Job summary
The post holder will provide senior specialist PSIRF, clinical quality and patient safety leadership, advice and expertise, and will lead on all PSIRF and patient safety related activities; as well as playing a key role in providing advice and support to the Hounslow Mental Health Borough, ensuring a seamless PSIRF quality response for the areas that they oversee.
The post holder will also provide leadership and oversight for all patient experience, audit, inquest and regulatory functions undertaken within the borough and / or service(s).
Main duties of the job
To support the Trust and borough(s) / service(s) regarding: PSIRF, patient safety, clinical assurance, PSIs, formal complaints, patient feedback and all associated learning.
To provide specialist expertise and oversight whilst contributing to delivering the Trust-wide safety agenda and achieving the Trust quality priorities.
To support the borough director, service managers and staff to discharge their responsibilities in the delivery of PSIRF and other safety related issues, including patient experience, quality assurance and meeting the required statutory and mandatory quality and governance requirements.Contribute to, and lead, on the development of quality, safety and patient experience reports for regular and ad hoc meetings.
To advise the Borough Director on all PSIRF and safety issues, as well as wider clinical quality issues that affect the borough(s) / service(s) they are supporting.

Job summary
As a Bank Band 6 MSK physiotherapist in the HCH MSK Physiotherapy team you will have the opportunity to work with a dynamic team of highly experienced physiotherapists, in a fast-paced environment providing high-quality physiotherapy care to the borough of Hounslow. Our large team consists of a physiotherapy assistant, band 5, 6, 7 physiotherapists, Band 8a Advanced Physiotherapy Practitioners and a Physiotherapy Consultant. Bank staff will be supervised by a senior physiotherapist during their time in post. You will be integrated into the team which is focussed on delivering high quality care at the right time and place, in our main clinic based in Isleworth and at outreach clinics in GP practises.
As the successful candidate you will have excellent communication and interpersonal skills to quickly integrate into the team and build rapport with patients. Highly developed organisational skills are required able to manage your own caseload and work effectively within a team. Strong ability to prioritise and make sound decisions, sometimes under pressure, is required. You will carry a varied caseload of musculoskeletal complaints, often complex, with a view to utilise all treatment and care options as appropriate, with the support of a named supervisor.
Main duties of the job
To carry out effective and appropriate assessments using knowledge and skills combined with clinical reasoning and to provide a accurate diagnosis.
Work with patients to identify and agree treatment goals delivering patient centred care based on assessment out-comes, which demonstrate a good knowledge of the complex interaction of risk, safety, patient choice, shared decision making, independence, areas of conflict and eligibility for the services
Use communication, reasoning and negotiation skills to establish a working partnership with patients, families and carers identifying barriers to communication
Plan and lead patient group sessions as clinically relevant
Address all risk areas associated with individual patient's ability to function
Contribute to effective discharge planning
Monitor and evaluate effectiveness of interventions/treatments using outcome measures
Represent the Physiotherapy Service within the multidisciplinary team and participate in relevant team/network meetings, clinics and to contribute to the discussion and planning of patient care.
Work with assistants and technical instructors regarding clinical caseloads and provide necessary support on clinical issues

Link Work will be a key specialism within MINT, overseeing recovery focused interventions that make use of community assets as well as those of the multi-disciplinary team.
Main duties of the job
You will play a vital role in connecting young people who are experiencing a challenge with their mental health with community supports and resources. As a Link Worker you will specialise in signposting to and supporting with referrals to identified appropriate community resources, fostering autonomy and promoting independence, building on the individual's strengths and aiding in gaining skills and confidence in collaboratively identified areas of support, as well as escalating concerns should they arise.
You will be comfortable working both on your own and with others from a range of different backgrounds. You'll need to be resilient, motivated, and committed to adapt to working in an ever-changing environment.
As the role develops, you will also spend a portion of your time working closely with CAMHS to support young individuals to access vital services in their community and ensure a smooth transition into adult services

Job summary
This is an exciting opportunity to lead an innovative new integrated community mental health service. You will play an integral role in further developing the service, teaching and training new team members and primary care staff and providing clinical and managerial leadership to the team.
The service is continually developing, and the configuration of staff and exact details of the post may change in order to facilitate this. We are advertising for two 6PA vacancies which are due to a combination of the existing post holder transferred to our community rehabilitation service and a historical vacancy caused by the previous consultant leaving the Trust to take up a Chief Medical Officer position. At present there is a plan for movement within the service to fill the other 6PA role in this team.
Main duties of the job
You will have a key role in a liaison/consultation model to provide early, easy access advice to GPs and the Primary Care Networks to support care of patients in the locality. GPs in the borough will be able to contact you via telephone or email for advice during working hours. You will also provide clinical leadership to the team in clinical multi-disciplinary team meetings and referrals meetings as well as being available for case discussions, advice and supervision within and outside team meetings. You will also have a role in the management of more complex cases, to offer senior assessments and more direct input in particular in risk and/or medication management and diagnostic assessments. This will also include Mental Health Act Assessments, CTO reviews and supporting the duty function of the team with urgent concerns and assessments. You will also share senior medical responsibility with job sharing consultant for the care carried out by the MINT team.

We are seeking a compassionate and organised Band 6 Kaleidoscope Nurse to help children receive safe, high-quality care in their own homes across Kensington and Chelsea, Hammersmith and Fulham, and Westminster.
Working with the Kaleidoscope Palliative Care Team, Children's Community Nursing Team and Social Services, you will coordinate home-care packages, support children receiving end-of-life care and help families navigate care with confidence.
You will work autonomously as part of a supportive multidisciplinary service and deputise for the Nurse Specialist to maintain continuity of care.
Applicants must be a Registered Nurse with current UK NMC registration, have relevant nursing experience at Band 5 and evidence of post-registration education. The role is based at Woodfield Road Medical Centre and requires travel, with participation in 24-hour on-call advice and visits for children receiving end-of-life care at home.
Main duties of the job
- Coordinate the Kaleidoscope service, including allocating Support Workers and arranging bank nurses or care workers when required.
- Receive and assess requests for home care, determine eligibility and refer families to appropriate services where needed.
- Complete risk assessments in community settings before care begins and respond to identified risks.
- Work with community nursing colleagues to arrange, check and maintain essential equipment in the home.
- Provide 24-hour on-call advice and visits for children receiving end-of-life care at home, alongside the Children's Community Nursing Team.
- Work with social workers, commissioners and families to establish and maintain safe packages of care.
- Support the transition of care packages from children's to adult services.
- Develop and review evidence-based protocols, training resources, assessment tools and home-care documentation.
- Maintain accurate clinical records and service data, including monitoring SystemOne recording.
- Participate in professional, discharge-planning and service meetings, using interpreters where required.

An exciting opportunity has arisen to join the West London Forensic Service in Ealing as part of our well-established Systemic Family Work Team. The team provides systemic psychotherapy to service users with complex mental health needs across medium and low secure services and within the community.
We are seeking a qualified Family and Systemic Psychotherapist to join us on a full-time basis, although part-time hours will be considered. The role offers excellent CPD opportunities alongside involvement in research, teaching, consultation, and supervision within the Psychological Therapies Department.
Appointments may be made at Band 7 or Band 8a, depending on experience. Candidates appointed at Band 7 will be supported through a structured preceptorship programme with a clear pathway to Band 8a progression.

As a Family and Systemic Psychotherapist, you will play a key role in supporting recovery, resilience and lasting positive change.
You will work closely with colleagues in our Reflecting Team, using a collaborative and compassionate approach to support families through complex and emotionally demanding situations. The role provides excellent opportunities to develop specialist systemic practice, with regular supervision, peer support and reflective practice.
You will also work in partnership with multidisciplinary teams across the forensic service, providing expert consultation, formulation and systemic interventions at individual, family and team levels. This is a new consultant post created to provide medical leadership to Hammersmith & Fulham's new Complex Psychosis Team, which brings together the existing Community Rehabilitation Service (CRS) and a new Assertive Outreach (AO) function under a single clinical and operational structure, each delivered as a distinct "arm" of one team with a single Service Manager. The post holder will hold overall medical responsibility for both arms, working closely with a 0.5 WTE Specialty (SAS) Doctor whose recruitment is running in parallel with this post.
Because the AO function is a new, NHS England-mandated service still in its mobilisation phase, this post carries a genuine and time-limited service development component: helping to finalise the tri-borough clinical model, establishing pathways and standard operating procedures, and shaping caseload and workforce arrangements as the service beds in. This is reflected explicitly in the job plan (Section 9)
Main duties of the job
Direct clinical care across the CRS and AO arms: new assessments, ongoing reviews, medication management, CTO reviews, and Mental Health Act assessments as required.
Assertive, community-based contact with the AO cohort, including home visits and joint visits with care co-ordinators, reflecting the AO model's emphasis on engagement as a therapeutic intervention.
Weekly multidisciplinary team meeting for the Complex Psychosis Team (both arms), plus daily flagging/triage of urgent concerns.
Support to the team's duty function for urgent risk and safeguarding concerns.
The post holder will be an Approved Clinician under the Mental Health Act and to hold Section 12 approval, consistent with the clinical responsibilities of this role.
No dedicated outpatient clinic ratio applies in the traditional sense; clinical contact is organised around MDT review clinics, home/community visits and duty cover, as set out in the job plan (Section 9). There are no inpatient beds or day-patient services attached to this post.
Be professionally accountable to the Clinical Director - H&F and the H&F Clinical Lead for Community Mental Health Services, and operationally accountable to the Clinical Director - H&F and the Service Manager for the Complex Psychosis Team.
Be a member of the H&F Medical Advisory Committee (MAC), contributing to operational and strategic matters and to discussion of major service developments.

An exciting opportunity has arisen for a motivated and enthusiastic Band 6 Community Psychiatric Nurse to join our MINT Team on a 6-month secondment. The team provides community mental health support to adults across the London Borough of Hounslow, supporting service users with mental health difficulties and associated physical health needs.

We are seeking to recruit enthusiastic, motivated, full time band 7 sonographers to join our busy, friendly Ultrasound team at Imperial College Healthcare NHS Trust. We provide a comprehensive ultrasound service to a wide range of specialities within the hospital and local GPs.
At Hammersmith Hospital, we perform a wide range of ultrasound examinations including general abdominal, gynaecological, transplant kidneys, small parts, venous and carotid Doppler and contrast enhanced microbubble scans.
At Charing Cross Hospital, we perform general abdominal, gynaecological, small parts, Testes, including Head & Neck, venous Doppler, MSK and contrast enhanced microbubble scans.
At St Mary's Hospital, we perform general abdominal, gynaecological, small parts, MSK and early pregnancy scans. We also have a busy recurrent miscarriage clinic where we provide ultrasound services to investigate for congenital uterine anomalies using 3D TV scan.
Posts are available at Hammersmith Hospital, Charing Cross Hospital and St Mary's Hospital. Sonographers are not required to rotate between sites and current working hours are Monday to Friday, 08:30 to 17:00 but this may change in the future to provide 7-day service.
All three departments are excellent to work in, where further training and development is supported. You will be expected to work independently, however support from other sonographers and radiologists is always available and encouraged.
Main duties of the job
- To provide an efficient high quality ultrasound service comprising unsupervised scanning sessions within the trust maintaining a high standard of patient care.
- To work as an independent practitioner by performing ultrasound scans to a high diagnostic standard, formulating a report and communicating the results to the appropriate clinician and to accountable for own professional actions.
- Provide specialist advice pertaining to the ultrasound service.
- Responsible for training and supervision of all staff within the ultrasound unit.
- To be actively involved and assist with the department Audit, Research and Clinical Governance programme.

Main duties of the job
You will hold a caseload of children and adolescents with mild to moderate emotional and mental health difficulties and will deliver evidence-based and time limited 1:1 and group interventions, employing the principles underpinning the national CYP-IAPT programme (Children and Young Peoples Improving Access to Psychological Therapies). Low intensity interventions (e.g. CBT guided self-help) will be delivered in a range of education settings (primary, secondary, special, alternative provision and sixth form and further education colleges). Work will include psychoeducation sessions to a range of stakeholders, including children and young people, parents/carers and education staff, promoting a whole school approach to mental health. The role runs throughout the year, and you will be part of our holiday provision, ensuring we can support children all year round.
You will work with people from diverse backgrounds, using interpreters when necessary, and will have a commitment to challenging health inequities, and increasing access to our services for all young people. The post holders will work as part of a multi-disciplinary team, holding a clinical caseload and undertaking the comprehensive assessment of children and young people with mental health needs (including risk assessment and the development of risk management plans) who present in crisis to CAMH Services, hospitals and sites covered by the Alliance Crisis Care.
In addition to providing direct work to families and young people, the post holders will also assist the CAMHS and/or paediatrics team with treatment planning and provide consultation to families/carers and other professionals involved in the life of the young person.
The post holder will participate in delivering the provision of specialist mental health services to children and young people and their families and carers across the West London NHS Trust 3 Boroughs CAMH Service and crisis pathway.

The post holders will work as part of a multi-disciplinary team, holding a clinical caseload and undertaking the comprehensive assessment of children and young people with mental health needs (including risk assessment and the development of risk management plans) who present in crisis to CAMH Services, hospitals and sites covered by the Alliance Crisis Care.
In addition to providing direct work to families and young people, the post holders will also assist the CAMHS and/or paediatrics team with treatment planning and provide consultation to families/carers and other professionals involved in the life of the young person.
The post holder will participate in delivering the provision of specialist mental health services to children and young people and their families and carers across the West London NHS Trust 3 Boroughs CAMH Service and crisis pathway.

This is an exciting opportunity to join a trauma-informed social care service within London Borough of Ealing, who are commissioning this service in partnership with West London NHS Trust. We are looking to recruit a full-time Band 7 Clinical Psychologist to join our Brighter Futures Clinical Psychology service across our Child Protection and Child in Need teams. 'Brighter Futures' in Ealing is a practice model across children's services which draws upon mentalization-based approaches, attachment and trauma-informed care approaches to centralise relationships as key to supporting children and families.
The postholder will be embedded across social care teams to work with children, young people and parents/families referred to the local authority following concerns around safeguarding and those supported under Child in Need (CIN) and Child Protection (CP) processes. The post holder will be supported and supervised by a Highly Specialist Clinical Psychologist within the Brighter Futures service. There is also a large and supportive Child Clinical Psychology Group. We are able to provide varied opportunities for professional development and training. Main duties of the job
The clinical psychologist will be expected to provide psychological consultation to social workers and the network around them; developing shared understanding, supporting their work with families, and offering reflective space to consider the impact of the work upon them. Social care colleagues will also be supported by the post holder to develop their reflective practice and case formulation skills through group supervision/ reflective practice groups and training. The clinical psychologist role will also provide a specialist psychological assessment, support and therapeutic intervention offer which may include parenting work, groups, and direct work with young people. There will be training available for the post holder to support them in their role.

Ealing MHST is a well-established schools-based early intervention mental health offer for mainstream schools, mainly offering interventions for mild-to-moderate low mood, anxiety and behaviours that challenge. We have strong links with the North London Clinical Psychology training courses and offer placements to trainees within our service. Job summary
Our CAMHS Eating Disorder Service has an exciting opportunity for a Recruit to Train (RTT) Trainee / Trainee Systemic Family Practice (SFP) Practitioner, as part of the NHS England (NHSE) funded Children and Young People (CYP) Psychological Training Programmes (formerly CYP-IAPT).
Recruit to Train is a government-funded initiative aiming to grow the child mental health workforce to meet much needed demand. To help reach this goal, NHSE covers salary and training costs for a 1-year post, allowing services to employ trainees to aid in implementing new or expanding existing specialist pathways to bridge the gaps in CYP mental health support.
This is a unique opportunity to apply your learning from weekly teaching into practical clinical interventions working with clients in the service making a direct impact in the lives of children, young people and their families.
Please note: The position will be based at 1 Armstrong Way, Southall, UB2 4SD but successful candidates will be expected to travel to Denmark Hill Campus of King's College London for teaching during the training year.

Main duties of the job
We are recruiting a SFP Practitioner to work within our West London CAMHS Eating Disorder Team. This is a training role within the Child and Young Persons Improving Access to Psychological Therapies (CYP-IAPT) Programme. We offer high quality supervision, with excellent CPD and career development opportunities.
This is a one-year fixed-term post (1.0 WTE), with this training year funded by NHSE. After successful completion of the course the fixed term training contract will end however applicants would be encouraged to apply for any vacant posts within the service and will be provided support and advice in job applications.
This role will be best suited for applicants that are able to balance a very busy work and study schedule. You will be an employee at the service but also enrolled on a Postgraduate Diploma course. As this is a Postgraduate level training, you will be required to study and perform at academic level. Applicants must be prepared to work towards their studies outside of protected study time (such as on weekends and after work). Protected study time or teaching days are typically two days a week during term (with possibility of extra university days near start) and will be a mixture of in person and online sessions. Applicants must be willing to travel into London to attend their lectures.
