Main duties of the job
- To act as a Specialist Palliative Medicine Consultant to all clinical services at City Hospice and contribute to sessions in Velindre Cancer Centre under auspices of Velindre University NHS Trust
- To provide high quality, timely, patient-centred and consultant led care for in-patients, outpatients, day-case and community-based patients and those accessing oncology assessment unit, radiotherapy and other services
- To work to ensure an integrated model of palliative care service delivery to all patients within the catchment areas Velindre services by collaborating closely with other specialist and generalist palliative care providers.
- To assess and manage patients attending Velindre who are acutely unwell and symptomatic, including acute oncology service input
- To contribute to teaching, audit, quality improvement and research activities

We are seeking to appoint an experienced, enthusiastic, motivated and forward thinking Band 6 occupational therapist to work within the Specialist Palliative Care service based at City Hospice. Experience of working in the community is desirable but not essential as the successful candidate will receive a full supportive induction when they join the team.

Main duties of the job
The post holder will contribute to the provision of specialist palliative care, supporting patient choice in maintaining dignified end of life care, to the people of Cardiff with malignant or non-malignant conditions who have a limited life expectancy. The post holder will be working autonomously and as part of a team. They will deliver therapeutic interventions in patients' homes, and/or when they attend City Hospice as an out-patient or attend our well-being services. They will provide advice, education and support to patients, their carers and other health care professionals. They will also promote advanced care planning, management of complex end of life symptoms and patient and carer expectations , whilst meeting the integrated care needs of the patient.
The post holder will also need to be passionate about delivering quality care to people receiving palliative or end of life care.

City Hospice is Cardiff's local hospice team offering palliative care in the community for people with life limiting conditions. Our CNS team supports them so that they can live their life in their place of choice, with dignity, and in comfort with those close to them in their bereavement.

The post of Palliative Care Nurse will be community based and will provide, as part of a team, specialist, intermediate and generalist palliative care specific to the needs of patients, their carers and families. They will be accountable clinically to a Consultant in Palliative Care Medicine, and managerially to the Lead Clinical Nurse Specialist.
