Main duties of the job

You will work across our Community Rehabilitation Hospital, Community Therapy Team & Hospital at Home Service, providing assessment, treatment & rehabilitation for patients following illness, injury or deconditioning

Working within a multidisciplinary team of physiotherapists, nurses, rehabilitation assistants, medical staff & social care colleagues, you will help patients maximise independence, recovery & safe discharge

You will assess & treat patients in their own homes, recommend equipment, adaptations & interventions, support admission avoidance, early supported discharge & rehabilitation closer to home, & contribute to integrated care planning, risk assessment & recovery for patients who may otherwise require inpatient care

We are looking for caring, motivated and enthusiastic Healthcare Support Workers to join our Enhanced Recovery Service.

Working as part of a multidisciplinary team, you will support people in their own homes following illness, injury or hospital admission to regain confidence, maximise independence and achieve their personal goals. You will provide practical reablement support, encouraging individuals to develop daily living skills and maintain their independence wherever possible.

You will work closely with patients, families, carers and health and social care colleagues to deliver person-centred care that focuses on what matters most to the individual. The role requires good communication skills, a compassionate approach and the ability to work both independently and as part of a team.

Main duties of the job

*The purpose of this role is to support the delivery of responsive, high quality and community based care that enables people to remain safely in their own homes and avoid unnecessary hospital admission.

*The post holder will contribute to an intermediate care service that supports timely discharge from hospital and promotes rehabilitation, reablement and recovery.

*The post holder will provide person centred reablement support in line with individual care plans, helping people to regain independence and achieve their personal goals.

Anstey Ward provides rehabilitation & recovery-focused care for patients with a range of physical health needs. We are looking for a dedicated nurse committed to delivering high-quality, person-centred care & supporting patients to achieve their rehabilitation goals.

As a Staff Nurse, you will provide expert clinical care within the ward environment, working closely with patients, families, General Practitioners & the multidisciplinary team. We are currently recruiting a Band 6 Clinical Charge Nurse for our Physical Rehabilitation Inpatient Ward. This is an exciting opportunity to work in a fast paced and dynamic team, within the Southeast Division of Hampshire and IOW Trust. We have the opportunity for a 22.5-hour position for an Urgent Community Response Nurse, contributing to the prevention of hospital admissions under a strong and robust frailty model.

We all know hospital is not the best place for a crisis and as a UCR Nurse, you can help to ensure patients are kept safe and looked after in their own home on our very own virtual ward. You will be responsible for their assessment of health needs, carrying out clinical review and ensuring they are safely managed in the community.

Main duties of the job

Working within professional guidelines and accountability, the post-holder will have responsibility for the management of a defined community caseload (via Nursing, Physiotherapy or Occupational Therapy interventions) for a group of GP Practices as part of the Integrated Care Team.

The post holder will have responsibility for managing co-ordinating and delegating work ensuring referrals are dealt with according to clinical need, holistic assessments and care plans are recorded in line with Trust and Professional record keeping standards and clinical care at all times meets acceptable standards of patient safety and quality outcomes.

As a Band 4 Senior Rehabilitation Assistant, you will work as part of a multidisciplinary team to support the delivery of rehabilitation programmes, helping individuals to achieve their goals and maintain independence. All Age Continuing Care delivers the patient specific Learning Disabilities and Autism Programme Integrated Care Board responsibilities.

Job summary

We are looking for an enthusiastic and experienced GP Receptionist to join our friendly and busy GP Practice. This will be primarily within our telephone hub at Waterside Medical Centre; although the expectation for the Care Navigator team is to work across all 5 of our Gosport sites. You will need to be able to demonstrate strong interpersonal, communication and presentation skills, be organised and have the ability to work as part of an integrated multi-skilled team, within a fast-paced environment. Flexibility is essential along with an understanding, acceptance and adherence to the need for strict confidentiality.

As a team player, you'll be able to prioritise your work in order to meet deadlines and have a customer focused approach. Previous experience of working within the NHS is desirable but not essential as full training will be provided.

Main duties of the job

The duties and responsibilities to be undertaken by members of the reception team may include any or all of the items in the following list. Duties may be varied from time to time under the direction of the Care Navigator Lead, dependent on current and evolving practice workload and staffing levels:

* Unlock and open surgery ready for business on time.

* Switch on and log in to all reception computers.
