Job summary
To provide nursing and therapeutic interventions, involved in the assessment and coordination of care needs, planning and implementation of evidence based care to children, young people and their families accessing the Solar Crisis and Home Treatment Team
The post holder will provide high quality assessment for patients referred into Solar, providing a role in risk assessment and management, formulation of need, information gathering, and offering advice and guidance to children and young people directly and those who support them (parents/carers, family and professional support networks e.g school nurses, teachers, social workers)
The post holder will manage a defined caseload
The post holder will work with partner agencies (statutory and third sector) to manage the care of children and young people in Solihull
The post holder will ensure the most relevant care pathway and care plan is devised and where needed provide seamless transition into other Solar teams and practitioners
The post holder will contribute to a reduction in avoidable presentations to A&E departments
Main duties of the job
To work with a defined caseload of service users as part of a multi-disciplinary team, providing assessment, care planning and care delivery for service users with moderate to severe mental health/emotional difficulties. This will include the provision of highly skilled professional care, psychological interventions, group work, clinical assessment, risk assessments I risk management, and promoting recovery and inclusion and ensuring coproduction with service users, families and carers and the education staff. Responsible for the appropriate clinical risk assessment of service users, providing an effective advice, guidance and decision making process culminating in the coordination of any reviewed plan of care in line with the Care Programmed Approach.
Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team
Responsible for the promotion of carer and service user involvement within the service and for its provision.
Responsible for establishing therapeutic relationships with service users, in accordance with Nursing and Midwifery Councils professional code of conduct or other allied professional registration

Job summary

Provide a gate keeping service to the in-patient wards. To facilitate early discharge and provide an effective and viable alternative to hospital admission for clients in crisis by developing an individualised approach to the practical and functional needs of the service user. To provide specialist crisis resolution support and advice to other agencies and members of the multidisciplinary team. To act up in the absence of the team manager as appropriate. Will act as an independent practitioner within a defined community setting while liaising with other professionals as necessary.
Provide a specialist assessment of risk to clients coming into contact with theservice.
Will be responsible for the assessment planning implementation andevaluation of nursing care and have a working knowledge of local, nationalpolicies and legislation, which govern current service provision and audit.
Main duties of the job

We are seeking to appoint an enthusiastic, self-motivated Dental Health Support Worker to assist the Oral Health Promoter/ Oral Health Improvement Practitioner (OHP/OHIP) in implementing and monitoring Oral Health Promotion programmes.
The job will include visiting participating schools as well as other sites at various locations.
We are seeking to appoint to our Caernarfon Base.
The ability to travel between sites in a timely manner is essential and will require both the use ofhealth board and other vehicles to travel between sites and transport stock and items.
The ability to speak Welsh is essential for this role.
Main duties of the job
- To support and assist the Oral Health Educator (OHE) in implementing and monitoring Oral Health Promotion programme in a variety of locations dependent upon the assigned oral health programme, such as Designed to Smile, or, Older Persons programmes; this will include attending schools, or, other community settings such as care homes.
- To communicate with patients to gain consent for Oral Health programmes.
- To order and monitor the use of Oral Health Promotion resources, under the guidance of the OHE.
- To work as a member of the Oral Health Improvement team conducting delegated tasks related to Oral Health Promotion activities and adopting a community development approach.
- To assist the OHE staff in the development of departmental resources to support the delivery of the Oral Health Promotion programme.
- Assist the OHEs with administrative and basic computer tasks.

An exciting opportunity has arisen in renal services to work as a Chronic Kidney Disease Nurse Specialist at Wrexham Maelor Renal Unit. JOB SUMMARY / JOB PURPOSE:
Prevention of the progression of CKD is paramount to the future management of renal services and the emphasis should be on pre dialysis care and transplantation. Research has demonstrated that good pre dialysis management will slow down the descent on to renal replacement therapies thus reducing the burden of secondary care services.
In conjunction with Consultant Nephrologists, Primary Care colleagues and other members of the renal multidisciplinary team, the post holder will assess, plan implement and evaluate holistic care for patients with chronic kidney disease (CKD) stages 3-5.
The post holder will be based in secondary care but will be expected to work closely with primary care, providing expert advice and training to both patients and health care professionals. An important aspect of this role is increasing the skills of non-secondary care health professionals within the area of prevention and shared management of CKD progression.
The aim will be to develop a clinical model for the management of chronic kidney disease patients through collaborative working. The focus is on creating a continuum of care which includes relevant professional colleagues within the Trust and the community working across professional and organisational boundaries.

Main duties of the job
Responsible for assessing, diagnosing and treating patients with a wide range of podiatric conditions, including systemic, vascular, neurological, dermatological and musculoskeletal disorders, wounds, deformities, nail and skin conditions, paediatric conditions and emergencies.
Use clinical reasoning and evidence-based practice to develop, implement, evaluate and document individualised treatment plans. Review patients, identify further intervention and make appropriate referrals to GPs, District Nursing, Acute Foot Services and other services, ensuring safe discharge.
Provide care to diverse patients, including children, elderly, housebound and vulnerable patients, adapting communication and treatment to individual needs.
Undertake diabetic and high-risk foot clinics, including diabetic foot assessments, risk stratification, management of diabetic foot disease and ulcers, wound swabbing, infection management and urgent referral or hospital admission where required.
Assess and treat lower-limb musculoskeletal conditions, including prescribing and evaluating insoles, orthotics, exercise programmes and bespoke foot orthoses.
Undertake minor surgery including nail surgery and electro-surgery, administer local analgesia and manage emergency complications including anaphylaxis.
Participate in rotations including Nail Surgery, Acute Foot Services, Biomechanics, Low Level Laser Therapy and Podiatric Surgery, maintaining clinical competence and delivering safe, patient-centred care.

This role is your opportunity to be part of the new community rehabilitation team which is part of the newlyDeveloped complex psychosis pathway which has been developed to provide a whole system approach to Mental Health Rehabilitation and Assertive Community Treatment in Leeds.
The pathway which includes Newsam Ward 5 (Level 2 Rehab & relocating to Parkside Lodge), Asket Croft and Asket House inpatient settings, (Level 1 Rehab), the Community Rehabilitation Team and Assertive Outreach Team. The pathway also includes Rehabilitation practitioners embedded within the Community Hubs providing early rehabilitative and reablement interventions and enabling transitions for service users between Community Services and more intensive Assertive Outreach and Community Rehabilitation Support. These are clinical delivery services.
The pathway includes an out of area /Trust case management role and mental health placement reviewer for people with complex psychosis who are placed in Level 2 rehabilitation Independent Hospitals outside of the Trust or in funded residential placements.
In addition, Leeds Recovery College, Arts and Minds and the Volunteers Service are included within the pathway, providing innovative networks supporting social inclusion and a recovery ethos, while continuing to serve the citywide populations of Leeds, LYPFT and care services directorates.
Main duties of the job
The pathway provides treatment, intervention and vocational opportunities for people and their families living with complex needs, associated with psychosis and with significant reductions in quality of life, described within NICE guidance as "severe and treatment resistant symptoms". Many people also experience other serious and cooccurring conditions that severely affect activities of daily living, social functioning, physical health, welfare, and limit vocational opportunities. The pathway provides a strong recovery ethos, with flexibility for differing levels of need and longer term support where necessary, and in the least restrictive environment possible. The pathway aims to provide a whole person approach working together with individuals and their families on agreed goals that improve mental and physical health, welfare, social inclusion and psychological wellbeing that maximises independence and quality of life.
As part of our Occupational Therapy offer we are looking to recruit a Band 5 OT with an interest of working with individuals within community and/or rehabilitation settings who are interested in supporting individuals to live meaningful lives in the community. The role will support managing a case load and co-producing care plans to meet an individual's recovery goals and evaluating the effectiveness of these.

Main duties of the job
To work as an autonomous practitioner, responsible for the provision of medical services to the practice population, delivering an excellent standard of clinical care whilst complying with the GMS contract.
Furthermore, the post holder will adhere to the GMC standards for good medical practice, contributing to the effective management of the practice, leading by example, maintaining a positive, collaborative working relationship with the multidisciplinary team.
About us
The practice is very friendly and has a supportive administrative and clinical team. Our clinical team currently consists of 3 GP partners, 1 Practice Nurse, 1 Physician Associate, Pharmacist and Pharmacy Tech, 1 Nursing Associates, Mental Health Practitioners, and a First Contact Physiotherapist.
The Admin Team consists of a Practice Manager, Data Manager, Secretary, Office manager, and 7 reception staff.
We use EMIS Web, Docman, and Accurx and have a "Good" CQC rating with Outstanding for Well Led and a high QOF achievement. We are also actively involved with the Primary Care Network and in the training of medical students.

We are looking for a Predictive Modelling Analyst to join the analytical and population health intelligence function at NHS Essex ICB. This is an opportunity to use advanced statistical analysis to move beyond descriptive reporting and help shape strategic commissioning, population health improvement and service planning across Essex. You will work with colleagues across analytics, population health, evaluation and commissioning, using complex health and care data to forecast demand, understand risk and test future scenarios. Main duties of the job
You will develop and apply predictive, regression, forecasting, risk-stratification and scenario models to answer complex questions about population need, service demand, outcomes and resource use. You will analyse large health and care datasets using statistical and analytical tools such as R, SQL and Power BI, with Python or Stata advantageous; select methods appropriate to the question; test and document assumptions; assess model performance and limitations; and present findings clearly to technical and non-technical audiences. The Phlebotomy service is pleased to be able to offer fixed term contracts of 12 months working as a Phlebotomist in the community to cover planned maternity leave.
The service provides community venepuncture and near patient testing for housebound patients within Leicester, Leicestershire and Rutland.
Our fixed term contracts will commence in October / November where full support and training will be provided, working 30 hours per week, Monday to Friday 08.00 14.00. - Work autonomously and be a car driver and owner.
The Phlebotomy service uses an electronic patient record system called SystmOne and experience of working on this system would be advantageous; however, the required training will be provided to successful candidates.

Whiston Hospital

Mersey and West Lancashire Teaching Hospitals NHS Trust
£338 million (PFI) investment has re-built and modernised both Whiston and St Helens Hospitals, resulting in modern state of the art health care facilities for the treatment and care of patients. St Helens Hospital was completed in 2008 and Whiston Hospital in 2010. The hospitals include 887 inpatient beds with state-of-the-art CT and MRI facilities existing on both hospital sites. Whiston Hospital a busy teaching hospital which provides a comprehensive range of acute Medical and Surgical Services, Emergency Services, Maternity Services, Paediatrics and Medicine for Older People. The Regional Burns and Plastic Surgery Units are also based on this site. There is a Critical Care Unit consisting of ICU, HDU, CCU and an onsite Cardiac Catheter Laboratory. The stroke consultants deliver 24/7 thrombolysis to eligible patients presenting with acute stroke.
