Job summary
Social prescribing empowers people to take control of their health and wellbeing, focus on what matters to me and take a holistic approach to an individual's health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.
Social prescribing link workers work as a key part of the primary care network (PCN) multidisciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people's active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
We are looking for someone who can work 22.5 hours a week over three days, ideally Monday to Wednesday but there may be some flexibility for the right candidate.

Main duties of the job
The Social Prescriber will work as part of the Primary Care Network's multidisciplinary team to support patients with non-medial needs that impact their health and wellbeing.
The role includes undertaking home visits where appropriate, completing wellbeing assessments and developing a personalised support plan. It involves collaborative working with health, social care and voluntary sector organisations as well as carers and families.

You will be working within Gordano Valley PCN, a network of four local GP surgeries: Clevedon Medical Centre in Clevedon, Harbourside Family Practice in Portishead, Heywood Family Practice in Pill and Portishead Medical Group in Portishead. The PCN serves over 50,000 patients.

Identify barriers to, or issues preventing people from taking up support and use a person-centred approach to develop solutions with the person

Job summary
We are looking for a passionate and experienced Research Nurse to join our friendly and supportive GP Surgery research team. We have been fortunate to see a lot of growth in our capacity for research, which is credited to our hardworking research team, and have a great reputation locally and nationally for delivering innovative research projects to our patients and the local community. We have recently used NIHR funding to create a bespoke Research hub within the surgery and are lucky to employ various clinicians in the current team.
Main duties of the job
- Monitor participants throughout clinical trials
- Maintain accurate and timely clinical documentation and records
- Promote participant safety and wellbeing throughout studies
- Work collaboratively with physicians, technicians & the wider multidisciplinary research team
- Screen potential participants, demonstrate knowledge of relevant study protocols
- Conduct study appointments, procedures, including vital signs, blood/urine sampling, ECGs, medical histories and study medication administration, as required
- Complete data entry, safety reporting
- Maintain accurate, legible and detailed records, including up-to-date training records
