Job summary
We have an exciting new role as a part-time administrator supporting our established and welcoming Complex Care Team. This is a neighbourhood working project, supporting people with frailty and complex care needs in the community. Working eighteen hours a week based at Harbourside Family Practice, you will be working with a dedicated team of staff from practices within the Gordano Valley Primary Care Network as well as Sirona. You will also be liaising with external agencies, including Social Care.
Flexibility around working days and willingness to travel is essential, as you will be required to attend one or more MDT meetings a week. These will be at one of the four GP practices in Portishead, Pill and Clevedon, and on different days depending on the location.
We are looking for a team player, with experience of managing a busy workload, who is passionate about supporting clinical staff to ensure best outcomes for patients.
Main duties of the job
Administrative Support for the Complex Care Team based at Harbourside Family Practice.
Liaison with external agencies who are involved in the work of the Complex Care Team.
Arranging and attending MDT meetings at four GP practices in Clevedon, Portishead and Pill.
Minuting meetings and ensuring all agreed actions are recorded and distributed.
Maintaining patient records on excel, EMIS and Black Pear.
Information governance for MDT meetings.
Support with data analysis of CTT outcomes.

Job summary
Social prescribing empowers people to take control of their health and wellbeing, focus on what matters to me and take a holistic approach to an individual's health and wellbeing, connecting people to community groups and statutory services for practical and emotional support.
Social prescribing link workers work as a key part of the primary care network (PCN) multidisciplinary team. Social prescribing can help PCNs to strengthen community and personal resilience, reduce health inequalities (in relation to timely access and outcomes) and wellbeing inequalities by addressing the wider determinants of health, such as debt, poor housing and physical inactivity, by increasing people's active involvement with their local communities. It particularly works for people with long term conditions (including support for mental health), for people who are lonely or isolated, or have complex social needs which affect their wellbeing.
We are looking for someone who can work 22.5 hours a week over three days, ideally Monday to Wednesday but there may be some flexibility for the right candidate.

Main duties of the job
The Social Prescriber will work as part of the Primary Care Network's multidisciplinary team to support patients with non-medial needs that impact their health and wellbeing.
The role includes undertaking home visits where appropriate, completing wellbeing assessments and developing a personalised support plan. It involves collaborative working with health, social care and voluntary sector organisations as well as carers and families.

Job summary

This is a varied and rewarding role supporting our patients with a range of clinical services, including phlebotomy, wound care and dressings, blood pressure monitoring, ECGs, NHS Health Checks and other routine healthcare procedures. The successful candidate will work closely with our GPs, nurses and wider practice team to provide high-quality, patient-centred care.
Previous experience within General Practice or a healthcare setting would be an advantage, and phlebotomy experience is particularly desirable. We are also keen to hear from candidates who are enthusiastic about developing their clinical skills, as appropriate training and competency assessment can be provided for additional aspects of the role.
Main duties of the job
The Healthcare Assistant will provide a range of clinical services to our patients, including phlebotomy, wound care and dressings, blood pressure monitoring, ECGs, NHS Health Checks and smoking cessation support. The role will also include assisting GPs and nurses and undertaking other appropriate clinical duties following training and competency assessment. The post-holder will work closely with the wider clinical team, maintain accurate patient records and ensure that all care is delivered safely, professionally and in accordance with Practice policies and procedures.

AIM OF POST
To work as an integral member of the Practice clinical team, providing high-quality healthcare and clinical support to patients of Harbourside Family Practice.
The Healthcare Assistant will undertake a range of clinical duties within their level of competence, following appropriate training and competency assessment and working in accordance with Practice policies, protocols and clinical guidance.
The role will include:
- Venepuncture and blood sampling
- Wound care and dressings

- Urine dipstick checking
- NHS Health Checks
- Smoking cessation support
- INR clinics, where appropriately trained and competent
- Assisting clinicians with minor operations and coils/implants

MAIN RESPONSIBILITIES
- To provide a high-quality clinical service to patients with due regard to safe clinical practice, infection prevention and control, confidentiality, dignity, consent and health and safety.
- To undertake venepuncture and blood sampling for adults and other appropriate patient groups, in accordance with Practice procedures and following appropriate training and competency assessment.
- To ensure the accurate identification, labelling, recording, handling and despatch of specimens in accordance with Practice and laboratory protocols.
- To maintain a working knowledge of blood collection systems, specimen containers, required sample volumes and the appropriate handling requirements for commonly requested investigations.
- To undertake additional specimen collection and testing in accordance with Practice protocols and within the post-holder's assessed competence.
- To undertake wound assessment and wound care within the limits of the HCA role, including routine wound dressings, removal and replacement of dressings and other appropriate wound care procedures following training and competency assessment. Any wound requiring assessment or treatment beyond the post-holder's competence must be referred to an appropriate registered clinician.
- To undertake blood pressure measurements and record results accurately within the patient's clinical record.
- To perform ECGs following appropriate training and competency assessment, ensuring recordings are referred to an appropriate clinician for interpretation.
- To undertake urinalysis and other point-of-care testing as authorised by the Practice and following appropriate training and competency assessment.
- To undertake NHS Health Checks and other health promotion activities in accordance with Practice protocols.
- To provide smoking cessation support and advice within the scope of the role and following appropriate training.
- To undertake INR clinics where appropriately trained, assessed as competent and authorised by the Practice, working strictly within the relevant Practice protocol and referring results outside agreed parameters to an appropriate clinician.
- To assist GPs and other clinicians during minor surgical procedures, including preparing clinical areas and equipment, providing appropriate assistance during procedures and ensuring appropriate infection prevention and control measures are followed.
- To assist clinicians undertaking coil and contraceptive implant procedures, including preparing equipment and the clinical environment and acting as a chaperone where appropriate.
- To act as a chaperone for patients and clinicians when requested, in accordance with the Practice Chaperone Policy.
- To undertake basic health promotion and lifestyle advice appropriate to the role and to signpost or refer patients to an appropriate clinician where further assessment or advice is required.
- To accurately record all consultations, observations, procedures and other relevant information within the patient's electronic clinical record.
- To maintain appropriate stock levels within clinical rooms, ensuring equipment and consumables are stored correctly and that expiry dates are monitored.
- To clean clinical equipment and maintain clinical areas in accordance with Practice infection prevention and control procedures.
- To ensure clinical waste, sharps and specimens are handled and disposed of safely and in accordance with Practice procedures.
- To assist with clinical audits, searches and other quality improvement activities as required by the Practice.

MAIN RESPONSIBILITIES
- To provide a high-quality clinical service to patients with due regard to safe clinical practice, infection prevention and control, confidentiality, dignity, consent and health and safety.

HEALTH AND SAFETY RESPONSIBILITIES

Job summary
We are seeking a Clinical Administrator to join our high-performing and supportive practice team. We are proud to have been ranked among the top practices in BNSSG, achieving 4th place in the 2023 NHS Patient Survey.
This role is available on either a full-time or part-time basis, with working days and hours negotiable for the right candidate.
The role will involve a varied mix of clinical administration and secretarial duties, supporting GPs, nurses and other healthcare professionals in the effective day-to-day running of the practice.
You will undertake a varied and engaging workload, helping to ensure that clinical correspondence, referrals and other administrative tasks are managed accurately and efficiently, while contributing to the delivery of a responsive and high-quality service to our patients.
We are looking for someone who is organised, approachable and detail-focused, with the ability to communicate clearly and professionally. Strong IT skills are essential, together with the ability to manage competing priorities and work effectively as part of a busy and supportive team.
Previous experience in a clinical, medical or secretarial administrative role would be advantageous but is not essential, as comprehensive training will be provided.
Main duties of the job
Managing incoming and outgoing telephone communications, taking accurate messages and responding to routine clinical and administrative queries where appropriate.
Accurately coding and entering information onto patient medical records in accordance with agreed practice procedures and coding standards.
Processing and filing hospital and clinic correspondence within the clinical system, identifying relevant information and initiating appropriate follow-up actions where required.
