Gladstone House Surgery is a dynamic, friendly practice offering an exciting new opportunity for an enthusiastic and highly motivated, flexible and pro-active Receptionist to join our innovative and progressive team.
Support and mentorship.
You will be supported by our surgery team and have regular training
We are looking for one 25hrs per week and one 35hrs per week post from Monday Friday shifts split between 5 receptionists during the hours 7.30am 6.45pm 5hrs per day on a rotating shift basis. The shifts vary between early, middle and late but are split fairly.

Main duties of the job
The role will involve patient contact both F2F and on the telephone, working in the back admin office and on the reception front desk, along with working with the two reception administrators upstairs. This is a fast paced environment.
You will work closely with the Practices multi-disciplinary team of other GPs, nurses and non-clinical staff to provide a high quality and accessible service to patients.

About us
At Gladstone we have a tight knit team, who pride themselves on working well together, with a strong work ethic and a team who provide mentoring and support on an on-going basis.
This team includes 3 GP Partners, 3 Salaried GPs, 1 x ACP, 3 Nurses and a supportive admin and cleaning team. We are also supported by our PCN and CCG pharmacists, midwife and care co-ordinators.
We pride ourselves on being a patient focused practice.
We also train Yr 1 & 2 medical students, along with student nurses and work experience school leavers.
Free parking for staff is available on site, along with a well-equipped kitchen and staff room area.

We are looking for an enthusiastic and compassionate Health and Wellbeing Coach to join the innovative WE: Flourish team, supporting residents in Kirk Hallam through the exciting SportEngland Place Partnership programme. This role will focus on helping people improve their health, wellbeing, confidence and quality of life through personalised coaching, behaviour change support and increased participation in physical activity and community opportunities.
Main duties of the job
As a Health and Wellbeing Coach, you will:
- Support residents through one-to-one and group-based coaching sessions.
- Help people identify their goals and make sustainable lifestyle changes.
- Encourage participation in physical activity, wellbeing and community activities.
- Build strong relationships with local organisations and community groups.
- Support delivery of community wellbeing initiatives and events.
- Work alongside Social Prescribers, GP practices, VCSE partners and wider health and care services.
- Help tackle health inequalities through personalised, person-centred support.
We're Looking for Someone Who:
- Has experience supporting people to improve their health, wellbeing, confidence or self-management.

Erewash Health Partnership provides core primary care services and support services across its constituent practices and is the prime provider for services delivered on behalf of Erewash Primary Care Network.
WE Flourish is a PCN-wide, non-emergency health and wellbeing service for adults aged 18 and over. This role has a particular place-based focus on Kirk Hallam and the Sport England Place Partnership, while operating through the wider WE: Flourish Single Point of Access rather than as a separate access route.
Job Summary
The Health and Wellbeing Coach will provide Tier 2 coaching and supported intervention for adults registered with an Erewash PCN practice, with a particular focus on residents of Kirk Hallam. The post holder will use personalised, strengths-based coaching to support health, wellbeing, confidence, motivation and self-management, and help people access appropriate lifestyle and physical activity opportunities.
A key focus will be strengthening physical activity for health pathways through the Kirk Hallam Sport England Place Partnership, supporting residents to identify what matters to them, set realistic goals, make sustainable changes and co-produce accessible local opportunities.
The role will contribute to reducing health inequalities and will work within a non-clinical coaching scope, agreed protocols, supervision arrangements and safeguarding, mental health and clinical escalation pathways.
Please note this role is not a clinical role.
General Duties
At all times, the post holder will behave professionally, promote high-quality person-centred support, contribute to a positive team culture and foster continual improvement across WE Flourish, EHP and EPCN.

Support the planning, coordination and delivery of community wellbeing activities, including Wellbeing Mondays or similar local initiatives.
Provide outreach, group activities, community sessions and signposting, and build trusted relationships with residents, community groups and local organisations.
Co-design and co-produce activities with residents and partners in response to local needs.
Share resident insight, lived experience, feedback and delivery learning to support development of the Sport England Place Partnership and wider place-based approaches.
WE Flourish Pathway, Case Management and Governance
Deliver support allocated through the WE: Flourish Single Point of Access. The Kirk Hallam project will not operate as a separate referral route.
Complete a structured Tier 3 Quality Conversation when allocated, competent and appropriately supervised, using the dedicated SystmOne template.
Contribute to Tier 3 support plans, case holding and multidisciplinary meeting activity under the oversight of the Health and Wellbeing Practitioner.
Where acting as case holder, maintain oversight of the persons journey, progress agreed actions, keep the person informed, follow up unresolved referrals and maintain an up-to-date record.
Review open cases at least every two weeks. WE: Flourish will normally case-hold for no longer than three months; support transfer, discharge, return to triage or multidisciplinary discussion where appropriate.
Where needs change, become more complex or the intervention is no longer appropriate, return the case to triage for reassessment and reallocation and document the outcome in SystmOne.
Follow agreed appointment confirmation, non-response, non-attendance, discharge and re-referral processes. Review known safeguarding, vulnerability or significant risk concerns before discharge.
Identify and escalate safeguarding, acute mental health, clinical or urgent concerns through agreed WE: Flourish routes.
Contribute to monitoring activity, outcomes, wellbeing measures, patient feedback, equity of access and learning from the Kirk Hallam work and the wider WE: Flourish service.
Deliver support by telephone, face to face, in PCN or practice settings and at agreed community locations.
Communication and Relationships
Work alongside the WE: Flourish team, Social Prescribing, Health and Wellbeing Workers, practice teams, VCSE organisations, Active Partners Trust, Sport England Place Partnership partners and wider place-based services.

Main duties of the job

Job summary
An exciting opportunity has arisen to join our multidisciplinary team as a Primary Care Network Pharmacy Technician. Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and members of our Practice and PCN multidisciplinary teams.

Main duties of the job
The post holder will work autonomously within their professional scope, using professional judgement and accessing clinical supervision when required.
They will provide pharmaceutical technical support to the medicines optimisation team and programme, undertaking medicines reconciliation, supporting medication reviews, checking medicine administration and promoting effective use of medicines, including inhaler technique. They will support clinical pharmacists with Structured Medication Reviews and help patients get the best from their medicines.
The post holder will improve prescribing quality, safety and cost-effectiveness by supporting agreed medication switches in line with approved protocols, improving repeat prescribing, promoting repeat dispensing and online ordering, reducing clinical risk and minimising medicines waste.
They will undertake clinical audits across the PCN, analyse findings and provide reports to GP practices to highlight progress, identify improvements and support appropriate actions.

Job Scope
Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and other members of the PCN multi- disciplinary team. The post holder is a Pharmacy Technician who acts autonomously within their professional boundaries, with appropriate clinical supervision and support where required. The post holder will provide expert pharmaceutical technical support to support medicines optimisation work programme. They will carry out medicines optimisation tasks, including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation, using their professional judgement and working autonomously within their scope of practice, with access to clinical supervision as required.
The purpose of the role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. The post holder will undertake clinical audits across the Primary Care Network (PCN), analyse and evaluate the findings, and share reports with GP practices to highlight progress, identify areas for improvement, and support the implementation of appropriate actions. Job Scope
Clinical Pharmacy in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver person-centred, coordinated care across the district. The Clinical Pharmacy in General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice.
Pharmacy technicians play an important role, complementing clinical pharmacists, community pharmacists and other members of the PCN multi- disciplinary team. The post holder is a Pharmacy Technician who acts autonomously within their professional boundaries, with appropriate clinical supervision and support where required. The post holder will provide expert pharmaceutical technical support to support medicines optimisation work programme. They will carry out medicines optimisation tasks, including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation, using their professional judgement and working autonomously within their scope of practice, with access to clinical supervision as required.
The purpose of the role is to lead improvements to maximise safe, cost-effective best practice in prescribing to improve the quality of patient care. The post holder will undertake clinical audits across the Primary Care Network (PCN), analyse and evaluate the findings, and share reports with GP practices to highlight progress, identify areas for improvement, and support the implementation of appropriate actions. The post holder will help patients to get the best from their medicines by switching medications to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines.

This role supports reducing health inequalities through proactive case management, care coordination, and structured clinical support within HCA/Nurse Associate competency and delegation frameworks.
The post holder works under the supervision of registered clinicians and is central to coordinating care across PCN multidisciplinary services, ensuring patients receive timely, coordinated, and person-centred care aligned to PCN DES and member practice requirements.
Clinical Governance & Delegation
The post holder undertakes delegated clinical activity in line with NHS England HCA/Nurse Associate competencies and PCN clinical governance arrangements, including:
- Basic clinical observations and structured monitoring
- Supporting long-term condition reviews under clinical protocols
- Supporting vaccination delivery programmes under direction
- Early identification of deterioration in frailty and LTC cohorts
- Escalation of clinical concerns to registered clinicians in line with agreed pathways
All clinical activity is undertaken within:
- Local PCN SOPs
- Clinical supervision arrangements
- Professional competency frameworks HCA / Care Certificate level or above
2. Core Responsibilities PCN DES Aligned
2.1 MDT Coordination EHCH & Anticipatory Care Delivery
In line with PCN DES requirements for MDT working, the post holder will:
- Coordinate and schedule regular MDT meetings for EHCH and high-risk cohorts
- Develop and maintain MDT case lists prioritised using risk stratification tools frailty, admission risk, care home residency, LTC complexity
- Collate and present relevant patient information including:
- Recent primary care interactions
- Secondary care admissions and discharges
- Community health input
- Medication changes
- Vaccination status and care gaps
- Relevant clinical observations where recorded by HCA/Nurse Associate or community teams
- Record MDT decisions, ensuring:
- Clear allocation of actions
- Named responsible professionals
- Agreed timescales
- Monitor completion of MDT actions and escalate delays or clinical risk to the PCN clinical lead
2.2 Personalised Care and Support Planning PCSP
In line with NHSE PCN DES personalised care requirements, the post holder will:
- Maintain and update Personalised Care and Support Plans PCSPs ensuring they are:
- Person-centred and outcome-focused
- Regularly reviewed following MDT discussions
- Updated post-discharge or following change in condition
PCSPs will include:
- Clinical summary GP-led input
- Functional, social, and wellbeing needs
- What matters to me statements
- Advance Care Planning ACP / Treatment Escalation Plans TEP
- Risk stratification frailty, falls risk, admission risk
- Preventative care status including immunisations
- Relevant HCA-contributed observations and monitoring data where applicable
2.3 Enhanced Health in Care Homes EHCH & Preventative Care
In alignment with EHCH DES requirements, the post holder will:
- Maintain oversight of care home and housebound cohorts
- Support proactive care planning for residents in care homes
- Maintain vaccination registers for priority groups:
- Flu
- COVID-19 boosters
- Pneumococcal
- Shingles
- RSV
- Support vaccination programmes through:
- Identification of eligible patients
- Pre-vaccination screening and consent processes within competence
- Coordination of vaccination delivery with PCN clinical teams and providers
- Administration of vaccines within competency
- Support outbreak prevention planning in care homes through timely data provision
2.4 Care Coordination and Navigation PCN DES Personalised Care Model
The post holder will:
- Act as a point of contact for patients requiring care co-ordination support
- Support navigation across health, social care, and voluntary sector services
- Facilitate access to:
- Social prescribing link workers
- Community and voluntary sector services
- Rehabilitation and support services
- Support patients and carers to understand:
- PCSPs

2.5 Structured Clinical Support HCA/Nurse Associate Function within PCN DES Delivery

- Support long-term condition monitoring pathways, including:

- Monitor discharge notifications from secondary care and community settings
- Ensure follow-up actions are coordinated within 7 days of discharge
- Support reconciliation of:
- Medication changes (with pharmacy/clinical teams)
- Care plans and PCSP updates
