Main duties of the job
As part of our model of care to ensure we have the right people, providing the right care in the right place and at the right time, we are offering Clinical Nurse Specialist (CNS) or Clinical Paramedic Specialist (CPS) roles to work as part of our multi-disciplinary team within the Hospice Outreach Service in the community.
The Hospice Outreach Team are the initial point of contact for all new community referrals and all telephone calls coming into the service. They are at the forefront of providing palliative advice and support to patients and families in the place they call home; this includes care homes and nursing homes. You will be part of a rota of clinicians covering our 7 day a week service offering telephone support as well as clinic and home-based assessments.
Our team care for patients who have a life limiting illness and our focus is on individualised care, focusing on improving quality of life and achieving patient centred goals. We care for people with long term conditions including heart failure, respiratory disease, frailty and neurological conditions, as well as those with an oncological diagnosis. Our team strives to work alongside other community teams and specialists to optimise patient experience and improve our team knowledge and development.
About us

Primary Job Purpose
To work as part of the multi-disciplinary team in HOS (Hospice Outreach Service) to deliver high quality care to patients with a life limiting illness in the community.
To offer the patient and those who care for them, the skills of a dedicated palliative care service which encompasses symptom control, social, emotional and spiritual care.
To offer advice, support and education to the primary health care team and other healthcare professionals on palliative care issues. Giving appropriate medications when immediate need is identified on community visits and supporting other community healthcare professionals with advice about safe medication administration when needed
To strive to help us extend our hospice reach to ensure we have the right people, providing right care at the right time
You will provide clinical advice and specialist support to other professionals on palliative care issues and play a significant role in helping patients to achieve their preferred place of care and their preferred place of death.

Take and analyse a clinical history in a relevant succinct and logical manner
Record concisely, accurately, confidentially and legibly the appropriate elements of the history, examination, results of investigations, differential diagnosis and management plan
Present cases clearly, consistently and professionally to the MDT contributing effectively in patient management
Assess the patients needs. Identify their goals and work with the patient and family towards achieving them
Interpret the results of investigations and discuss and liaise with colleagues to order them appropriately
Develop knowledge of the options for achieving control of symptoms and ability to advise the primary health care team
Develop an ability to explore the personal resources of families/carers in order to ensure patients are cared for in the place of their choice
Support, advise and coach carers at home in managing a relative whose condition is deteriorating
Recognise the impact of caring for dying patients on primary health care team colleagues and offer support
Demonstrate sensitivity and respect for patients and families from different religious and cultural background and sexual orientation
Demonstrate safe mobilisation, transfer and movement of patients in the community and how and where to obtain appropriate equipment
Recognise and manage palliative care emergencies
Bring closure to the nurse-patient relationship and provide for a safe transition to another care provider
To be involved with and support the Gold Standard Framework (GSF) meetings with GPs and in Care Homes and Nursing homes.
Attend weekly MDT meetings

Liaise closely with HOS Manager and wider team on control, distribution and management of clinical workload.
Be cognisant of the Hospice in the Wealds Three-Year Strategy and Operational Business Plan.
Work to maintain a high level of risk awareness about self and patient safety
Assist in the development of policies, procedures, and guidelines to continuously improve ways of working.

Develop skills in managing complex communication issues with patients and families as a lone worker in the community;
Anticipate barriers to communication, and take action to improve the communication skills and strategies of the team

Write and maintain accurate notes/documentation and input computerised data, keeping all information up-to-date
Develop presentation skills, both oral and written.

Break bad news in steps appropriate to the understanding of the individual and be able to support distress

Job summary
We are seeking an enthusiastic and motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the Weald Primary Care Network. This is an exciting opportunity to work alongside a multidisciplinary team that includes Nurses, Clinical Pharmacists, GPs, and Care Coordinators, supporting the delivery of high-quality, patient-centred care across our member practices.

Main duties of the job
The successful candidate will play a key role in managing and supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. The postholder will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work collaboratively with GPs and the wider multidisciplinary team to coordinate ongoing care and treatment.
The role will also involve delivering a range of nurse-led clinics and Housebound and Care Home visits, including vaccinations.
We welcome applications from experienced Band 5 nurses looking to develop within primary care, newly qualified nurses with a strong interest in community and frailty care, and Band 6 nurses seeking a varied and autonomous role. The ideal candidate will be confident working independently across the PCN, demonstrate excellent communication skills, and be committed to collaborative multidisciplinary working to improve outcomes for patients.
This position offers an excellent opportunity to develop clinical skills, build strong relationships across practices, and make a meaningful contribution to integrated neighbourhood healthcare delivery.

Then join West Kent Primary Care and become a vital member of our Tunbridge Wells Primary Care Network team as a Senior Clinical Pharmacist.The Senior Clinical Pharmacist will work as part of the Primary Care Network multidisciplinary team in a patient-facing role, using expert medicines knowledge to assess, treat and support patients across the PCN. The post holder will provide senior clinical pharmacy leadership, support delivery of the NetworkContract DES and ARRS role requirements, and contribute to safe and effective person-centred medicines optimisation, quality improvement and improved outcomes for patients.
Tunbridge Wells PCN comprises of 8 practices covering 85,500 patients. Our pharmacists are given base practices to work from. Our PCN team as a collective are proactive and innovative with services being delivered by not only our large clinical pharmacy team but also by first contact paramedics, first contact physiotherapists, Mental Health Navigators, Care Co-ordinators, a dedicated pharmacy team for our local care homes and more.

Main duties of the job
The Senior PCN Pharmacist will support in the leadership of the clinical pharmacy team to improve the quality and safety of care offered to patients of the PCN. They will work alongside the PCN Head of Operations and Senior colleagues to co-ordinate the activity of the PCN Pharmacy Team, as well as maintaining their own clinical pharmacy role within the team. The Senior PCN Pharmacist will have line management responsibilities of clinical pharmacists and pharmacy technicians. The post holder must be enrolled in, or have qualified from, an approved 18-month training pathway or equivalent that enables safe and effective practice and prescribing in primary care. The role will contribute towards evening and weekend enhanced access clinics.

Job summary
We are seeking a motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, ANPs, and Care Coordinators, delivering high-quality, patient-centred care.

Main duties of the job
The successful candidate will play a key role in supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. You will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work with GPs and the multidisciplinary team to coordinate ongoing care and treatment.
The role also involves delivering a range of nurse-led clinics and housebound and care home visits, including vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, and ear irrigation etc.

Serving approximately 70,000 patients; Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices and local health and care services. Serving approximately 70,000 patients; Tonbridge PCN strives to develop collaborative working across Tonbridge GP practices and local health and care services. Tonbridge recently won a Kent and Medway award for its Integrated Home Visiting Service, jointly staffed by PCN GPs, paramedics and local community nursing teams.

Job summary
We are seeking a motivated Band 5 or Band 6 Registered Nurse to join our expanding Single Neighbourhood Team within the PCN. This is an exciting opportunity to work alongside a multidisciplinary team that includes Paramedics, Clinical Pharmacists, ANPs, and Care Coordinators, delivering high-quality, patient-centred care.

Main duties of the job
The successful candidate will play a key role in supporting our frailty cohort, including palliative care patients, housebound individuals, and those living with frailty. You will contribute to the delivery of Comprehensive Geriatric Assessments (CGAs) and work with GPs and the multidisciplinary team to coordinate ongoing care and treatment.
The role also involves delivering a range of nurse-led clinics and housebound and care home visits, including vaccinations and, where appropriately trained, procedures such as FeNO testing, ECGs, and ear irrigation etc.

Job summary
The Ridge PCN are looking for a compassionate and confident Advanced Clinical Practitioner (ACP) to play a key role in supporting the health and wellbeing of residents living in care homes.
Main duties of the job
In this role, you will bring your advanced clinical skills, warm communication style, and independent prescribing expertise to help residents stay well, avoid unnecessary hospital visits, and feel cared for. Working closely with GPs, community teams, and care home staff, you will become a familiar face in the homes you support. Working within the scope of your practice, you will assess, diagnose, prescribe, and plan care.
The role is flexible in terms of hours (up to 25 hours per week).

Tonbridge PCN is seeking to recruit an Advanced Nurse Clinical Practitioner to complement our growing PCN team of ANPs, paramedics, pharmacists, and care coordinators to support our practices in the delivery of urgent care. The PCN aims to support and strengthen delivery of primary care, offering high standards of patient care centred on the needs of our population, whilst working to support a happy and resilient workforce through the development of robust multi-disciplinary teams. A Same Day Access PCN Hub service with GP supervision using ACP clinical skills is currently in development, and we hope this will not only support patient access but provide a greater range of clinical experience for ACPs working at PCN level.

Clinical practice
The role will mainly involve working in our Same Day Access Hub seeing patients for urgent on the day issues. The role will involve working alongside PCN team of GP supervisor / Paramedic Practitioner in the PCN Same Day Access Hub seeing patients on the day with urgent issues.
The post may include cross cover with the award-winning Frailty Visiting Team visiting patients who are housebound for urgent on the day issues.
Work as part of a multi-disciplinary team to deliver safe and high-quality care.
Carry out clinical practice at an advanced level using expert clinical skills, critical judgements & knowledge to deliver appropriate and safe care.
Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with acute, long term and complex health needs.
Manage undifferentiated undiagnosed condition and identify red flags
and underlying serious pathology and take appropriate action
Triage and treat patients, including same day triage, performing assessment of their needs and makes referrals as necessary to other members of the primary and secondary health care teams as appropriate.
Interpret investigation findings to determine effective treatment plans & discuss with GPs where a second opinion is required
Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition, using both non-pharmacological and pharmacological treatments.
Co-ordinate the planning and delivery of care ensuring patients and carers or relatives are involved where required. Actively take a personalised care approach and population centre care approach to enable shared decision making.
Prescribing and reviewing medication that is appropriate for patient needs and in accordance with latest evidence-based practice, national and practice protocols and within the scope of qualified practice.
Diagnose and manage acute and chronic conditions, including drug and non-drug based treatment methods into a management plan.
Request diagnostic investigations and interpret within agreed protocols e.g. x-ray, blood tests, scans, etc.
Assess, diagnose and treat patients in practice premises or their own home who require medical attention and/or refer to appropriate agency.
