Job summary
Sadly due to one of our Practice Nurses leaving due to a new role closer to home, an opportunity has arisen for a proactive and enthusiastic practice nurse to join our highly effective and friendly practice team at Horncastle Medical Group. This is ideally for a full time position, however a part time position will be considered.
The role of the Practice Nurse is to provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs/ANPs, and other members of the practice and primary care team.
The ideal candidate will be registered with the NMC and have experience of working in GP practice setting.
Main duties of the job
Being a Practice Nurse will give you the opportunity to have a variation in your working week.
Some of the duties include:
- Cervical Smears
- Chronic Disease Management
- Immunisations (eg. Travel / Childhood / Flu )
- Minor Injuries
- Treatment Room Clinics
- Travel Vaccinations

Clinical Practice
- To provide and maintain a high standard of nursing care for patients as well as providing nursing assistance to GPs, and other members of the practice and primary care team.
- Assess, plan, develop, implement and evaluate treatment plans that promote health and well being, and for patients with long term conditions.
- Proactively identify, help in diagnosing and manage treatment plans for patients at risk of developing a long term condition.
- Work with patients in order to support adherence to prescribing treatments.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.

Job Summary
Horncastle Medical Group is looking for an enthusiastic and committed General Practitioner to join our friendly, established and supportive team.
We are ideally seeking a GP to work 8 sessions per week, although part-time applications are very welcome and will be considered.
You will be joining an experienced multidisciplinary clinical team including 4 GP Partners, 2 Salaried GPs, 3 Advanced Nurse Practitioners, a Paramedic and a Mental Health Practitioner, alongside our wider nursing, dispensing and administrative teams.
We are an established training practice, currently caring for 10,366 patients, and are CQC rated Good. We use SystmOne and provide dispensing services on site.
This is an exciting time to join Horncastle Medical Group. Our surgery is based in a beautiful Old Vicarage, providing a distinctive and welcoming environment for both our patients and our team. We are also investing in the future of the practice, with expansion works currently in progress to create additional clinical rooms.
