An exciting opportunity has arisen for the recruitment of a Community Staff Nurse based at Great Eccleston Nursing Team. The advertised role is a Temporary Fixed Term 12 month post (30.0 hours per week).
It is essential that you have a full UK driving licence and access to a car for this Community Nursing role.

NMC Level 1 Registration (Adult Branch) & Enhanced DBS clearance required.

Main duties of the job
The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.
To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.
To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.

We have an exciting opportunity to join our Urology Secretarial / Waiting List Officer Team based at Stepping Hill Hospital. Poole North Primary Care Network (PCN), a network of 4 GP practices covering 52,000 patients is looking for a Pharmacy Technician to join it's team of 3 techs, 6 pharmacists and care coordinator to help support the work our team does across our practices each day, replacing a member of our team who is retiring.
Working under the supervision of the Clinical Pharmacist, our Technician team ensure the safe, accurate and timely supply of prescribed medication to patients and provide technical and administrative support to the Clinical Pharmacists and Clinicians within the PCN.
Theysupport the primary care team with queries around medication from patients; including face to face and telephone support to patients, primary and community staff and will, after training run their own clinics in areas of interest to them and need of the practice.
Main duties of the job
Our team provide medication review services to patients that maximise cost-effective prescribing and improve the quality of patient care andsupport the achievement of the practices prescribing Quality and Outcomes Framework targets and to be involved with any DES/LES/NHSE/ICB prescribing initiatives and targets.
Our team deal with medication queries including acute medication requests, patient medication and prescription queries.
They liaise with secondary and primary care colleagues to ensure correct medicines are continued following the transfer of care and upon discharge from hospital to ensure full medicines reconciliation to include other transfers of care (e.g. new patients, care home residents), to ensure that medication changes are safely implemented in a timely fashion.
They link with hospital and community pharmacy colleagues to address discrepancies, supply issues and queries within competency and will support patients to manage their medications, for example; synchronisation of medications, reviewing patients suitability for monitored dosage systems, and setting up electronic repeat dispensing.
They ensure actions are undertaken from any Central Alert Notifications regarding medications by running searches for patients on said drugs, agreeing to actions with GPs, contacting patients affected regarding medication changes and logging actions on practice-based evidence spreadsheets.

Poole North Primary Care Network is a supportive and forward thinking organisation which has grown over the last six years to include pharmacists, pharmacy technicians, care coordinators, social prescribers, MSK Practitioner, counsellors other allied healthcare professionals.
Our pharmacy team are co-located at our hub at Birchwood Medical Centre where the technicians are supported every day by an on duty Pharmacist.
The team is supportive and friendly and new recruits are guided through their first few months with support and regular supervision.

Main duties of the job
- Answering calls using our Call Centre Storm System for Acute and Clinical Assessment services.
- Manage and action worklists efficiently, ensuring patient pathways remain appropriate and up to date.

- Apply RTT guidance accurately, escalating concerns where delays or pathway issues are identified.

Job responsibilities
To type clinic notes, operation notes, pre operative notes, ward notes, specialist reports, discharge summaries, conference notes and correspondence of varied medical terminology, accurately through audio typing distributing as necessary to the relevant healthcare professions. This work involves a significant use of word processing and the Trusts computer systems, PAS, EPR and Microsoft Office and digital dictation system.
Process incoming mail and distribute as necessary on a daily basis.
To scan documents into the correct section of the patients EPR record, performing a quality assurance check for each scanned document.
To regularly check Big Hand for urgent dictation. Also checking for missing dictations and alerting the Medical Secretary.
Ensure all clinical letters are completed and dispatched, where possible, within the10 day standard as directed by the Trust liaising with medical staff to encourage dictation and signing of letters in a timely fashion.
Complete Inter Provider Transfer forms accurately to effectively coordinate patients pathway with other hospitals.
Requesting external case notes and x-rays via IEP.
The use of photocopying handling skills and fax machine skills to reproduce or fax documents as required.
Ensure that all entries and information on the hospital computer systems such as PAS and EPR are accurate and complete.
To liaise with the Medical Secretary and when directed to answer incoming telephone calls as well as carrying out external telephone calls.
The ability to deal with frequent interruptions whilst maintaining levels of concentration.
