Our service also delivers TB healthcare at HMP Wormwood Scrubs, offering you a truly varied and impactful role.

Main duties of the job
- Manage a designated caseload of adult and paediatric patients.
- Collaborate with the UK Health Security Agency (UKHSA), pan London Find & Treat services, and other TB services across London.
- Deliver safe, effective, and compassionate care aligned with NICE guidelines and the National Collaborative TB Strategy.
- Promote public health and raise TB awareness within both clinical settings and the wider community.
- You will provide specialist nursing expertise to ensure the delivery of high-quality, person-centred care for patients with suspected or confirmed TB, whilst supporting their families and carers. Working both autonomously and as an integral member of a multi-disciplinary team, you will contribute to achieving local and national TB targets through effective case management.
- Your role will involve overseeing the care of a designated caseload, including contact tracing, screening, and participation in Cohort Review
- This is a varied role requiring flexibility, including community-based work such as home, school, or workplace visits to carry out assessments, phlebotomy, and Directly Observed Therapy (DOT).
- You will also play an active part in staff education and service improvement, contributing to audits, supporting team development, and delivering ward-based teaching. In collaboration with senior nursing colleagues, you will be involved in initiatives that enhance the TB service and improve outcomes across our diverse patient population.

Job summary
We are looking for an enthusiastic and motivated Band 5 Physiotherapist to join our 6 monthly rotation scheme. Our band 5 rotation scheme includes rotations at the three main hospital sites, St Mary's Hospital, Charing Cross Hospital and Hammersmith Hospital across a wide variety of areas, including Stroke, Respiratory, Community, Medicine, Neurosurgery, Musculoskeletal, Paediatrics, Neuro Rehabilitation & Orthopaedics. Band 5's will be expected to complete rotations in the core areas of Physiotherapy; Medicine and Respiratory. Potential for substantive and fixed term contract opportunities available.
You would be joining a supportive, well established team committed to providing a high standard of clinical care. The department actively supports Continuing Professional Development and runs an established appraisal system and a comprehensive in-service training programme.
The successful applicants must have their degree qualification and should be HCPC registered or have submitted their registration forms already.
The physiotherapy department operates as a 7 day service and therefore this post will require working at weekends as part of contracted hours of work. This post will also support the current overnight on-call service (subject to competencies).

Main duties of the job
- To manage own caseload of patients as an autonomous practitioner with appropriate clinical supervision.

Job summary
We are seeking an enthusiastic Specialist Clinical Psychologist keen to develop a career in neuropsychology, to provide 12 months' parental leave cover.
This post offers an excellent foundation for developing specialist neuropsychology skills within the friendly and supportive Clinical Health Psychology and Neuropsychology Department at Charing Cross Hospital, part of Imperial College Healthcare NHS Trust and including the Northwest London Regional Neurosciences Centre.
The role includes specialist neuropsychological assessment and intervention within our outpatient service, which receives referrals from a wide range of neurology and neurosurgery clinics including cognitive disorders and dementia, traumatic brain injury, movement disorders, epilepsy, multiple sclerosis, neuro-oncology, infectious diseases and general neurology. There are also opportunities to contribute to exciting service development initiatives. Weekly supervision is provided by a senior Clinical Neuropsychologist.
Main duties of the job
The post holder will provide comprehensive neuropsychological assessment and formulation for patients presenting with a wide range of neurological disorders referred from specialist neurology and neurosurgery clinics. The role requires experience of neuropsychological assessment, a high level of attention to detail and an ability to assimilate information from a range of sources to arrive at a neuropsychological formulation of a patient's current difficulties and presentation.
The post holder will also contribute to the delivery of a specialist intervention pathway providing psychological interventions for patients presenting with mental health difficulties associated with Parkinson's disease, as well as supporting the development of new evidence-based intervention pathways for other groups of neurological patients.
In addition to direct clinical assessment and intervention, the post provides opportunities for involvement in service development, staff consultation and training, and research and audit.

We are looking for an enthusiastic professional who can help the Community Independence Service (CIS) make Hammersmith & Fulham (H&F) a great place to grow old. Join us and be part of a committed and caring team as we deliver high quality patient care.
Your role as a Health Care Assistant (HCA) within the CIS Home First Team will involve providing a rapid response care service for people who need urgent help with primary acute physical health illness in their own homes, often immediately after a hospital admission. Please note that this role is not an inpatient ward based or mental health service, and the vast majority of our patients need support with functional activities.
As a HCA you will be expected to provide support to our patients in a wide range of community and patient home environments, working autonomously and collaboratively in supporting the integrated multidisciplinary team in the delivery of excellent patient care. You will be responsible for the holistic, comprehensive care of complex community patients, delivering clinical care delegated by a registered professional and contributing to the evaluation of person centred care plans.
You will have excellent communication skills, IT skills, and the ability to be adaptable, flexible and reliable as changes to your caseload and patient visits may happen on a daily basis.
The service is operational 7 days a week 8am to 8pm. Hours of work for HCA may vary between these hours.
Main duties of the job
Key Responsibilities:
Carry out day to day care of patients within their own home environment, such as personal care, medication prompting, toileting and meal preparation.
Carry out safe moving and handling of patients with the use of equipment prescribed by Occupational Therapists and Physiotherapists, for example rollator frame, sara steady, re-turn and hoist.
Review and adjust basic equipment that has been prescribed and installed by Occupational Therapists or Physiotherapists, for example mobility aids or toilet frames.
To be able to take observations (for example blood pressure and temperature) after completing the appropriate competency, NEWS2 scoring and early identification and escalation of the deteriorating patient.
Participate and contribute to audits, service development and projects associated with improving the quality of service of CIS.

The CEN Service is a specialist Tier 3 community service supporting adults experiencing complex emotional needs, including emotional instability, relationship difficulties and difficulties managing distress.
The role is open to professionals from Social Work, Nursing and Occupational Therapy backgrounds. The Hounslow Community Neuro Rehab Team (CNRT) is a client-centred, integrated multi-disciplinary team comprising of an Advanced Clinical Practitioner, Physiotherapists, Occupational Therapists, a Neuro Psychologist, a Pharmacist, a Specialist Nurse practitioner, an Occupational Therapy Assistant and Rehab assistants.
The Neuro-rehab team works across both intensive short-term rehab and longer-term rehab pathways. There are opportunities to develop skills in rapid assessment and facilitating discharge from hospital, long term condition management and specialist neuro-rehab.
This role provides the opportunity to work across both rehabilitation pathways within the service, with both Occupational Therapists and Physiotherapists. This role involves providing therapy for patients within their own home environments as well as the local community. There will be expectations for lone working and independent monitoring of patients progress with timely feedback to the MDT.

Job summary
We are seeking a passionate Clinical, Forensic or Counselling Psychologist to join the inpatient team at West London Forensic Service (WLFS). The post is 1.0 WTE for 1-year fixed term and will be based on a male low secure complex rehabilitation ward. It can be appointed at Band 7 or Band 8a depending on experience and with responsibilities adjusted accordingly. For Band 7, this will be a preceptorship towards an 8a role.
The successful candidate will be responsible for the provision of a highly developed and comprehensive specialist psychology service to culturally diverse service-users with enduring mental health difficulties and offending histories.
We provide a comprehensive induction and ongoing training and development, with opportunities to develop specialist skills and receive specialist supervision (e.g. EMDR and schema therapy), in addition to regular supervision. There are excellent staff benefits including a generous annual leave allowance and an on site gym (with membership at just £10 per month).
Main duties of the job
You will be responsible for providing a high quality psychology service on a low secure complex rehabilitation ward. As a core member of the MDT, you will be responsible for developing and implementing psychologically informed treatments for male service users who present with enduring mental disorders (both mental illness and personality disorder) and an offending history. As such you will be able to utilise varied skills in assessment, formulation and evidence-based intervention, tailored to individual needs.
As well as individual ward-based work, you will contribute to the cross-ward group therapy programme. You will also advise on risk assessment and risk management, and support psychological practice within the team through consultation and training.
In addition to direct clinical work, the role may involve staff supervision, service development and audit, and collaborative working within the psychological therapies department. There are also opportunities for research within WLFS.

Main duties of the job
To provide high quality, skilled nursing care and to assist the Community Nursing Team Manager in the on-going planning, implementation, and evaluation of the holistic needs of the patient/client, family, and/or carer.
Deliver high quality nursing care to housebound patients with complex needs to enable them to remain in their own homes. Working closely with out of hours service providers, specialist nurses, allied health and social care professionals to meet the needs of the patients without unnecessary attendance in A&E or be admitted to hospital.
This will involve working independently but as part of the Community Nursing Team covering a particular geographical area. This will include first assessments and reviews, planning, implementation, monitoring, and evaluation of individual programmes of care. Job summary
Following an internal promotion, an opportunity has arisen for an experienced Physiotherapist with an interest in specialist medicine to join our integrated therapy team at Charing Cross Hospital.
The post holder will predominately be based on one of our 8th floor wards where they will help lead the team seeing a range of patients requiring therapy intervention prior to returning home or transfer to community rehab units. There will be a range of ages, conditions, co-morbidities and complex social situations to manage and the post holder will form an integral part of the multidisciplinary team, helping plan timely and safe discharges from the wards.

In addition, you will have supervisory responsibility for a rotational Band 5 Physio and will work closely with a Therapy Support Worker. Supervisory support and advice is always available.

Main duties of the job
- The caseload will include adult patients presenting with a varied range of acute and chronic medical illnesses, falls, cognitive limitations, neurological impairments and complex psycho social issues.

West London Mind is looking for an experienced and compassionate Lead MHST Supervisor to join our long-standing Mental Health Support Team (MHST).

The Dementia Link Service provides targeted care and support for people with dementia and their carers, and provides a link between primary and secondary care teams.

The West London NHS Trust (WLT) Mental Health Single Point of Access (MH SPA) and NHS111 Option 2 for Mental Health (NHS111*2) provides 24/7 access to mental health services, every day of the year. Both services support people in a mental health crisis and provide a telephone support line for service users, their families and wider support networks. This includes a triage, signposting and referral pathway to appropriate onward care, if indicated. The post is cross site and will be based at Hammersmith or Charing Cross site and will require the post holder to organise and participate in the preparation of chemotherapy, clinical trials and parenteral nutrition for adults.

Main duties of the job
- To participate in the cleaning and preparation of the Aseptic Unit and its equipment and supplies ready for the compounding of sterile products in accordance with the principals of Good Manufacturing Practice (GMP).
- To undertake the compounding of sterile products and their labelling and packaging.
- To help ensure that service provision is flexible and responsive to clients/patient's needs.
- To ensure that all stock transactions are appropriately recorded on the pharmacy computer system and to help ensure the aseptic unit stock holdings are accurate and appropriate.
- To answer the telephone, assess the significance of the caller/content, then take appropriate actions/ refer to senior staff.
- In-depth and extensive in-house training and accreditation will be provided in all areas to enable fulfilment of the above roles.

Main duties of the job
The post holder will work with the qualified Occupational Therapists in the services delivering assessment and treatment sessions. The Occupational Therapy Assistant see patients on and off the ward. The sessions will generally be group sessions but could at times be one to one sessions.

An exciting opportunity has arisen for Band 7 Community Cardiac Nurse post to work across our community out-patients Cardiac Services Service. Community sites are based in Ealing, Hammersmith & Fulham, Maida Vale and Ladbroke Grove.
The post holder will be a highly experienced Cardiac Nurse to help support triaging of referrals coming into the service, support staff when there are 'unwell patients' in clinic and manage Band 6 and Band 3 Clinical staff.
The post holder is required to have a substantial cardiac nursing background to competently manage these referrals and help risk stratify patients into clinic. The post holder will also need to have good problem solving and inquiry skills to manage referrals. The Post holder will be competent at interpreting 12 Lead ECGs, 24hr ECGs, Echo and 24hr ABPM (Ambulatory Blood Pressure Monitoring) .
The post holder will need to be able to use their previous knowledge in assessing referrals and liaise, where necessary, with the GP's and Cardiologists around referrals into the service. The post holder will be expected to work autonomously but seek advice/support where applicable. Along with referrals the post holder will help to manage a GP help line.
Main duties of the job
- The post holder will work as a senior member of the community Cardiology multidisciplinary team, supporting the safe and effective delivery and management of cardiology services across our community sites.
- The role requires a high level of autonomous clinical practice, including vetting and assessing GP referrals, reviewing and interpreting diagnostic investigations, determining clinical urgency and appropriately triaging patients into community cardiology pathways.
- The successful candidate will provide specialist clinical advice and support to patients, GP's and multidisciplinary colleagues.
- They will provide senior clinical leadership across the service, supporting and developing nursing and support staff, promoting evidence-based practice and acting as a professional role model.
- The post holder will contribute to clinical governance, quality improvement, audit, research and service development, identifying and managing clinical risk and maintaining high standards of patient care.
- They will work collaboratively with the Consultant Cardiologists, primary care and the wider MDT ad support operational management of the Community Cardiology Service.

An exciting opportunity has come up for the Adult Community Nursing Service, Night Nursing. We are currently looking for a skilled person who can do 21 hours a week (1 overnight shift- 11 hours and 2 twilight shifts- 10 hours) to work across the Hounslow locality based at Clayponds Hospital on a Fixed Term Contract for 6 months to cover maternity. The service operates from 19:00 until 07:00 seven days a week. The service has 2 shifts, 19:00 until 12 midnight (Twilight) and 19:00 until 7:00 AM (Overnight).
Please note that the service is currently undergoing service re-design and the operational hours will change by 7th December 2026 from 7pm-7am to 8pm-8am under phase 1 transformation project. The phase 2 (TBC) will be happening shortly after the implantation of phase 1 with the Night Service re-design involving the twilight and the overnight shifts. We are looking for someone who will be very flexible in supporting the new service structure in the future.
The service also delivers urgent community response between 20:00 and 22:00 as part of the service specification and to prevent hospital admission.
Main duties of the job
To carry out clinical duties and be prepared to undertake clinical skills as per needs of the service and specific patient after training and competence assessment with support from an identified mentor for example, venepuncture, Blood Pressure, pulse and temperature recording, urinalysis, blood glucose monitoring, simple wound dressings and non-complex tasks such as continence reviews.
To undertake basic duties as delegated and overseen by the registered nurse and actively report any changes in a patient's condition in a timely way to the registered nurse.
Liaise with other health care and multi-agency workers to provide a multi-disciplinary approach to care.
To communicate with patients at a time and level appropriate to their needs in a manner that promotes positive health.

The Community Recovery service is an integrated health and social care service for adults identified with uni and multi-disciplinary needs for recovery. The service will support independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances. It aims to restore and maximise function or prevent deterioration in physical, psychological and social functioning through episodes of rehabilitation and reablement.
Therapists provide support and rehabilitation in the home.
The Community Recovery Service provides therapy for various pathways including: Core/ general rehabilitation, Discharge to Assess Reablement (CRS+) Integrated discharge hub at west Middlesex hospital.All health and social care professionals within the Community Recovery Service will have a set of core skills that all staff in the team are able to deliver regardless of profession. This supports the single assessment process, maximises use of resources and minimises duplication.
In addition, team members will retain specialist skills and knowledge which will be utilised as required to support specific clinical pathways. Specialists will also support other team members' work to ensure fulfilment of relevant goals.
Main duties of the job
The successful candidate will act as a specialist clinician, participating as a member of the team while independently managing a specialist clinical caseload.
Manage and prioritise a professional caseload within the interdisciplinary service through efficient workload management, supervision and adjustment of service provision to meet changing priorities and ensure individual patients receive intervention timeously and appropriately.
They will provide high quality comprehensive Physiotherapy assessment and treatment to adults referred to the Hounslow Community Recovery Service.
Develop and deliver an individualised treatment programmes based on a problem solving and advanced clinical reasoning approach to the patients referred and monitor, evaluate and modify treatment to measure progress and ensure effectiveness of intervention using valid outcome measures.
Work closely with the multi-agency team to meet the needs of the patients and enable them to receive a comprehensive service.

On 1st April 2022, paediatric and child health services across Chelsea & Westminster NHS Foundation Trust and Imperial College Healthcare NHS Trust came together under a single and integrated governance and leadership structure creating West London Children's Healthcare (WLCH).
We have an ambitious vision to improve the health and wellbeing of children and young people across West London through collaboration of clinical teams and by building stronger, equitable common care pathways with close links to research and education colleagues at Imperial College.
WLCH runs paediatric services from four sites, St Mary's Hospital (SMH), Chelsea & Westminster Hospital, West Middlesex and Hammersmith Hospital. In addition, we are working in partnership with other acute providers in NWL and with our partners in community, mental health, local borough and voluntary sector services to improve care for all children and young people
WLCH are delighted to offer this 8PA substantive post to join our ambitious and highly specialised Paediatric Allergy Team at St Mary's Hospital. This post is suitable for those fully registered with the GMC with a licence to practice who hold a CCT in Paediatric Allergy or for those whom will be within 6 months of CCT or equivalent at the interview date.
Main duties of the job
This post offers the opportunity to deliver complex allergy care and to help shape the future of the service by delivering innovative therapeutic approaches within the clinical and research MDT. Imperial is the Lead Allergy Service for the North West London Allergy Network caring for over 5500 children with allergy and their families. The post offers the opportunity to contribute to the care of children with allergy across the network through shared learning and by modelling of excellent care.
The postholder will be expected to undertake clinical activity including General and Complex Paediatric Allergy Clinics, Baby clinic and participate in support and supervision of Paediatric Allergy Day unit activity. The successful applicant will show that they will be calm and effective in emergency scenarios, holding a valid APLS or equivalent certificate.
The postholder will actively contribute to weekly MDT meetings including Complex Allery MDT.
Formal and informal education and supervision of resident doctors and education and support of medical students and MDT members of allied specialities is an important part of the role. The successful applicant will bring an open, warm and collaborative approach to teaching and supervision in these areas.

The Mental Health Single Point of Access is at the centre of urgent and crisis mental health care across Ealing, Hammersmith & Fulham and Hounslow. Alongside our 24/7 SPA, the team also delivers NHS 111*2 mental health provision and Mental Health Urgent Community Response, creating a unique service where senior clinicians can work across telephone triage, urgent response, crisis pathways and system-wide decision-making.

This is a role for someone who wants more than a traditional clinical post. You will influence pathways, lead practice across a fast-moving service and play a key role in the continued development of urgent and crisis mental health care
Main duties of the job
As a Band 7 Senior Clinician, you will be one of the key clinical leaders within SPA, NHS 111*2 and MH UCR. You will provide senior oversight of complex referrals, telephone triage, urgent and crisis presentations and professional-to-professional contacts, supporting the team to make safe, confident and proportionate decisions.
You will lead the service clinically and operationally on shift, providing immediate support with risk, escalation and complex pathway decisions while maintaining oversight of service pressures and patient flow.
You will supervise and develop staff, contribute to quality improvement, audit and performance and work closely with CATT, primary care, ambulance services, police and wider partners. You will have the opportunity to influence practice, develop new ways of working and help shape a service that sits at the heart of urgent mental health care across three boroughs.
