Job summary
An exciting opportunity has a risen at our practice for an Experienced Practice Nurse. We are seeking a full-time employee; however, we are open to discussing part-time (30+ hours) or compressed hours for the right candidate.
You will benefit from the support of an experienced Lead Practice Nurse and supportive Management Team.
Please note interviews will be held on the morning of Tuesday 27th October
Main duties of the job
The ideal candidates will have recent experience within a Primary Care setting, be adaptable, highly motivated and relish a challenge. You will be part of a dynamic and innovative team who value teamwork, professional practice, mutual support and a friendly working environment.
Working with the Lead Practice Nurse, you will be a member of the practice multidisciplinary team, ensuring that nursing services are delivered effectively to our patient population.
The post holder will be responsible for several clinical areas including chronic disease management, as well as supporting the practice management team in the reviewing and delivery of clinical policy and procedure.

We are the lead practice for our PCN, working collaboratively with two other practices, Sid Valley Practice and Coleridge Medical Centre. We have developed a wider range of PCN teams and roles, including, Mental Health and Wellbeing Team, First Contact Physiotherapists, Clinical Pharmacists and Pharmacy Technicians. Through the PCN we are also developing a Practice Nursing Network, led by our Lead PCN Nurse.

We are looking for a friendly, motivated and IT confident Care Coordinator to join our busy GP practice in Honiton. This is a patient facing and coordination role where excellent communication, accuracy and professionalism are essential.
Hours
This role includes some long working days, in line with GP practice opening hours 08:00-18:30. We are recruiting for a parttime role with the following potential working pattern:
Monday 14:00 - 18:00 (4 hours)
Tuesday 8:00 - 6:00 (9 hours)
Wednesday 14:00 - 18:00 (4 hours)
Thursday - 14:00 - 18:00 (4 hours)

Main duties of the job
As a Care Coordinator, you will support patients through their care journey, ensuring they receive the right care, at the right time, with the right clinician. You will work closely with the clinical and administrative teams to coordinate care, manage information, and support practice processes.
You will be a key point of contact for patients in person, by phone and online. Helping them understand next steps, navigate services and complete required actions.

Honiton Surgery is seeking an experienced and motivated Advanced Nurse Practitioner (ANP) to join our friendly, forward-thinking Practice, serving approximately 18,500 patients across East Devon. Main duties of the job
As an Advanced Nurse Practitioner, you will:
