Main duties of the job

Job responsibilities
The following are the core responsibilities of the Practice Nurse. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
a. To be instrumental in the support of the Infection Prevention Control (IPC) lead and to work to the IPC requirements by providing audits, training, support and evidence of compliance to the practice manager
b. To support in the publication of the IPC annual statement in conjunction with the practice manager
c. To develop, implement and embed an effective care management programme for the frail and housebound patients registered at the practice

Job summary
We are looking for an experienced dietitian to work within a Primary Care Network in Haverings. This is an exciting opportunity to work at the forefront of primary care, delivering specialist dietetic services directly within GP practices.
As a PCN Dietitian, you will play a vital role in supporting patients with nutrition-related conditions, helping to reduce pressure on GPs while improving access to high-quality dietetic care in the community.
This is a temporary contract.
Main duties of the job
You will manage your own clinical caseload, working autonomously within GP practices as part of a wider multidisciplinary team. Your focus will be supporting patients with conditions such as pre-diabetes, type 2 diabetes, weight management, and frailty - helping them make lasting changes through person-centred care and behaviour change approaches.

Central Park Surgery is an NHS organisation. We have very good and stabile clinical team of 2 partners, 1 salaried GP, nurses and HCA. Our proactive approach is reflected on our achievements and also our engagement in Havering North Primary Care Network. We are currently looking to recruit to the following position on a permanent contract, to work Monday to Friday. We have great supportive team, efficient and experienced leadership all working in very pleasant environment.

Hours: 20 hours per week
Work pattern can vary from 8:00am to 6:30pm, working on shifts.
The successful candidates will demonstrate excellent interpersonal and organisational skills as well as a level of competency in Information Technology. Ideally you must be a team player who is adaptable and can demonstrate good communication skills, motivation, initiative, assertive, and a willingness to learn.
