Job summary
We are seeking a compassionate and motivated Cancer Care Coordinator to join the friendly and supportive team at Coastal Fareham & Gosport PCN.
Working across three GP practices, you will support patients following a new cancer diagnosis, acting as a dedicated point of contact. You will help patients and their families access information, navigate healthcare services, and connect with the support available to them, ensuring they feel informed, supported and heard.
This role is ideal for someone with a clinical background who is looking to step away from direct patient care while continuing to make a meaningful difference. You will use your knowledge, empathy and communication skills to support and improve the experience of patients at a pivotal time in their lives.
We are looking for someone who is:
- Compassionate and patient-focused.
- An excellent communicator.
- Organised and able to manage a varied workload.
- Proactive and motivated.
- Able to build positive relationships with patients and professionals.
- A flexible team player.
In return, you will join a welcoming multidisciplinary team committed to delivering high-quality care and improving patient experience within our local community.
If you are looking for a rewarding role where you can support patients when they need it most, we would love to hear from you.
Working pattern: 22.5 hours per week across 3 days (practice-based)
Interviews: Week commencing 23 November 2026
Main duties of the job
Care Coordinators play an important role within Primary Care Networks (PCNs), working proactively to identify and support individuals - including the frail/elderly, early diagnosis of cancer and end of life care. In this role you will coordinate and navigate across a range of health and social care services, ensuring patients receive a seamless and person-centred approach.
You will work closely with the wider PCN team and contribute to the implementation and delivery of the NHS England Early Cancer Diagnosis Designated Enhanced Service.
You will ensure that patients affected by cancer are provided with timely information, appropriate onward referrals, and support tailored to their individual needs.
The role also involves supporting practices across the PCN to enhance the quality of their cancer-related services, including improving the uptake of national cancer screening programmes.
You will support MDT meetings and manage a caseload of patients identified through these discussions, collaborating with the Cancer Clinical Lead and Clinical Care Coordinator.
This is a pivotal position within the PCN, essential to strengthening multidisciplinary ways of working and ensuring coordinated, compassionate care for patients living with cancer.
About us
