An exciting opportunity has arisen within Mid and North Hampshire Division to appoint a motivated and dynamic Community Charge Nurse (Learning Disability) in the North and West Hampshire Community Learning Disability Team.

We have bases in Winchester, Basingstoke and Totton. This role is based in our Totton office, providing leadership within the team and delivering nursing services to people with learning disabilities within the West of the county.

You will provide supervision, support, and inspiration to nurses and nursing students in the Learning Disability Service. You will co-lead on the delivery of an evidence-based, person-centred nursing service that meets the needs of this complex client group.

You will play a vital role as a Community Charge Nurse in a forward thinking and innovative team. You will work closely with the nursing and multi-disciplinary team. There will be opportunities for continued professional development and reflective practice sessions.

You will need to demonstrate effective nursing and inter-personal skills, good communication and presentation skills, be organised, and self-motivated.

A car driver is essential given the large geographical area this post covers.

Main duties of the job

The post holder will:

• Be a member of an integrated inter-agency, multi-disciplinary learning disability team.
• Undertake comprehensive assessments of patients referred to the team.
• Contribute to effective multidisciplinary working within the Learning Disability team and build effective liaison with community, primary, other secondary and tertiary services and agencies to meet individual need and service development.
• Carry out all relevant forms of care without direct supervision, and will be required to demonstrate procedures to, and supervise qualified and unqualified staff.
• Manage a caseload of patients independently and be accountable for assessing, interpreting, planning and implementing nursing treatment and care to patients within professional guidelines.
• Undertake specialist and complex assessments and care planning.
• Lead nursing care for patients referred to the team for nursing assessment and intervention.
• Ensure that systems are used to incorporate any adopted models of quality assurance and governance into everyday practice.
• Deputise for and report to the Locality Senior Nurse as required.

We are looking for a highly motivated, experienced and innovative Band 6 mental Health Practitioner to join our CMHT. Meadows CMHT is a close, lively, dynamic, nurturing, and supportive team with excellent peer support and regular supervision. You will manage a reasonable-sized caseload of service users, covering shared care and carrying out assessments. You will be involved in training, mentoring and supervising new and unregistered staff as well as providing excellent front-line clinical care. We are looking to expand our clinical team and recruit additional Health Care Support Workers to help deliver the care delivered at Elmleigh. We are looking for compassionate individuals with a genuine interest in mental health recovery. Elmleigh is a 33 bedded adult acute mental health hospital situated in Havant caring for people with a functional mental illness - such as depression, a personality disorder or schizophrenia. Elmleigh is made up of two wards, Sapphire Ward (Male Ward) and Ruby Ward (Female Ward). In addition to these acute wards, Elmleigh also holds a place of safety in our urgent care zone.

We are looking to recruit a passionate, enthusiastic and motivated Band 6 Music Therapist to work as part of psychological services, supporting patients across the acute and crisis mental health inpatient wards and Crisis Resolution Home Treatment Team at Antelope House (Southampton).

This post will be fixed term for 6 months, part time, 22.5 hours per week, to support a wider music therapy project taking place in the service.

The Acute & Crisis psychology team at Antelope House consists of practitioner psychologists, assistant psychologists, and music therapist working collaboratively to provide assessment, brief interventions and consultation for adults with severe and/or enduring mental illness, requiring short-term admission to hospital or support from our crisis resolution and home treatment team to either avoid admission or support discharge.

This is an exciting opportunity for an early career music therapist to develop specialist skills within an acute and crisis mental health setting. The post has support from an experienced Music Therapist, regular supervision and opportunities for professional development with a supportive multi-disciplinary environment.

Main duties of the job

The postholder will provide music therapy assessment and intervention for service users within the acute and crisis services delivering both individual and group therapeutic work. Antelope House is a modern acute mental health inpatient and psychiatric intensive care unit. We offer high quality, recovery focused care to adults with a range of mental health needs, across inpatient and crisis services. We work transdiagnostically with a range of presentations, including psychosis, trauma, and relational difficulties.

You will contribute to the implementation of the Comprehend, Cope and Connect (CCC) approach as we continue to embed this within the service. Our Assistant Psychologist will provide support to both qualified and pre-qualified psychology staff in delivering assessment, formulation and direct interventions.

A key part of the role will be in supporting the implementation of our group programme. With appropriate support and supervision, there will be opportunities for direct work with service users within individual and group settings, and you will contribute to the screening and prioritisation of referrals.

You will receive weekly clinical supervision from a qualified psychologist. We work closely alongside other local psychological services, offering a rich network of both qualified and pre-qualified psychologists.

Main duties of the job

The post holder will work as a recognised specialist Speech and Language Therapist in the Forensic Learning Disability Service for Adults with a Learning Disability. They will take a lead role in the assessment and treatment of patients presenting with complex communication disorders and challenging needs and/or forensic history. Job summary

We are looking for a compassionate trauma-informed psychologist who is interested in working within the area of acute mental healthcare. Our service at Parklands Hospital includes male and female wards, and a PICU. Your role will focus on clients referred to our Crisis Resolution and Home Treatment Teams (based primarily in Basingstoke, as well as one base in Winchester), supervised by a CAT-trained consultant clinical psychologist.

You will join a sizeable diverse team of warm, supportive, and creative psychologists and assistant psychologists, who are committed to providing inclusive person-centred care, and a psychologically-minded multidisciplinary team committed to embedding a recovery culture. We work transdiagnostically with a range of presentations, including psychosis and trauma, and relational difficulties. We are seeking enthusiastic and motivated administrators within our busy Integrated Single Point of Access (ISPA).

The team provides a vital service that includes taking calls from patients, carers, GPs, hospitals, the Ambulance Service, and other healthcare agencies and professionals, and processing email referrals and messages received by the team. The team provides administrative support to the community nursing teams, community therapy, and the Frailty Support Service. ISPA work entails using a variety of software packages so good IT skills are also vital to this position. You work well in a team and individually with the ability to prioritise your work to meet deadlines and you will have a customer focused approach.

The community respiratory service consists of the respiratory virtual ward, long term respiratory management, home oxygen and pulmonary rehabilitation. The team provides urgent care to people in their homes.

Within the respiratory virtual ward, the team aims to deliver holistic assessments and interventions to individuals who are at risk of hospital admission. The aim of the team is to enable people to stay at home, recover, and re-engage with the important aspects of their life and prevent admission to acute sectors.

Following assessments and intervention, if patients require further support to help keep them at home, patients will be admitted to the virtual ward. The virtual ward provides a safe alternative to hospital for patients living with long term respiratory condition through community-based acute health and care delivery

The home oxygen team is a community assessment service for predominantly respiratory adult patients who require assessment and ongoing management of their oxygen requirements in line with BTS guidelines and key standards of practise.

Pulmonary rehabilitation is a prescribed and progressive exercise and education programme designed for people with long term respiratory conditions who experience symptoms of breathlessness and helps them better understand their condition and symptoms to support effective self-management.

The team acts as an expert resource for other healthcare professionals dealing with complex respiratory patients.

We are a busy happy team who welcome new staff members and encourage staff development and learning. We are seeking a registered nurse to join us in providing care and treatment to adults living in the area who have long term or complex health needs. The scope of your role will focus on supporting admission avoidance, early supported discharge, promotion of independence and assisting in the effective management of patients with long term or complex conditions. You will be working in a multi-disciplinary way alongside a range of primary care services including those from GP's, social and voluntary sector. You will be responsible for managing your allocated work on a day-to-day basis ensuring patients are prioritised according to clinical need and that assessments and care plans are recorded in line with the Trust's standards.

To support you within your role, the Trust have an excellent learning and development department, access to further develop your clinical skills and leadership development are available and encouraged.

Our service runs 7 days a week and therefore this position requires flexible working, and all requests will be considered.

Access to a vehicle and possession of a full driving license is essential along with business insurance.

Main duties of the job

The primary role of the Senior Community Nurse is to provide strong holistic leadership to the team and support the Community Matron. To include day to day leadership responsibilities, development and creative leadership, responsive leadership, enabling the team to achieve a 'well led' culture.

The community teams in conjunction with a wide range of clinical colleagues and specifically, GP's, Practice Nurses and Social Care professionals, will lead and facilitate a patient or client focused, co-ordinated case management approach across primary and secondary care for people who are most vulnerable to, and at high risk of repeat admissions to hospital.

The candidate will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management and assure early intervention through the proactive provision of care in or as close to the patient's own home as possible. The candidate will work across the caseload and the single point of access or (equivalent), using their clinical skills to identify the needs of patients and the correct services to liaise with.

The candidate will provide an expertise in their own discipline to the wider team.
