this new, advanced clinical role. This is an exciting opportunity to play a crucial role in providing equitable care for Motor Neurone Disease across the Coventry and Warwickshire region.
This post has been made possible by the MND Association for the next four years, to establish this service and work with patients, carers & other services to enhance MND care and support.
You'll be working collaboratively with the Care Coordinator at Mary Ann Evans Hospice, where there is already an MND service, as well as other regional partners, multidisciplinary teams, neurology, respiratory, speech therapy, & palliative care services.

We are looking for a Registered Nurse or Allied Health Professional to join us in this new, advanced clinical role. This is an exciting opportunity to play a crucial role in providing equitable care for Motor Neurone Disease across the Coventry and Warwickshire region.
This post has been made possible by the MND Association for the next four years, to establish this service and work with patients, carers & other services to enhance MND care and support.
You'll be working collaboratively with the Care Coordinator at Mary Ann Evans Hospice, where there is already an MND service, as well as other regional partners, multidisciplinary teams, neurology, respiratory, speech therapy, & palliative care services.

- To support the family bereavement process including follow up support and notifying other agencies involved in the patients care about their death.
Care Coordination:
- Undertake complex holistic needs assessments and complete management plans across the disease trajectory from diagnosis to end of life.
- To co-ordinate with key workers within the community to prevent delays in supportive treatment and to avoid hospital admissions.
- To support, within the relevant care setting, those newly diagnosed with MND.

Job summary
Working alongside Social Workers and Social Care assessors you will be utilising a strength-based approach to promote independence and well-being. You will work in line with the Care Act to promote service users choice and control in their daily lives while respecting the key elements of the act to ensure service users can contribute to society and continue to have meaningful, autonomous and self-determined lives

Actively encourage customer participation in the delivery, monitoring and evaluation of the service
Initiate research, recommend best practice and implement changes in policy within a specialist area of service
Support in the development and operation of team workload management systems to ensure that resources address priority cases and respond to changing needs and circumstances
Undertake complex assessments and care planning
Carry out full functional assessments of people of all ages and with a wide range of disabilities. Assessments should also take into account the needs of carers and a carers assessment should be completed as appropriate.
Advise on appropriate care packages and refer on to other services as necessary
Recommend and arrange for the provision of specialist equipment, ensure correct installation, instruct client and/or carer in safe use and monitor the effectiveness of the provision as appropriate
Advise on and facilitate major adaptations, including major adaptations to properties through consultation with service users, Housing department, Environmental Health services, builders and other professionals as appropriate

Main duties of the job
As a Band 7 Critical Care Outreach Nurse Practitioner, you will undertake clinical, teaching and management responsibilities. You will be required to demonstrate significant experience in caring for critically ill patients, have a specialist course in a critical care or Emergency area and have a minimum of 2 years in a Band 6 role or above, with shift leading experience. This post requires motivation, flexibility and initiative, the ability to prioritise workload in challenging situations and excellent communication skills. The applicant should be able to demonstrate the use of evidence-based practice and be proactive in managing change.
The main duties of the role include:
- triage of acutely ill adult in-patients, autonomous rapid and advanced patient assessment, clinical intervention, care delivery and documentation

- supporting ward staff in caring for deteriorating patients and developing their skills through education and role-modelling

ICHNT SPC supports one of the largest acute trusts in the country, with a major oncology inpatient and outpatient service, major trauma unit, a large number of intensive care beds, specialist renal and haematology services and cardiac services. Main duties of the job
This post will be based at CXH, a teaching hospital with 491 beds. Palliative care works closely with oncology, acute medicine, medicine for the elderly and intensive care.Potential areas for development include increasingin-reachto A and E and ongoing work integrating palliative care in the ICU, as well as the development of an outpatient service.
SPC is well integrated with other departments and specialist teams. One of the roles of palliative care is to support the care of patients with complex medical problems towards the end of life, supporting patients to meet their preferred place of care and death, and helping to relieve the pressure on acute services. SPC champion end of life care across ICHNT and work very closely with the End of Life (EOL) Team (EOL Clinical lead, lead nurse and practice educators). SPC work alongside the treating teams to support decisions on direction of care, advance care planning and promote the use of the universal care plan.

We are seeking some highly motivated and passionate Healthcare Assistant to join the MDT team at Community inpatient unit. The service provides short term rehabilitation and reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User.
You will work alongside a wider MDT based at CIU including Nurses, Social Workers, Therapists and Assistant Practitioner to support service users to reach their rehab potential and move forward in their health journey.
You will work under direction of the Assistant Practitioner and Nursing team and will carry out assigned tasks involving personal and nutritional care needs to patients.
You will follow protocols, guidance, and procedures under the direction of senior staff/ manager/supervisor.

We are seeking an enthusiastic, experienced and flexible individual with excellent organisational and communication skills to become part of our dedicated team. The primary focus of this role is to provide high-quality, compassionate and person-centred care, while ensuring patient safety and maintaining high standards of care within the ward environment.
The successful candidate will work as an integral member of the multidisciplinary team, undertaking a range of delegated clinical and non-clinical duties in accordance with Trust policies, procedures and agreed standards of practice. This will include providing dignified and respectful personal care, supporting patients with their daily needs and contributing to a positive patient experience.
The post holder will undertake, record and report basic clinical observations and patient test results, contributing to the ongoing assessment and monitoring of patients. The post holder will work under the direction and supervision of an appropriate NMC-registered Nurse, who retains delegated accountability for the duties and responsibilities assigned.
Main duties of the job
The post holder will work under the direct supervision and delegation of a Registered Nurse/Midwife and will be expected to understand the requirements of their role, working within their scope of practice at all times. They will follow reasonable requests and instructions and undertake duties in accordance with Trust policies, procedures and agreed standards of practice. Duties may include supporting patients as a chaperone and assisting with the safe transfer of patients, equipment and goods between departments.
Following appropriate training, education and competency assessment, the post holder will undertake a range of clinical tasks, ensuring they have the necessary theoretical knowledge and practical skills to perform these safely and effectively. These may include:
- Undertaking and recording basic clinical observations, including heart rate and other vital signs.
- Performing and recording blood glucose measurements.
- Undertaking urinalysis

- Maintaining accurate fluid balance records.
- Undertaking 25-point skin assessments
- Performing bladder scans
- Undertaking venepuncture

Job summary
The Urgent Care Teams are a 24/7 service providing, first point of contact, comprehensive triage service, telephone and face to face clinical assessment to patients and/or their carers/relatives, enabling a safe and appropriate clinical decision and plan of care.
The Urgent Care teams, including the Clinical Assessment Service, Community Urgent Care Team and Crisis Support Team, are responsible for health and social care triage including supporting prevention of hospital admission and/or early hospital discharge through assessment and treatment of individuals in crisis, assessing complex and specialist needs to develop, implement and evaluate programmes of care. The Urgent Care Teams also support individuals to remain in community settings in their own home environments during crisis periods of crisis and treatment.

Salary: Range 4 £27,288 - £29,112 pro rata per annum - you will be appointed on a fixed salary point within the salary range.
You will be required to work a rota pattern which includes weekends and Bank Holidays
The post holder will provide a comprehensive, administrative service to the Urgent Care Sector supporting team members, managers, senior managers and executives by performing a variety of administrative tasks. Working independently, prioritising work as necessary and without direct supervision on routine matters. You will undertake an important role in the delivery of performance monitoring within CUCT whilst continually developing and utilising new systems to further streamline, modernise and improve the service. You will be responsible for managing office operations, scheduling appointments, planning meetings, and providing general assistance in a fast-paced environment.
