The Position
As a Clinical Nurse Consultant, you will provide leadership within the team in supporting palliative clients in a variety of locations, including home visits to patients and their families, assessing conditions and needs for supports and reporting back to the specialist team. Working collaboratively with the wider team, you will ensure timely response to requests for support and assess response to interventions. This role requires excellent communication and documentation skills to thrive.

The Position
The Clinical Nurse Consultant – Aged Care is the clinical lead for Lyell House, the Residential Aged Care Facility at West Coast District Hospital.
Key responsibilities include:
- Leading and promoting best practice in aged care nursing and geriatric medicine
- Acting as Infection Control Lead for West Coast District Hospital
- Conducting comprehensive assessments and care planning with residents, families, and multidisciplinary teams
- Providing expert clinical advice and support to staff Supporting quality improvement, risk management, and compliance activities
- Assisting the NUM and DON with documentation, reporting, policy development, and admissions
- Supporting staff education and professional development in collaboration with the Clinical Nurse Educator
- Collaborating with the Activities Coordinator to oversee resident activities

The Youth Hospital in the Home (Y‑HiTH) team is part of the Child and Youth Mental Health Service (CYMHS) in North‑West Tasmania. Based in Devonport, this multidisciplinary team provides intensive, home‑based mental health care for young people aged 16–24. Their work addresses a critical service gap in the region by offering timely mental health support outside of hospital settings.
The Position

Be part of a multidisciplinary youth mental health service (16-25 years of age) delivering high quality mental health care and, in accordance with Mental Health Service principles, National Mental Health Standards, Agency policy, legal requirements and relevant professional competencies.
You will need to:
- Provide clinical expertise in contemporary nursing practice within the Y-HiTH setting, promoting professional standards of practice and an environment conducive to innovation and change that promotes best practice and meets established accreditation standards of practice.

We're looking for a dedicated & compassionate Specialty Doctor to join Home Treatment Teams (HTT) in West Surrey for Older Adults. This will also provide cover to urgent referrals via SPA for Older Adults. The teams covered would include North East Hampshire HTT, South West Surrey HTT & North West Surrey HTT.
Our HTT offers urgent & intensive mental health support to adults in crisis, helping people stay at home & avoid hospital admissions wherever possible. You'll be working with a skilled & welcoming multi-professional team.
As our Specialty Doctor, you'll provide clinical leadership, expert assessments, & treatment planning. You'll also support early discharge from hospital & work closely with local teams, services & carers. The role is central to helping people recover in the community with the right support in place.
The post-holder is also expected to liaise effectively with the other general and specialist teams operating in Surrey to ensure effective handover of care when patients transfer between teams as part of the patient pathway.

Main duties of the job
The HTT interfaces with Inpatient Acute Services, Single Point of Access Triage Services, the Section 136 Places of Safety, Community Services, Liaison Services & the Criminal Justice System. Most patients are referred from NMHT & Liaison Services.
The post holder will be responsible for delivering Consultant psychiatric input & clinical leadership to the MDT & acting as a consultant/advisor to the team in line with the developing models of practice under 'New Ways of Working'
On average, a full time Specialty Doctor will have 10-12 visits per week.

An exciting opportunity has arisen for an experienced Advanced Clinical Practitioner to join our Urgent Community Response, Virtual Ward and Reablement Team within Nottingham CityCare Partnership.
The Advanced Clinical Practitioner plays a pivotal role in delivering rapid, high-quality clinical assessment and intervention for adults experiencing an acute health or functional crisis in the community. The ACP provides expert autonomous practice to prevent avoidable hospital admissions, support safe and timely discharge, and enable effective reablement, ensuring people can remain safely at home wherever possible.
The postholder leads clinical decision-making within a multidisciplinary team, provides advanced assessment and treatment planning, prescribes medication where appropriate, and contributes to service development, staff training, and integrated working across health and social care partners.
The ACP carries clinical responsibility for a complex caseload, applying advanced investigative and analytical skills to lead safe, evidence based practice, support timely discharge planning across community settings, and provide specialist advice to colleagues across the multidisciplinary team.
The post holder will have a Trust-wide responsibility to promote clinical excellence in the care of patients presenting to their area by providing clinical advice and support to nursing staff and other health care professionals.

Main duties of the job
The ACP is accountable for independent clinical assessment, diagnosis and treatment of patients with undiagnosed and undifferentiated conditions. They will practice at an advanced level demonstrating in depth knowledge and competence in all aspects of UCR/Virtual Wards, encompassing advanced clinical assessment, critical thinking and clinical management skills that are evidence based and deliver high quality patient centred care.

Our HTT offers urgent & intensive mental health support to adults aged 18-65 years in crisis, helping people stay at home & avoid hospital admissions wherever possible. You'll be working with a skilled & welcoming multi-professional team, supporting people across Woking, Elmbridge, Spelthorne, Runnymede.
As a Specialty Doctor, you'll provide clinical leadership, expert assessments, & treatment planning. You'll also support early discharge from hospital & work closely with local teams, services & carers. The role is central to helping people recover in the community with the right support in place.
The post offers flexible working options, admin support, & CPD opportunities.
Main duties of the job
The post holder will be responsible for delivering psychiatric input & clinical leadership to the MDT & acting as a consultant/advisor to the team in line with the developing models of practice under 'New Ways of Working'
The post-holder is also expected to liaise effectively with the other general & specialist teams operating in Surrey to ensure effective handover of care when patients transfer between teams as part of the patient pathway.
On average, a full-time Speciality Doctor will have 10-12 visits per week.

Job summary
The ESTEP team is a community Early Intervention in Psychosis service for people aged 14-65 experiencing a first episode of psychosis.
Psychological practitioners work autonomously within professional guidelines to provide specialist, evidence-based assessment, formulation and intervention for service users and their families, in line with NICE and Department of Health guidance. The role involves developing comprehensive psychological formulations, delivering tailored individual and group interventions, and creating collaborative care plans that are communicated effectively to service users, carers and the multidisciplinary team.
The postholder will work closely with colleagues and partner services to provide high-quality, cost-effective psychological care, contribute to referral management through MDTs, and support clinical decision-making by developing working hypotheses and informing care pathways. They will also provide specialist psychological consultation to professionals across services.
In addition, the role includes contributing to service development, audit and evaluation, advising managers on the effectiveness of psychological interventions and models of care, and helping to shape innovative, psychologically informed practice. All duties are undertaken in accordance with Trust values, HCPC and BPS standards, and a commitment to compassionate, person-centred care.

Job summary
The National Patient Safety Team is searching for an exceptional individual who can demonstrate extensive experience in patient safety improvement and outcomes, ideally at regional and national level.
As a member of the team leading the development and implementation of the patient safety incident response framework (PSIRF), you will work on the design, oversight and delivery of national programmes associated with this policy.
Main duties of the job
Based on a strong understanding of quality improvement science, patient safety in healthcare systems, and programme management expertise, the post holder will have a national role in quality and safety improvement associated with PSIRF and will be required to work with a broad range of stakeholders across the system to support appropriate and aligned improvement strategies.
Essential elements of this position include:
Support the planning and delivery of Safety Improvement Programmes, together with stakeholders.
Work jointly with our delivery partners the Health Innovation Network's, 15 regionally based Patient Safety Collaboratives, and ensure programme/project management are aligned with the programmes plans and provide coaching, advice and guidance on quality improvement methodologies and approaches, including measurement for improvement.
Work with Patient Safety Collaboratives to ensure that all reporting is accurate, appropriate and timely and demonstrate the impact and value of the programme, through the assurance process.
