We have an exciting opportunity for aGP/ Specialty Doctor,to join the Wiltshire Hospital at Home (Virtual Ward) service. This is a senior clinical role delivering medical assessment, decisionmaking and leadership for patients receiving acute, hospitallevel care in their own homes.

The service is undergoing an exciting period of transformation and expansion, making this an excellent time to join a dynamic and innovative multidisciplinary team focused on admission avoidance, early supported discharge and personcentred frailty care.

The post holder will provide clinical assessment and management of patients, support the multidisciplinary team in delivering Comprehensive Geriatric Assessment (CGA), and promote safe and timely discharge through close collaboration with MDT colleagues, patients and their families.

Applicants should have a background in Geriatric Medicine, Acute Medicine, General Medicine or General Practice, with experience working with patients living with frailty and complex comorbidity.

The Hospital at Home medical establishment is4.3 WTE, deployed across Wiltshire to meet population need:

Job Introduction

We are seeking a dedicated and reliable Pharmacy Technician to join our healthcare team within the HMP Chelmsford prison. This role offers the opportunity to contribute to the delivery of pharmaceutical services in a secure and structured environment. Job Introduction

Working as a Pharmacy Team Leader you will provide high quality care and medication treatment to patients with clinical indications in a prison setting. We have an exciting opportunity for a bank Specialty Doctor/ GP to join ourHospital at Home service. The service is currently undergoing an exciting period of transformation, making this an excellent time to become part of a dynamic and innovative team. Post will be based at either Chippenham, Salisbury, Peasedown, Chippenham, Devizes or Swindon (with some remote working).

The successful candidate will provide clinical assessment and management of patients, support the multidisciplinary team in delivering Comprehensive Geriatric Assessment, and promote safe and timely discharge through close collaboration with the MDT, patients, and their families.

The role is comprised of 2 x 9-month rotations.

• Inpatient Stroke Rehabilitation on Runfold ward, with some additional care of the elderly/neuro rehabilitation

Package Description: Are You Ready to Train as a Health Visitor?

HCRG Care Group is excited to offer funded SCPHN (Specialist Community Public Health Nursing) training posts for the course starting in January 2027.

We are looking for dedicated, reflective, and dynamic Nurses or Midwives who are passionate about making a real difference to children, families, and communities across Surrey.

This year we have 6 Health Visitor SCPHN student posts available across Surrey.

The course is delivered exclusively through Oxford Brookes University and leads to a Master's level qualification.

The SCPHN programme at Oxford Brookes University is a Master's level course (PG Dip / MSc) and is available on both a full-time and part-time basis. The course combines online distance learning with supervised practice placements:

• Online distance learning - 2 days per week (theoretical and academic component)
• Practice placement - 3 days per week (clinical learning and hands-on experience)
• The 50/50 split of theory and practice ensures you develop both academic and clinical skills
• Upon completion you will be registered as a SCPHN and eligible to apply for Band 6 Health Visitor role

To be considered for a student post you must meet the following criteria:

Essential:

• Registered with the NMC as a Nurse or Midwife
• UK resident
• Demonstrate either: achievement of an undergraduate degree OR completion of a Diploma in Nursing
• A full UK driving licence is required

Desirable:

• Completion of a recent Level 6 (or above) module
