Band 6 Nurse/Practitioner in Proactive Support Service
Leeds Community Healthcare NHS Trust View all jobs
Posted 21 hour(s) ago
Reference: 2413378
Job summary
We are seeking a motivated, compassionate and values-driven practitioner to join Leeds Community Healthcare NHS Trust's new Proactive Support Service.
We welcome applications from Registered Nurses, particularly those with experience in Learning Disabilities, Mental Health, Children's Services or Complex Care, as well as Social Workers and other suitably qualified practitioners with the skills, experience and values to support children and young people with complex needs.
Funded by Leeds Integrated Care Board and delivered in partnership with Leeds City Council's Corporate Parenting Team, this innovative multidisciplinary service supports children and young people at risk of care entry, placement breakdown, or delayed discharge from hospital.
The service promotes stability, prevents escalation into care and improves long-term outcomes for vulnerable young people. The team also supports the Children with No Onward Destination (CNOD) pathway, working with young people who are medically fit for discharge but unable to leave hospital due to placement instability, accommodation issues or complex social circumstances. This is an exciting opportunity to join a small, developing team where you will help shape a new service, work collaboratively across agencies and make a real difference to vulnerable children and young people.
For an informal discussion, please contact Lisa Bolton on 07851 103544.
The role is full time but we will consider job shares or part time hours.
Main duties of the job
This role would particularly suit a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD). However, we also welcome applications from Registered Social Workers and other suitably qualified and registered practitioners who can demonstrate relevant experience and skills in supporting children and young people with complex needs, trauma, behavioural challenges, and placement instability.
We are looking for a reflective and innovative practitioner who is passionate about trauma-informed and relational approaches to supporting children and young people.
Experience of Dyadic Developmental Practice (DDP), therapeutic parenting approaches, or similar models would be advantageous. Training and/or experience in Positive Behaviour Support (PBS) is also desirable.
The role involves working intensively to support children and young people at risk of placement breakdown, care entry, or delayed discharge through the Children with No Onward Destination (CNOD) pathway.
You will provide assessment, consultation, direct intervention and system-wide support, working closely with families, carers, residential providers, social care, education and health colleagues to build resilience, improve stability and reduce crisis.
A key aspect of the role is supporting and empowering the networks around the child, helping others understand and respond to behaviours of distress through a trauma-informed and strengths-based approach.

About us
As one of the largest acute teaching Trusts in England, we offer learning and development opportunities to help you achieve the career you aspire to. UHS is rated 'Good' by the CQC and in the latest national NHS Staff Survey, we were in the top 10 acute trusts for staff engagement and for staff recommending the hospital as a place to work or receive care.
We support flexible working and will consider requests taking into account the needs of the service. UHS employees are able to access a range of NHS discounts, are entitled to a minimum of 35 days paid holiday (pro rata), and we offer a generous pension scheme.
Southampton is an attractive place to live and work situated on the south coast, with an international airport and direct trains to London. The New Forest National Park and beaches of the Jurassic coast are also right on our doorstep. The city offers living costs 30% lower than London and 17 schools rated outstanding by Ofsted.
We believe that using technology wisely shows strong time management and commitment to innovation. However, personalising your recruitment application to highlight your unique skills and experiences is crucial. Relying too heavily on generic, AI-generated content instead of drawing from your own strengths and accomplishments may lead to your application being rejected if multiple candidates present identical or similar information.

Job summary
Applications are invited for the 6 months fixed term post of Consultant Ophthalmologist, Specialist Interest Glaucoma at the University Hospitals Birmingham NHS Foundation Trust (UHB). The post will be based on a full time 10.25 PA contract. This post is to cover a sabbatical for six months and will manage glaucoma work from the region, including the Birmingham Midland Eye Centre.

As well as consultant ophthalmologists the eye service is supported by ophthalmic specialist trainees, staff-grades, fellows, orthoptists, optometrists, ophthalmic nurses and ophthalmic technicians. There is a programme of training for extended roles, with Clinical Nurse Specialists in a number of specialities.
Imaging facilities vary across the hospital sites depending on the service needs, however, it currently includes OCT, OCTA, Bluepeak AF, Multicolour, HRA-OCT for FFA/ICG, California UWF Optos, Ultrasound, UBM, AS-OCT, Corneal topography and research access to FLEX mobile Spectralis and Anterion SS AS-OCT.

About us
Lancashire Teaching Hospitals NHS Foundation Trust provides systemic and radiotherapy treatments for Lancashire and South Cumbria with its base at the Royal Preston Hospital at the Rosemere Cancer Centre.
We work together with our partners across the Lancashire and South Cumbria Cancer Alliance. The Cancer Centre has grown considerably since we opened in 1997, and we are proud of our reputation for excellent patient centred care and collaborative working.
The centre saw a large expansion in 2012 co-locating chemotherapy and out-patient services alongside radiotherapy. A second chemotherapy unit was opened in 2017 at the Chorley site, continuing the investment into Oncology services in the organisation. In late 2021, our dedicated 24 bedded oncology Ribblesdale ward was fully refurbished.
Services are supported by our Macmillan Information Centre and by the Rosemere Cancer Foundation Charity.
