The post holder is responsible for ensuring the delivery of safe and effective care to Health and Beyonds Diabetic/Pre-Diabetic patients across the organisation.
You will work closely with the Senior Diabetes Specialist Nurse and the Diabetes Specialist Nurse along with the Diabetes team to manage our patients living with Diabetes across all sites, in their own homes and also patients living with Diabetes in our Care Homes, working alongside our management, clinical and admin teams with a view to improving our patients health outcomes.
You will be able to work autonomously providing assessment, screening and education to Diabetic patients running specific clinic sessions and maintaining robust call and recall systems.
Main duties of the job

About us
Our building has been purpose-built to cater for the modern regulations relating to disabled access. All patient areas are on the ground floor and a disabled patients toilet is within easy reach of the waiting area. A loop system for the hard of hearing surrounds the waiting area. If, because of hearing loss, you are unlikely to hear your name on the public address system, please notify a member of staff who will make arrangements to notify you when your doctor is ready to see you. A similar enquiry should also be made by partially sighted or blind patients if you do not have a sighted person to assist you.
