Job summary
A new opportunity has arisen for an experienced Clinical Pharmacist to join Glossop PCN. You will work as part of a multidisciplinary team responsible for providing a comprehensive Medicine service, providing appropriate medicine and prescribing information and advice.
This role is pivotal to improving the quality of care and operational efficiencies, so requires motivation and passion to deliver an excellent service within general practice. The pharmacist will provide direct, accessible and timely medicines expertise; including face to face contact with patients and embracing new ways of working including telephone and video consultations.
Main duties of the job
The role involves providing expert pharmaceutical care through patient-centred medication reviews, medicines optimisation and safe prescribing support. The successful candidate will manage patients with long-term conditions, address medication-related concerns, promote self-care and shared decision-making, and support repeat prescribing. As an NMP-qualified professional, they will work collaboratively with multidisciplinary teams to improve patient safety, health outcomes and the quality of services.

Job summary
We are looking to recruit a dedicated and experienced Paramedic to join our PCN based in Glossop for 30 hours a week.
The successful candidate will work flexibly as part of a multidisciplinary team to assess, diagnose, and treat patients presenting with a wide range of minor illnesses and urgent care needs in clinics across our four practices. You will play a key role in improving access to clinical services, supporting chronic disease management, and contributing to the overall health and wellbeing of the local patient population.

Main duties of the job
The Paramedic will act as a specialist clinical resource, working autonomously and flexibly within the PCN practices to assess, diagnose, treat, refer, or signpost patients presenting with undifferentiated or undiagnosed conditions related to minor illness or injury. Collaborating closely with the wider multidisciplinary team, the role involves delivering holistic, patient-centred care, supporting self-care and informed decision-making.
The post holder will also be required to carry out home visits as and when required and must have access to a vehicle.
