The New Forest CMHT (East) team based in Totton, near Southampton has an exciting opportunity for a 24-month secondment, on a full-time basis for a Band 6 Mental Health Practitioner.

You will be responsible for care-coordinating a caseload of service users with complex long-term mental health problems who find it difficult to engage with services. You will work in a strengths-based and recovery-focused way to support service users to meet their mental health needs.

This will include physical health monitoring.

To have a proactive approach to the role and needs of carers/families. To be innovative and responsive to individual needs tailoring care accordingly. To be proactive in monitoring mental health needs and respond in a timely fashion if needs change. To participate fully in caseload supervision and management, to ensure that care plans are matched to need and service users can 'flow' through pathways of care. To work in line with all relevant policies and procedures, including care planning, risk and safeguarding. To alert the Team Leader or Manager to any concerns in a timely manner.

A fantastic opportunity exists for an experienced or newly qualified nurse who has a passion to deliver the best care for our patients on our wards. We are looking for a motivated and enthusiastic individual with a passion for rehabilitation and excellent outcomes.

Keats Ward and Conan Doyle Ward are situated in the West Wing, Western Community Hospital. They offer level 3 physical rehabilitation services to patients 18 years upwards to help regain their independence and rehab potential. Admissions are either 'Transfer of care' which are patients transferred from acute inpatient wards, or 'Step Up' via community teams/Acute Medical Unit/ Emergency Department to prevent acute admissions.

The post comes at an exciting time for rehabilitation within Southampton, South-West Hampshire and nationally after recently relocating to the Western Community Hospital within our new specialist rehabilitation unit - South of England Rehab Centre. We also currently operate a Level 2b Neurological Rehabilitation unit and a Neuropsychiatric unit which provide a range of specialist rehabilitation.

Job summary

HIOW Healthcare NHS Trust is looking for an inspirational registered nurse to work at Band 6 level as a Diabetes Senior Link Nurse working autonomously and as part of a team and to provide, and support to develop, a specialist service providing care to adults with diabetes within their own homes.

You will be responsible for delivering direct care, education and support to people with diabetes, their families and carers, and community nursing services in order to reduce hospital admissions and ensure people with diabetes continue to remain within their home environments. There will also be an opportunity to work towards creating an in-reach service to local acute hospital trusts in order to provide safe an effective discharge routes that enable patients to prepare for managing their diabetes in a community setting.

To reduce number of community nursing referrals for long term support with the administration of insulin, by supporting safe hospital discharge, developing an in-reach service to UHS to prepare insulin requiring pts on discharge, as appropriate.

To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. The Focus Care Practitioner aims to deliver support and to utilise psychological skills within the MDT. The practitioner will work in collaboration with the patient to deliver care plans aligned to recover focused outcomes. Primrose Ward is a busy Older Peoples Mental Health inpatient service, and the Ward Clerk plays a central role in keeping the ward organised, responsive and running safely. This post provides comprehensive administrative support to the Ward Manager, Clinical Lead Nurse, Matron and wider multidisciplinary team, ensuring that clinical staff are able to focus on delivering high quality patient care.

You will oversee the smooth flow of information across the ward, acting as a key contact for patients, families, carers and external professionals. They will ensure patient records and documentation are accurate, up to date and managed in line with Trust standards. The Ward Clerk will support the coordination of ward activity, including arranging meetings, preparing paperwork, managing diaries and ensuring timely communication of essential information.

The role also involves supporting workforce and operational processes such as maintaining safer staffing data, updating rosters, processing sickness and training information, and liaising with NHSP.

Main duties of the job

The Ward Clerk provides comprehensive administrative support to Primrose Ward, ensuring the smooth running of the OPMH inpatient service. The Neighbourhood Care Clinician role will work with Primary Care Network (PCNs) practices to proactively management patients living with frailty and other long-term conditions. Working autonomously to a high standard of clinical care, the clinician will be responsible for liaising with the PCNs to identify patients living with frailty who would benefit from an intervention to either prevent further deterioration of their frailty or other long-term conditions and to prevent hospital admission.

Using the medical model to assess, diagnose and prescribe for acute medical conditions, the clinician will use a holistic approach to consider methods and interventions to prevent recurrence of problems and complete referrals to appropriate members of the wider multidisciplinary team.

Following clinical assessment using comprehensive geriatric assessment either in the patients home or clinic environment, multi-disciplinary patient pathways will be designed to support patient choice, improve quality of life, promote self-management and assure early intervention through proactive provision of care in or close to the patients own home as possible.

Job summary

We are seeking an experienced and motivated Registered Nurse or Allied Health Professional to join our Frailty Urgent Community Response/Hospital@Home Team within Hampshire and Isle of Wight NHS Foundation Trust based in Lymington. This is an exciting opportunity to work in a nationally recognised service delivering urgent, holistic care to people living with frailty in their own homes.

Although the successful candidate's nominated base in Lymington the expectation is that they will travel across the area covered by the service to meet demand as needed.

The service operates 7 days a week, 08:00--20:00, with weekend rotation

Main duties of the job

Maintaining and continuously updating their existing knowledge, skills and expertise to inform their enhanced clinical judgement

Use existing knowledge and expertise and enhanced levels of clinical judgement to independently undertake complex and holistic assessments

Critically evaluating and analysing complex clinical interventions to make evidence-based decisions on how to manage the delivery of complex clinical care for a defined population, which may include individuals from diverse cultural, social and belief systems. They often manage a caseload, sometimes providing interventions as part of a dedicated clinical pathway.

Caring for people with complex care needs using their existing knowledge and expertise, enhanced clinical assessments, diagnostics, interventions and equipment. They must have the confidence and ability to think critically, and to apply a depth of knowledge and highly developed skills to provide enhanced evidence-based care

Working as part of a wider health and care team whilst being able to work alone when seeing people in their own homes in the community or in a clinical facility. They provide complex clinical care in the context of continual change, challenging environments, different models of care delivery, an older and more diverse population, vulnerable children and young people, mental health needs and innovation and rapidly evolving technologies

Are you an organised, adaptable, and proactive individual who enjoys variety in your work. We are looking for a Flexible Business Support Administrator to join our Physical Health Service, providing essential support across multiple teams.

No two days are the same in this role. While you will provide reception cover when required, your primary focus will be supporting three different teams across the service, stepping in where demand is greatest and ensuring our patients and staff continue to receive an excellent service.

• Build effective working relationships with colleagues across the service.

Main duties of the job

• To support the Reception, Therapies, Community Paediatric Medical and Community Nursing Service.
• To work as part of a skill mix team to provide comprehensive administrative support to the service
• To effectively prioritise and manage own workload and oversee workload of team as necessary, whilst maintaining confidentiality and diplomacy at all times, as you will be working with extremely confidential and sometimes distressing information
• To work flexibly and be willing to cover any administration tasks across the wider Business Support teams
• Produce letters and/or emails using standard Trust templates, ensuring accurate filing, recording and distribution
• To work without direct supervision and show extensive use of initiative in prioritising and making decisions in relation to effectively supporting the smooth operation of the service
• To carry out other duties as necessary to meet the changing needs of Solent NHS Trust, in accordance with management requirements and appropriate to banding
• All duties will be carried out within the Trusts Information Governance, recording, and data sharing procedures and guidance
• Occasional home working at weekends will be required.
