Are you able to provide interventions to both patient and carers to maintain wellbeing and prevent crisis?
If so, come and join our innovative Community Dementia Service.
Our Community Dementia Service is provided in collaboration with the Alzheimers Society and will form a key part of support throughout the dementia journey. Sitting within our Older Adult Community Mental Health Team (OACMHT), you will be providing brief interventions for people living with dementia with the aim of reducing distress, maintaining wellbeing and avoiding crisis. You will have access to the wider MDT where patients can be stepped-up to the OACMHT for more complex or on-going support. You will also spend time each week undertaking routine dementia assessments or other OACMHT activities.
Working hours are Monday to Friday 09.00-17.00. Part time and flexible working will be considered.
Main duties of the job
A typical day will include a mix of direct clinical assessment and interventions for people living with dementia as well as providing clinical advice to the Dementia Advisors and other services e.g. Primary Care. You will offer short term interventions to patients and their carers to reduce distress, maintain wellbeing and avoid crisis. Your interventions will be goal based and look to empower those accessing the service. You will support the development of the service through the collection of outcome measures and other metrics and engage in promotion and education events, as well as your own professional development. You will provide a seamless transition for patients supported by the Dementia Advisors into your service and utilise your expert knowledge to assess need and guide patient care. You will take a collaborative approach to all of your work, and ensure accurate documentation is maintained.

Job summary
We are looking to recruit a dynamic, motivated Consultant Psychiatrist to join the team at Redditch Neighbourhood Mental Health Team.
Worcestershire is a mixed rural and urban county of over 500 square miles and around million population. It covers the City of Worcester together with the towns of Kidderminster, Stourport, Bewdley, Tenbury Wells, Bromsgrove, Redditch, Droitwich, Evesham, Pershore, Malvern and Upton upon Severn.
Herefordshire & Worcestershire offer excellent living and working prospects. Nearby are the Wye Valley, an Area of Outstanding Natural Beauty; the Forest of Dean; the Malvern Hills; and the Brecon Beacons National Park; with easy access to the M5 and M50. The Redditch Neighbourhood Mental Health Team (NMHT) underwent Community Mental Health Transformation in 2021, following the principles outlined in the NHS Community Mental Health Framework (2019). Our way of working aims to improve access, reduce fragmentation, and help people maintain wellbeing, stability, and meaningful lives within their own communities.
This is full time post, but we would consider applicants looking for part time.
Main duties of the job
The team provides accessible, person-centred mental health care supported by local Primary Care Networks. Its purpose is to offer holistic, trauma-informed support for adults with a range of mental health needs, including severe, complex and enduring conditions. The team aims to meet the needs of both people with Severe Mental Illness (SMI) as well as those with trauma histories, personality difficulties, co-morbid substance use, long-term physical health conditions, and social adversity. To this end, the team works collaboratively with primary care, social care, voluntary sector partners and community services to deliver integrated, recovery-focused care. The aim is seamless care across organisational boundaries with a no wrong front door referral policy and a step-up and step-down flow of patient care between primary and secondary services.
The teams accepts referrals for working age adult patients aged 17 and over. Referrals are routinely received from:

Job responsibilities
Mental Health Practitioner caseloads are generally limited to 15-20 cases, allowing the team to deliver a range of psychosocial interventions, including Crisis Safety Planning, Emotional Management and Distress Tolerance Skills, Anxiety Management, Early Warning Signs / Relapse Prevention, Behavioural Activation and Sleep Hygiene.
The Team also offers a Managing Emotions Group to patients experiencing emotional dysregulation, often as a preliminary to further psychological work such as DBT.
The team is working to deliver access to NMHT services within 28 days of referral, and are currently meeting this target.
There is a twice weekly referrals meeting where new referrals are discussed together with cases of concern, and a monthly team meeting for education and team administration.
Psychological intervention is offered via three pathways. There is a dedicated Bipolar and Psychosis pathway, a trauma-specific Specialised Individual (SI) pathway and the more generalised Specialised Teams (ST) pathway. The Worcester City NMHT has 4.4 FTE funded posts for psychological provision.

Mental Health Practitioner caseloads are generally limited to 15-20 cases, allowing the team to deliver a range of psychosocial interventions, including Crisis Safety Planning, Emotional Management and Distress Tolerance Skills, Anxiety Management, Early Warning Signs / Relapse Prevention, Behavioural Activation and Sleep Hygiene.
The Team also offers a Managing Emotions Group to patients experiencing emotional dysregulation, often as a preliminary to further psychological work such as DBT.
The team is working to deliver access to NMHT services within 28 days of referral, and are currently meeting this target.
There is a twice weekly referrals meeting where new referrals are discussed together with cases of concern, and a monthly team meeting for education and team administration.
Psychological intervention is offered via three pathways. There is a dedicated Bipolar and Psychosis pathway, a trauma-specific Specialised Individual (SI) pathway and the more generalised Specialised Teams (ST) pathway. The Worcester City NMHT has 4.4 FTE funded posts for psychological provision.

An exciting opportunity has arisen for an experienced communications professional to join our team as Marketing and Communications Lead on a 12-month maternity cover contract. Main duties of the job
The main duties of a healthcare support worker are:
Assist with admitting patients to the ward and the care of their relatives
Undertake clinical procedures following assessment of competencies by qualified senior members of staff

Job summary
An opportunity to become a Mental Health Practitioner in Malvern has arisen, which is a great opportunity to flex and develop your skills in working with people with a wide range of mental health-related needs in a creative way, supported by a close-knit, warm and friendly team.
Your role includes representing our service within a GP surgery, acting as a point of expert contact for GPs and other professions working within a defined locality. It also involves the exercise of considerable autonomy, though with support readily available and is perhaps unique in terms of the range and variety of people for whom you will be providing care, as well as being a role that involves learning skills for managing longer-term therapeutic relationships. Your working day will involve spending time both at our base with the team, as well as time out at GPs and in the community. Homeworking, in-keeping with the autonomy inherent to the role, is also available.
This is a full time role.
Main duties of the job
Travelling to your GP surgery in the morning to see two new patients for assessment. While there, dropping in on one of the GPs to talk about one of the assessments and run through another proposed referral to decide on the best mental health pathway for them.
Travel to base and write up the assessments and have the opportunity to talk through any concerns with fellow team members. You decide who you will be taking onto your caseload; no one allocates to you.
Attending professional meetings, including ward reviews via Teams or in person to review patients in your care
Attending MDT every Wednesday via Teams. Tend to work from home that day. Made some internal referrals to other team members and discussed some concerns about other patients you've been working with.
Meeting some of your longer-term patients and reviewing where you've got to with their care. Its good to work from a well-established relationship.

This is a full time post.
You will be integrated with nurses and therapists and instrumental in the journey of patients on the intensive support pathway. Assessing patients being discharged from hospital or those requiring support to prevent admission, identifying short-term care support for rehabilitation and immediate equipment needs. The role is fast paced and responsive.
Alongside intensive support, you will triage and assess patients requiring functional OT assessments in their homes, including manual handling and provision of specialised and standard equip, working closely with GPs, physios, social services, HASE (housing adaptations & specialist equip) and the voluntary sector.
There are opportunities for training and development, alongside in-service training, appraisals and clinical supervision.
Main duties of the job
A shift is 7.5hrs between 0830 1630hrs, Mon to Fri.
A typical day includes coordinating your caseload, triaging referrals, supporting junior staff and assistants and prioritising urgent assessments alongside colleagues.
The role will involve:
Completing holistic assessments of patients needs following hospital discharge or to prevent admission.
Completing assessments for low level equipment including riser recliner chairs, minor adaptations (e.g. rails, half steps) and manual handling.
Co-ordinating care needs of patients on the intensive support team by liaising the GP, MDT, relatives and other agencies. Including organising individual client case reviews, communicating plans with the MDT, and being responsible for the ongoing actions in relation to the patient.
Line management of Band 5 and Band 3 team members including training, supervision and appraisals.
Maintaining contemporaneous clinical notes to the high professional standards of the COT.

We would like to appoint an experienced Clinical/Counselling Psychologist to work in the Adult Mental Health (AMH) Neighbourhood Mental Health Services (NMHS) in Worcestershire to particularly work with people who present with Psychosis or Bipolar. The NMHS teams have integrated psychological pathways for individuals who present with moderate/severe, severe/enduring, psychosis/bipolar and emotional regulation difficulties. We aspire to encourage a sense of empowerment in our service users through working with trauma informed service delivery.
The post holder will deliver Clinical or Counselling Psychology core values and clinical practice, will work with a diverse service user group presenting with Psychosis and Bipolar with a range of complex mental health needs.
The post holder will input into teaching, training, research and supervision. We have excellent links with the local doctorate training courses and offering a training placement would be encouraged.

Main duties of the job
The post holder will be an integral member of the NMHS, working with other psychologists and mental health professionals. They will be a valued member of the psychology provision within the Trust.
The post holder would work individually, providing psychological assessments and formulations with individuals presenting with Psychosis or Bipolar with complex psychological presentations. Group work will also be provided for service users and their families. Team consultation, reflective practice, supervision, research and teaching would also be delivered by the post holder as appropriate for the working hours. Job summary
We would like to appoint a Clinical/Counselling Psychologist to work in the Adult Mental Health (AMH) Neighbourhood Mental Health Services (NMHS) in Worcestershire to particularly work with people who present with Psychosis and Bipolar. The NMHS teams have integrated psychological pathways for individuals who present with moderate/severe, severe/enduring, psychosis/bipolar and emotional regulation difficulties. We aspire to encourage a sense of empowerment in our service users through working with trauma informed service delivery.
The post holder will deliver Clinical or Counselling Psychology core values and clinical practice, will work with a diverse service user group presenting with Psychosis or Bipolar with a range of complex mental health needs.
The post holder will have the opportunity to input into teaching, training, research and supervision. We have excellent links with the local doctorate training courses and offer training placements.
This post is a Band 7, development role, which will move to a Band 8a upon successful completion of a development plan. We welcome applications for full time working but would be happy to discuss alternative working hours as necessary.
Main duties of the job
The post holder will be an integral member of the NMHS, working with other psychologists and mental health professionals. They will be a valued member of the psychology provision within the Trust. As a development role supported personal and professional development will be a priority.
The post holder would work individually, providing psychological assessments and formulations with individuals presenting with Psychosis or Bipolar with complex psychological presentations. They would also be involved with Group work. The post holder would be supported to provide team consultation, reflective practice, supervision, research and teaching as appropriate for the working hours. We provide both in person and virtual interventions alongside applied psychological ways of working with MDT colleagues.
The post holder will receive both clinical supervision and line management. Priority will be given to wellbeing and personal and professional development.

You will be joining a friendly and close-knit consultant body in the county.
The Older Adult Community Mental Health team operates from Benet Building, Belmont, Hereford with patient contact being primarily in the community or via outpatient clinics. Inpatients wards are located in the Stonebow Unit, Hereford.
You will be delivering ECT alongside other consultants and join 1:11 on-call rota. The overall sessional pay with on-call commitments will be 11 PAs plus 3% plus 1%. You will have a dedicated office and secretary.

Main duties of the job
Main duties of the role include:
- Safely assess and manage patients
- Undertake MHA assessments as the approved doctor under Section 12(2) as required
- To carry a community caseload of patients
- To carry out outpatient clinics, home visits, and telephone conversations with service users
- To manage both functional and organic inpatients across 2 specialised wards, act as Clinical Lead and Responsible Clinician
- Providing Electroconvulsive therapy sessions in the county as part of a rota
- Work with local managers and professional colleagues

- To develop and maintain care pathways and pathways of communication with other services
- To liaise and give advice to other statutory/voluntary agencies

Hereford & Worcestershire Health & Care Trust is excited to two new innovative Band 5 podiatrist roles . These permanent posts have been designed to provide newly qualified and early-career podiatrists with a unique opportunity to gain experience across all aspects of podiatry.
Working across Worcester City and Malvern sites, you will develop a diverse clinical skillset whilst being supported by experienced clinicians, and multidisciplinary teams. This role offers exposure to a wide range of patient groups and clinical environments, providing an excellent foundation for future career progression.
Within our community podiatry service, you will experience and have exposure to caseloads including wound care, musculoskeletal presentations, nail surgery and domiciliary care. You will undertake regular home visits and be provided with remote IT equipment to support agile working.
Within our Acute Foot Health service, you gain valuable experience working alongside multidisciplinary teams managing high-risk foot complications. Opportunities include exposure to busy acute clinics including vascular and orthopaedics and diabetic foot MDTs.
You will receive comprehensive induction, preceptorship programme, supervision and ongoing professional development.

This role as a band 3 Health care assistant offers lifestyle flexibility as part of the evening team working 18:30-23:30 There are 15 hours available working 3 shifts a week.Although the team is a countywide team with a central base in Droitwich, there are several satellite bases across the county. Main duties of the job
As an HCA on the evening team you will be allocated via our electronic system onto a handheld tablet, patients to visit in their own homes, primarily across the North of the county. These would normally be within a geographic area, but some flexibility is expected to cover patients countywide. The role of the HCA is varied, in supporting patients in their own home with personal care, mobility, and simple wound care and catheter care. All appropriate training will be provided as part of the induction process. You will work independently but also as part of a team, with some visits requiring 2 members of staff. Join Our Dedicated CAMHS Eating Disorder Team
This is an exciting opportunity to join a dynamic and rewarding service where no two days are the same. We are looking for an enthusiastic, compassionate, and motivated individual with excellent problem-solving skills, flexibility, and a genuine passion for improving outcomes for children and young people.
As a key member of the Eating Disorder Service, you will play an important role in supporting the delivery of the clinical pathway and helping to monitor the physical and emotional health and wellbeing of children and young people experiencing eating disorders. You will work alongside registered professionals to support the assessment and monitoring of young people with complex mental health needs, promoting early intervention, healthy lifestyles, and positive wellbeing outcomes.
This role offers a unique opportunity to develop specialist skills within a well-established service while making a meaningful difference to the lives of children, young people, and their families. Successful candidates will demonstrate the ability to build effective relationships, offering person-centred care that meets the individual needs of each young person.

Main duties of the job
The ability to communicate effectively with others at all levels.
Working flexibly to provide the service delivery need across the county (the service operates from 9 am - 5 pm, Monday to Friday excluding bank holidays). The service operates across Hereford & Worcestershire and there may be occasions where cover is required across both counties.
An enthusiastic, motivated and innovative thinker to become part of a Children and Young People facing community.
The ability to work autonomously to manage your own diary whilst working within a multi-disciplinary team.
The ability to problem solve and manage time effectively.
