Job summary
Are you a passionate and experienced Nurse Prescriber looking to make a real difference in the lives of some of the most vulnerable members of our community?
We are seeking a compassionate and highly motivated Band 7 Nurse Prescriber / Visiting Nurse to join our friendly and supportive practice team. This rewarding role focuses on delivering high-quality, person-centred care to care home residents and housebound patients, helping them remain safe, well and independent in their own homes wherever possible.
Working autonomously and as part of a collaborative multidisciplinary team, you will provide clinical assessment, diagnosis, treatment and prescribing, while building meaningful relationships with patients, families, carers and care home staff. You will play a key role in coordinating care, preventing avoidable hospital admissions, and ensuring patients receive the right care at the right time.
You will also have the opportunity to share your knowledge and expertise through mentoring, supporting and developing colleagues, contributing to service improvement and helping shape the future of community-based care within the practice.
This is an exciting opportunity to join a forward-thinking organisation that values kindness, teamwork and innovation, where you can use your clinical skills, leadership abilities and professional judgement to make a genuine difference every day.
Main duties of the job
The Nurse Prescriber Visiting Nurse will work as an experienced and autonomous member of the multidisciplinary primary care team, delivering high-quality, evidence-based nursing care predominantly to care home residents and housebound patients.
The post-holder will undertake comprehensive clinical assessment, diagnosis, treatment and prescribing within their professional competence, providing safe and effective care to patients who are unable to attend the practice.
The working day will primarily be structured around:
Mornings: proactive and responsive visits to care home residents and housebound patients, including assessment, prescribing, treatment planning and clinical review.
Afternoons: routine and straightforward home visits that can safely and appropriately be managed by an experienced prescribing nurse.
Appropriate liaison and escalation to the patient's GP, community services, urgent care or secondary care where the patient's needs fall outside the post-holder's competence or require medical review.
The post-holder will work closely with GPs, nursing colleagues, care home staff, community nursing teams, pharmacy professionals, social care and other services to provide coordinated care and help avoid unnecessary hospital attendance or admission where appropriate.
The role will additionally provide clinical leadership, mentoring and professional support to nursing colleagues and other healthcare professionals.
