Job responsibilities
Duties
- Number of patient contacts mix of f2f/tel A minimum of 15 booked appointments per clinical session but more if demand requires.
- Home visits A proportion of visits up to a maximum of 2 visits per working day, unless undertaking a triage day.
- Administration Paperwork, Prescriptions, Correspondence, Queries and Test Results As and when required.
- On Wednesday per month afternoon PLT sessions when the practice is closed, if you are not required to attend, then this session would be owed back to the practice, using any holiday/ bank holiday allowance upon agreement between yourself and the practice (dependent on working days)
Clinical responsibilities:
In accordance with the above timetable, you will undertake a variety of duties including surgery consultations, telephone consultations and queries, visiting patients at home, checking and electronically signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the health care needs of patients with undifferentiated and undiagnosed problems
Screening patients for disease risk factors and early signs of illness
In consultation with patients and in line with current practice disease management protocols, developing care plans for health
Providing counselling and health education
Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
Recording clear and contemporaneous consultation notes to agreed standards
Collecting data for audit purposes
Compiling and issuing electronic acute and repeat prescriptions
Prescribing in accordance with the practice prescribing formulary (or generically) whenever this is clinically appropriate
In general the post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care.
Baby checks as and when required
Completion of cremation certificates and medical reports (NHS and private) when required (fees to be retained by practice (unless prior agreement with GP partners and completed outside of normal working hours/sessions)

We are an established Practice looking after approximately 9500 patients over a wide geographical area. We are a forward thinking and innovative GMS Practice, situated in the thriving market town of Stone.
Main duties of the job
You will be responsible for the assessment of health and clinical needs of our patients, undertaking consultations as an autonomous practitioner, using your advanced clinical practice skills, and own clinical judgement to diagnose, treat or refer patients.
