Job summary
We are looking for Salaried GPs to join our established and supportive primary care team at Holderness Health. This is a flexible opportunity with sessions - up to 8 per week - negotiable, including a regular Enhanced Access session one day per week, offering additional flexibility for clinicians interested in extended hours working.
The role is ideal for an experienced GP or someone newly qualified who is looking to develop their career within a forward thinking practice.
The post holder will provide high quality, patient centred general medical services to our registered and temporary patients, working collaboratively with the wider clinical and nonclinical team to meet a broad range of health needs.
Main duties of the job
You will deliver high quality, patient centred GP services including face to face and telephone consultations, home visits, Duty Doctor cover and weekly participation in Enhanced Access services. You will manage acute, chronic, complex and undifferentiated conditions and develop care plans with patients.
You will prescribe safely in line with guidance, keep accurate clinical records, and contribute to QOF and quality improvement activity. Working collaboratively with the multidisciplinary team of GPs, Advanced Nurse Practitioners, First Contact Physiotherapists, Proactive Care Team including Social Prescribers and Mental Health Worker, you will be committed to delivering excellent patient care and positive patient outcomes, with a focus on supporting continuity of care for our most complex patients.
We are looking for a fully qualified GP with GMC registration, on the NHS Medical Performers List or nearing completion of training. You will have strong clinical judgement, excellent communication skills, good organisation and a compassionate, professional approach. You should be able to manage workload effectively, work independently and value teamwork, prevention and reducing health inequalities. Commitment to patient care.

The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Clinical Responsibilities:

To process all relevant prescriptions as authorised by GPs and prescribing nurses where appropriate in accordance with the principles of good practice.
Job responsibilities
To ensure that all medicines and appliances dispensed are checked against the prescription and whenever there is doubt about the appropriateness of the item, or about the dose or labelling instructions to check with the authorising doctor or duty doctor.
To collect all due prescription charges and ensure that the patient declaration on the reverse of the FP10 is duly filled in and signed by the patient.
To ensure that all monies received or handled on behalf of the practice are appropriately stored and banked and a record kept of all financial transactions.
To endorse all prescription forms as appropriate, collate and forward the forms in a secure manner to the PPA for processing and reimbursement. The forms should be bundled in accordance with current PPA guidelines and include any necessary accompanying paperwork such as the FP34D and invoices as required by the PPA.
To promptly forward all invoices and dispensary related correspondence to the practice manager or dispensary manager according to practice policy.
To notify the authorising doctor of any FP10 returns/feedback from the PPA so that any appropriate remedial action may be taken.
To operate efficient stock control appropriate to the needs of the practice with the objective of ensuring continuity of supply for patients and minimising wastage through out of date stock.

Job summary
We are seeking an experienced and autonomous Urgent Care Practitioner to join our team. As one of our Care Co-ordinators, you will be responsible for supporting the practice in delivering excellent patient care by effectively co-ordinating different elements of care within and outside the practice.
The Care Coordinator team is responsible for coordinating the proactive elements of patient care such as long-term condition annual reviews, helping people to stay well. They are the first point of contact for the 18 care homes within our community. They book and track recalls, vaccination programs and proactive cancer screening. Main duties of the job
Our successful candidate will have a busy and varied role including service co-ordination, care planning for patients and providing patient support. You will co-ordinate and liaise with clinical and non-clinical teams within Holderness Health and across our partner agencies and neighbourhood.

Role Purpose:
To support the practice in delivering excellent patient care by effectively co-ordinating different elements of care from multi-disciplinary teams within and outside the practice
Service Co-ordination
Provide coordination and navigation for patients and their carers across health and care services, working closely with Social Prescribing Link Workers, Health and Wellbeing Coaches, and other primary care professionals
Co-ordinate the work of healthcare professionals and non-clinical staff involved in the care of patients registered at the practice
Co-ordinate MDT meetings, collating information on patients requiring review and providing secretarial and administrative support, and ensuring the completion of resulting actions

An exciting opportunity has arisen for a motivated and detail oriented Finance Administrator to join our busy Finance Team on a fixed-term basis to provide maternity cover.

Ensure the kitchen is kept clean and tidy and air and tidy the waiting room in turn with other staff.
Premises:
Open up premises at the start of the day when first to arrive, de-activate alarm and make all necessary preparations to receive patients.
When last to leave at the end of the day, ensure that the building is totally secured, internal lights are off and the alarm activated.
Ensure efficient running of appointment system and monitor flow of patients into Treatment/Consulting rooms.

Job summary
We are looking for Salaried GPs to join our established and supportive primary care team at Holderness Health. This is a flexible opportunity with sessions - up to 8 per week - negotiable, including a regular Enhanced Access session one day per week, offering additional flexibility for clinicians interested in extended hours working.
The role is ideal for an experienced GP or someone newly qualified who is looking to develop their career within a forward thinking practice.
The post holder will provide high quality, patient centred general medical services to our registered and temporary patients, working collaboratively with the wider clinical and nonclinical team to meet a broad range of health needs.
Main duties of the job
You will deliver high quality, patient centred GP services including face to face and telephone consultations, home visits, Duty Doctor cover and weekly participation in Enhanced Access services. You will manage acute, chronic, complex and undifferentiated conditions and develop care plans with patients.
You will prescribe safely in line with guidance, keep accurate clinical records, and contribute to QOF and quality improvement activity. Working collaboratively with the multidisciplinary team of GPs, Advanced Nurse Practitioners, First Contact Physiotherapists, Proactive Care Team including Social Prescribers and Mental Health Worker, you will be committed to delivering excellent patient care and positive patient outcomes, with a focus on supporting continuity of care for our most complex patients.
We are looking for a fully qualified GP with GMC registration, on the NHS Medical Performers List or nearing completion of training. You will have strong clinical judgement, excellent communication skills, good organisation and a compassionate, professional approach.
