Main duties of the job
This is an exciting opportunity to join our team as a Neonatal Milk Kitchen Assistant. The successful candidate will support the safe and accurate preparation, handling, labelling, storage and distribution of all milk feeds within neonatal intensive care and special care units. You will work to strict hygiene, infection control and food safety standards, maintain accurate records, monitor stock levels and ensure feeds are prepared and delivered safely and on time.
The post holder will report to the Neonatal Milk Kitchen Manager and will work closely with nurses and doctors, contributing to the highest standards of care and safety for vulnerable babies within the neonatal service. The ideal candidate will be reliable, organised, attentive to detail and able to work effectively as part of a busy multidisciplinary team.

Main duties of the job
This is an exciting opportunity to join our team as a Band 6 Dietitian with a keen interest in complex nutrition support to join our acute adult dietetic team at University Hospital Southampton.
The post will work across the intestinal failure and colorectal teams providing oral, enteral and parenteral nutrition support and advice on high output stoma management.
The post will cover primarily type 1 intestinal failure patients requiring parenteral nutrition and work as part of our Nutrition Support and Intestinal Failure multidisciplinary team. In addition, the post holder will be the dietetic lead for colorectal surgery, following patients throughout their journey as inpatients and outpatients with a weekly colorectal outpatient clinic part of the role.
The department of dietetics and speech and language therapy is a large and friendly team, employing approximately 130 staff. We have clinical experts working in a range of specialties, providing acute and community services in adults and paediatrics. We are a tertiary referral centre for many specialisms, and have a commitment to providing clinical excellence.

Job summary
We are looking for highly motivated HCPC-registered Band 6 Biomedical Scientists to join the Specialist Services Team within the Laboratory Medicine Service at Swansea Bay University Health Board.
The Laboratory Medicine Team seeks to modernise our service by embracing new ways of working, committing to developing our knowledge and skills and actively seeking to develop a professional multidisciplinary team across the Service that provides a 24/7 365, year-round service for our patients.
In this role, you will be part of a rotational team that provides the specialised laboratory services using specialist analytical techniques such as flow cytometry, protein electrophoresis, spectrometry, immunofluorescence, mass spectrometry and specialist coagulation techniques. As part of future planning, this role will include out-of-hours working within Specialist Sections.
The Specialist laboratories are well equipped with equipment from suppliers such as Sebia, Werfen, Sysmex, BD, Thermo Fisher Scientific, Aesku and The Binding Site.
Ideally the post holder will have existing knowledge and skills in specialist analytical techniques such as those listed above, however we welcome applications from candidates who are interested in developing their knowledge and skills specialist analytical techniques.
Salary: Initially Annex 21 Band 6 - 70%, progressing to 75% after year 2.
Main duties of the job
As a Band 6 Specialist BMS you will provide a comprehensive Laboratory Medicine service to aid the effective screening, diagnosis, monitoring of patients. You will deputise for senior staff and assist in the training and supervision of junior members of the team.
The Laboratory Medicine Service is a 24/7/365 operation and a flexible approach to working hours is required. Once competency has been achieved you will be expected to participate in the out of hours rota and expected to work at any of the networked laboratory sites as required.
Candidates will be expected to have completed the IBMS Specialist Portfolio or have significant years of experience as a Band 6 - this is a requisite for employment at Band 6 Biomedical Scientist AfC grade.
If you have completed your IBMS Specialist Portfolio and are awaiting assessment please clearly indicate this in your application.
Consideration will be given to candidates who are already working towards completion of the IBMS Specialist Portfolio and will initially be paid on the Band 6 Biomedical Scientist AfC scale as per our Annex 21 framework. Trainee Specialist BMS - Applicants who do not hold the full qualifications will be considered for a trainee position and will be subject to Annex 21 of Band 6. We are looking for aqualifiedEducational Mental Health Practitioner (EMHP) or Children's Wellbeing Practitioner (CWP) who wishes to join an existing school based Mental Health Support Team (MHST).
The post-holder will work in the East MHST which is a fully established MHST. The is a caring and supportive environment to work in with good leadership providing clear objectives and direction with regular supervision. The team supports 56 educational settings across the borough and we have built solid working relationships with each setting.

Main duties of the job
This role will involve working alongside qualified and trainee EMHPs/CWPs, Senior Practitioners and a Team Leader. The service provides early interventions to children and young people experiencing mild to moderate mental health concerns in an educational setting.
The Mental Health Support Teams operate year-round, providing support to young people during school holidays in addition to term time. The core hours will be Monday - Friday 9-5 but some work (facilitation of group work/workshops) may be required outside of these times.
The post-holder will work with people of different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities.

The Children's service provides support to children, young people and their families within a progressive universal model of service delivery dependent on assessed need.

Main duties of the job
Responsibilities will include caring for children with urinary catheters, enteral feeds, and medication requirements along with an awareness of tracheostomy and stoma care within the school and community settings. Responding to emergency situations and contributing towards specialist individual care plans to meet the child/young person's needs alongside staff training is an important aspect of the role.
Public Health
The post holder will contribute towards undertaking holistic assessments of children, young people and families in planning and delivering the Healthy Child Programme, including health screening and immunisations, plus a requirement to safeguard and support the health needs of any vulnerable children/young people within the caseload across the city.

We are currently advertising for a Specialty Doctor in Anaesthesia. This post is for 10 programmed activities. The post holder will be expected to participate in a 1:8 on-call rota initially on the intensive care unit. A breakdown of the draft job plan for the post is included in the information pack.
The post offers the opportunity to work in a small, ACSA accredited department in an award-winning hospital in an area of outstanding natural beauty within the heart of England in the rolling countryside near Skipton and Ilkley, close to the Yorkshire Dales National Park.
The post-holder must have completed at least three years of postgraduate training in the specialty of Anaesthesia and must have basic competencies in obstetric anaesthesia and critical care medicine. Or must have completed three years of single stem intensive care training and have basic anaesthetic competencies.
Main duties of the job
The successful applicant will join a department of 24 consultants, 12 specialty doctors/associate specialists and up to 12 rotating residents from the Yorkshire Deanery. We encourage involvement in our existing quality improvement projects and as well as new ideas for improving or developing new work streams. The department has recently been awarded Anaesthesia Clinical Services Accreditation (ACSA) having been commended by ACSA in many aspects including team communication, simulation training and Specialty Doctor career development.
The initial job plan is for 10 PAs with opportunity for extra PAs (up to 12 in total) to include in hours and resident out of hours working on the critical care 1:8 rota. Time is allocated within job plans for any additional responsibilities in addition to SPA time. Professional leave is available for agreed additional responsibilities in addition to study leave.

The post holder will play a key role in identifying learning opportunities across the organisation.

Job summary
The post holder would be expected to undertake a wide range audiological investigations, which may be of a specialist nature on adults, children and babies.
Additionally, to plan and deliver complex audiological and rehabilitative treatments on adult patients who have hearing impairments and that may have complex additional needs i.e. patients who have mental health problems, disabilities and tinnitus.

Job summary
The Acute Oncology Service (AOS) delivers a 7-day service to patients admitted to Peterborough City Hospital and as well as Hinchingbrooke Hospital (5 days a week) with complications of systemic anti-cancer treatments (SACT) and radiotherapy treatments.

This is an exciting opportunity to help establish a high-quality urgent care service from its inception, providing assessment, diagnosis and treatment for patients with urgent healthcare needs.

Job summary
Our practices are looking for a highly motivated and enthusiastic full-time clinical pharmacist to join our clinical team and work as part of a multidisciplinary team in a fully patient-facing role.
This is an exciting opportunity for a Clinical Pharmacist to join an established medicines team within the PCN footprint. You will be based across 2 to 3 practices so you will have the opportunity to build up relationships with the primary care team.
The role will be supported by a lead Clinical Pharmacist in our PCN. There are regular teaching sessions, and all our practices are engaged in ensuring the role is optimised and developed.
We are keen to hear from pharmacists of all backgrounds and level of experience, whether you have already completed or would like to complete the independent prescriber (IP) qualification.
Main duties of the job
The post holder will work as part of a multi- disciplinary team in a patient-facing role. The PCN Clinical Pharmacist will take responsibility for areas of chronic disease management and work proactively with other healthcare professionals as part of a multidisciplinary team across the PCN. The successful candidate will join a core Pharmacy Team and will be supported by a Senior Clinical Pharmacist and a GP Supervisor. Regular development sessions, teaching, training and opportunities to upskill will be provided.
Clinical pharmacists have a key role in supporting delivery of the new network contract Directly Enhanced Service (DES) specifications.
The PCN Clinical Pharmacist will need to be committed to improving health and putting patients first. Key qualities will be the ability to build effective relationships with patients, GPs, nurses, community pharmacists, hospital and practice staff, and to work effectively as part of a team. Demonstrable general pharmaceutical skills and knowledge in core areas, including long term conditions, together with a good understanding of general practice are essential.

Commissioned improvements for QIPP and medicines optimisation
Support the practice and the PCN in delivering the outcomes as required in the medicines section of the Clinical Commissioning Contract
Signposting
Ensure that patients are referred to the appropriate healthcare professional for specialist referral, monitoring and/or referred to the social prescriber for further support services where required.
Repeat Prescribing
Review, implement and support adherence to the practice repeat prescribing policy.

Medicines Reconciliation
Reconcile medicines following discharge from hospitals, intermediate care and into care homes, including identifying and rectifying unexplained changes and working with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
Commissioned improvements for QIPP and medicines optimisation
Support the practice and the PCN in delivering the outcomes as required in the medicines section of the Clinical Commissioning Contract
Signposting
Ensure that patients are referred to the appropriate healthcare professional for specialist referral, monitoring and/or referred to the social prescriber for further support services where required.
Repeat Prescribing
Review, implement and support adherence to the practice repeat prescribing policy.
Manage the repeat prescribing reauthorisation process by reviewing patient requests for repeat prescriptions and reviewing medicines reaching review dates and flagging up those patients needing a review.
Ensure patients on repeat prescriptions have the required monitoring and assessments undertaken as appropriate to their condition(s) and medicines prescribed.
Clinical Governance
Communicate, action and support implementation of MHRA alerts, product withdrawals and other local and national guidelines and formulary recommendations.
Undertake clinical audits in priority areas identified by the CCG medicines management team or within the PCN and feedback results to practices and the PCN and support them in implementing recommendations.
Support practices and PCNs in identifying and reporting onto NRLS any incidents relating to medicines. Be involved in investigating medicines incidents and identifying and implementing any required system improvements.
Support implementation of shared care protocols in practice, including processes to improve monitoring of high- risk drugs.
Review the use of medicines most commonly associated with unplanned hospital admissions and readmissions through audit and individual patient reviews.
Identify cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches e.g. using PINCER and other tools.
Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
Education and Training
Contribute to the provision of education and training on medicines optimisation and specific therapeutic areas to healthcare professionals within the PCN and to the wider CCG geography.
Public Health
To support public health campaigns.
To provide specialist advice on all public health programmes available to the general public.
General
Undertake any other work supporting medicines optimisation in GP practices and the PCN commensurate with the banding of the post and which is agreed between the postholder and the manager
Key Collaborative Working Relationships
The post-holder will be required to work with a wide range of stakeholders. It is important that the post holder recognises the roles of other colleagues to person-centred care, and liaises with other stakeholders as needed for the collective benefit of patients including:
Patients and Patient Participation Groups (PPGs)
GPs, nurses and other practice staff
Other healthcare professionals including CCG pharmacists, pharmacy technicians, optometrists, dentists, health and social care teams, dieticians etc.

Has experience/awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
Minimum of 2 years post graduate experience in pharmacy, as demonstrated within a practice portfolio.
Holds an independent prescribing qualification, is working towards a qualification or has the intention to complete this in the future.
Demonstrates an understanding of the role of a clinical pharmacist within primary care and where it fits into and how it complements the practice team and the wider PCN team.
Accountable for delivering professional expertise and direct service provision.
Able to follow legal, ethical, professional and organisational policies/procedures and codes of conduct.
