Amongst other neuroinflammatory conditions. As part of this service we have over 100 demyelination patients, with more than 400 hospital visits per year. The service in addition manages a further 50-100 children with auto-immune encephalitis, opsoclonus myoclonus syndrome, antibody mediated movement disorders, auto-immune myasthenia gravis as well as children with complex neurological presentations as a result of systemic inflammation. The post will provide experience in both the acute care and long term follow up, including managing a range of immune modulating treatments.

The Therapies department at The Birmingham Children's Hospital is seeking an experienced, well-motivated, enthusiastic person to play a key role in supporting the team, offering treatment and ensuring the smooth running of the service.

Stevenage South PCN are looking for an experienced and motivated health professional to support our upcoming COVID vaccination programme, primarily delivering vaccinations to care home residents and housebound patients across Stevenage.
This short-term role will focus primarily on COVID vaccinations, with the potential to support additional vaccination programmes such as RSV, Shingles and Pneumococcal where required. This role is offered with some flexibility - weekday availability is required with ideally Saturdays and Sundays.

An exciting training opportunity has arisen for multiple Trainee Education Mental Health Practitioners (EMHPs) within our established Mental Health Support Team in Schools, based in Warrington, Halton and Knowsley.

This opportunity will offer a fully funded Post Graduate Diploma leading to qualification and professional registration as an Education Mental Health Practitioner (EMHP), via an approximately 12-month training programme, beginning January 2027, delivered by Greater Manchester Mental Health NHS FT (gmmh.nhs.uk), Prestwich or Edge Hill University dependent on post.

**Mersey Care NHS Foundation Trust currently has 2 full-time Clinical Team Manager vacancies within our CYPMHS service line. Warrington Mental Health Support Team (in schools) and Knowsley core community CYPMHS**
The post holder will carry managerial responsibility for the children's community nursing team(CYPMHS), within the Community Care Division and be responsible for the overall standards which will include referral management and access, assessment of need and delivery of care within children's community nursing.

Job summary
An exciting opportunity has arisen for Band 5 Nurses to join the acute out-patient parental antimicrobial therapy (OPAT) service as part of UHD's Hospital at Home/Virtual Ward Team. This team will also be providing care to general medical patients in the future with the development of new patient pathways being explored.
The post requires flexible working between the acute sites and the service hopes to develop in the future to provide care in patients' homes to administer intravenous antimicrobials and perform patient observations and assessments. Therefore a valid driving license and access to a vehicle is necessary to fulfil this role.
Providing a high-quality service that is patient-centred is important to us; the successful candidate will be a highly motivated and enthusiastic nurse with a flexible, dynamic approach to patient care.
The role requires an active UK NMC Registration. Unfortunately we are not able to take any pre-NMC registered nurses in this role.
Base Location: Poole Hospital but cross site working will be required. OPAT run a clinic at Bournemouth Hospital once weekly.
Hours: OPAT have 30 hours available. Unfortunately we cannot offer any more than this. We work 7.5 hour shifts.

Main duties of the job
The UHD Hospital at home pathway is exciting and always evolving. Working through pathways in order to achieve flow and get patients back into their own environment. With the aim of delivering a high level of person centred acute care outside of a hospital ward environment. Safe practice is therefore essential.
Patients who meet defined eligibility criteria and have a 'Criteria to Reside' in acute hospital care, may be able to have observations, monitoring and treatments easily delivered at home, under the right supervision of our specialist teams.
You will be work alongside a team of band 7's ECPS to run a daily clinic at the Poole hospital site. This clinic sees patients to assess their treatment plan, take bloods, deliver antibiotics and provide other treatments. Other clinical duties involve screening of patients who are appropriate for the service. You will work closely with our wider MDT including; the lead Consultant Microbiologists and Antimicrobial pharmacists as well as engaging with clinical teams from a variety of specialities and community teams including patients GPs.
The Hospital at Home service is continually developing and being part of this will be part of your role.
Your role will also be vital in assisting with the development and management of the service, service improvement and governance.

Job summary
We are looking for a Paediatric Audiologist to join our team working at community sites across East Lancashire. The successful candidate will be competent in assessment and hearing aid rehabilitation of children of all ages. They will be able to demonstrate excellent communication skills and be experienced at communicating with children, families and other professionals. Experience of testing using ABR and other electrophysiological techniques would be beneficial. The successful candidate will have evidence of clinical audit and service development, and will be expected to supervise students and trainees in the department. We are closely linked withthe Newborn Hearing Screening Programme at Lancashire Women's and Newborn Centre, and are part of an active multidisciplinary network, theEast Lancashire Children's Hearing Services Working Group.
Main duties of the job
The post holder will be carrying out routine hearing assessments on children aged six months and older working with an Assistant Practitioner,or more senior colleague for complex assessments. They will work within the wider multidisciplinary team and with children's families toformulate and implement management plans when hearing loss is confirmed. There will be responsibility for leading appointments to fit hearingaids to children with permanent and temporary hearing loss, and for reviewing these patients, carrying out real ear measurements and aidedand unaided speech testing. A basic knowledge of tinnitus counselling/hearing therapy is expected. Experience in electrophysiologicalassessments would be desirable, but is not essential.The successful candidate will demonstrate a good understanding of current, relevant professional guidance and recommended procedures, beable to work autonomously and have a recent CPD record. Knowledge of clinical governance and safeguarding processes is essential. The abilityand flexibility to travel to community clinics, sometimes during the working day, is necessary. Main duties of the job

We are recruiting passionate and proactive Neighbourhood Case Managers to join the Hertsmere Neighbourhood Proactive Care Service. This is an exciting part time opportunity (26.25 hrs per week) to play a key role in a new team delivering community-based, person-centred care for residents. The focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a 'neighbourhood health service'.
You'll work as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care, and voluntary sector services.
This service will see the shift from reactive care to proactive identification by using risk stratification searches within Primary Care records. The searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, their overall experience and reduce the likelihood of further unplanned admissions. The service will operate alongside a number of acute and community services, with the ability to step patients up into appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.
As this is a community-based role, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
The post holder will be responsible for:
Overseeing care provided in the Proactive Anticipatory Care Service. This will include line management of the Neighbourhood case coordinator, ensuring proportionate and holistic patient assessment is completed, developing integrated care plans which anticipate potential future needs, using an effective multidisciplinary platform for discussion of complex cases, and maximising the value added by the role of senior clinicians.
This will include:o Provision of a proactive, personalised and multidisciplinary case management approach to "high risk" patients identified through a risk stratification search in the Neighbourhood. This includes those with end stage disease who are approaching end of life, have moderate to severe frailty or dementia and have had 2 or more unplanned hospital admissions.o Facilitation of timely, effective, and purposeful multidisciplinary discussion and decision making, resulting in coordinated and integrated care plans which can effectively anticipate and respond to changing patient needs.o Undertaking comprehensive assessment of complex care needs using tools and templates available within the electronic clinical record.
