At Sirona care & health, you'll have the opportunity to make a genuine difference to people in your local community every day. Working as part of a supportive and experienced multidisciplinary team, you'll help deliver high-quality, person-centred care that enables individuals to remain independent and receive treatment in their own homes wherever possible.
As a Nurse Associate, you will work alongside Nurses, Physiotherapists, Occupational Therapists, Administrators and other healthcare professionals to provide holistic care to patients across the community.
This is a varied and rewarding role that requires excellent clinical, communication and organisational skills. You'll be confident working both independently and as part of a team, managing your workload effectively while responding to the changing needs of patients.
You'll be working on a rota basis between 8am and 8pm, across a 7-day service, contributing to the delivery of responsive and compassionate care.
A full UK driving licence and access to a vehicle are essential for this role as you will be travelling between patient locations within the community.

Main duties of the job
Undertake appropriate delegated initial holistic assessments and implement programmes of care for patients who are acutely and chronically unwell
Assess and monitor service users' health needs, assessing patient condition and well-being
Use clinical reasoning skills, within the designated competency framework, to assess, plan, implement and evaluate, patient- centred intervention programmes
Collaborate with service users, carers and family in identifying appropriate and achievable goals
Contribute to the facilitation of appropriate referrals to other services
Work within a multi-disciplinary team, to provide integrated skills and share knowledge
Assess and arrange the provision of equipment
Take every opportunity to support and educate service users and their carers to promote independence, good health and personal safety
Provide and maintain a safe and comfortable environment for all patients and carers
Support, supervise, assess and act as a role model to other learners such as Nursing Associate trainees, first year student nurses and health care support workers as required
Maintain accurate, informative and legal records of all interventions in accordance with policy
Actively participate in training; this may involve assisting in the delivery of training
Take responsibility for your own delegated objectives, training, proficiencies and competence, as outlined by the NMC code of conduct
Support delivery of vaccination campaigns

NHUC are looking for an experienced and motivated Remote Monitoring Service Manager to lead the operational delivery of our newly established Remote Monitoring service. This is an exciting opportunity to play a key role in transforming patient care through digitally enabled services that support people to remain safely at home while reducing reliance on urgent and emergency care services.
As the operational lead, you will be responsible for service mobilisation, performance management, workforce leadership, stakeholder engagement, quality assurance, and continuous service improvement. This is a full-time role based at our Farnborough head office with travel across Hampshire and the Isle of Wight as required.

Main duties of the job
The Remote Monitoring Service Manager will provide operational leadership for NHUC's Remote Monitoring service, ensuring safe, effective and patient-centred care is delivered across Hampshire and the Isle of Wight. The post holder will lead service mobilisation and ongoing delivery, manage performance against key quality and contractual standards, and support continuous service improvement.
Working closely with senior leaders, clinical teams, system partners and subcontracted providers, the role will oversee workforce planning, operational performance, patient onboarding processes, stakeholder engagement, governance and reporting arrangements. The Service Manager will also support the development of digitally enabled models of care that help patients remain safely at home and reduce reliance on urgent and emergency care services.

NHUC is a not-for-profit Community Benefit Society, working in partnership with the NHS. NHUC provides services through two divisions, Same Day Care (SDC) and TalkPlus.
SDC provides services through the Integrated Urgent Care (IUC) contract which covers the in Hours Clinical Assessment Service (CAS) and Out of Hours (OOH) provision for North Hampshire, North East Hampshire and Farnham & Surrey Heath places which are part of the Hampshire and the Isle of Wight ICB and Surrey & Sussex ICB respectively. This role is for a band 7 Sonographer rotating between the City campus, Treatment Centre ,QMC campus and the Community Diagnostic Centre due to open in 2027. NUH provides both Obstetric and non-Obstetric services across the 4 current sites which includes GP (AQP), outpatients, DVT /vascular, and inpatient scanning, and a gynaecological scanning service for the Nottingham Treatment Centre.
The Ultrasound Department is open primarily between 8am-6pm Monday-Friday. However, we are transitioning to a 7 day service to cover services within Early Pregnancy, Inpatients, CDC and SDEC.
You will be accountable to the Ultrasound Modality Manager and the Diagnostic Pathway Assistant General Managerfor Ultrasound, and as part of this role, you will work with a team of Sonographers', Radiologists and other specialties within NUH to ensure a high quality Ultrasound service to our customers.

We are looking for Radiographers who have a level of CT experience or who are considering moving to a role in CT. The post holder will work wholly within the CT scanning department. This post is for a whole-time position (37.5 hours per week), however we are willing to discuss part-time options if necessary. For those who cannot demonstrate relevant competencies, this will be a training post at band 5. Once relevant training and competencies have been completed the role will be uplifted to Band 6. This is expected to take approximately 3 months but no longer than 6 months. In your role, you will manage the workload in your area, provide high quality diagnostic CT imaging, and support more complex procedures after appropriate training. About us
The CT team is led by our Superintendent Radiographer and clinically supported and supervised by two Consultant Radiologists. If you join our team, you will join a service that is multi-disciplinary, staffed by a mix of permanent CT Radiographers and colleagues with split CT and Plain Film roles.
You will be working 37.5 hours per week across our 24/7 service, largely working 12 hour shifts. However applications from those wanting part time work will be considered.

Ward-based - Hub Coordinator
We require enthusiastic and motived individuals to work as part of the Single Transfer of Care TeamHub. The role of a Hub Coordinator either on the ward or in the Hub itself is fast-paced, busy, with changing priorities, and we are seeking staff to join this highly valued team. Supporting effective patient flow across all Pathways for admission and discharge, your contribution to the work can be both rewarding and influential as the service continues to develop and evolve.

Are you an Employment Specialist looking to make a difference in peoples lives? Do you wish to help people who are under our Secondary Mental Health Services get back into paid employment? If so you're in luck! Midlands Partnership University NHS Foundation Trust have an opportunity to join our Step On team for a 12-month fixed term contract,
You will champion of the Trust's Individual Placement and Support (IPS) Service via the Step On Team and effectively manage a caseload of 20-25 people in receipt of secondary mental health services, in order to facilitate individuals to gain and retain paid employment and/or engage in Higher Education.
A key to success will be enabling these clients to successfully make the transition into the labour market and reducing the number of service users who are not engaged in any form of vocational activity. You will work directly with mainstream employers to secure employment opportunities for service users whilst providing on-going support according to needs therefore to enable people who have experienced mental health problems to gain and retain education/employment.
You will be trained in Individual Placement and Support (IPS) or be willing to train to diploma level.
Main duties of the job
As an Employment Specialist you will effectively manage a caseload of people in receipt of secondary mental health services.
As parts of this role, you will be expected to:
- Receive complex and sensitive information in the form of referrals from Care Coordinators, to meet regularly with Case Managers to co-ordinate and integrate vocational services into mental health treatment.
- Communicate effectively with clients, giving empathy when disclosing sensitive or contentious information, providing reassurance, training and the motivation to move forward and to overcome any barriers to understanding.
- Understand a range of work procedures and practises following the Centre for Mental Health's intensive IPS Training course, Motivational Interviewing training and benefits advice programme.

We are offering an exciting and unique new role as a Band 4 Health Care Support Worker within our Integrated Mental Health Teams (IMHT) across South Staffordshire, supporting our new First Transitions Pathway.
The First Transitions Pathway is a new service to help support individuals with complex mental health as they move from the structured support of forensic services to the community-focused care offered by our Integrated Mental Health Teams. This transformation is underpinned by a central commitment to promoting sustained recovery, fostering meaningful integration within the community, and empowering service users to lead fulfilling lives. This specialised team will work in close collaboration and partnership with the six IMHTs spanning South Staffordshire.

You will be instrumental in providing practical and emotional support directly to service users, both during the immediate transition phase and in the crucial weeks and months that follow. Their focus will be on building rapport, implementing effective engagement strategies, and facilitating access to community resources.

We are looking for a highly motivated and autonomous Specialist Occupational Therapist to join our well established and evolving North Staffordshire Integrated Community Stroke Service (ICSS) giving you the opportunity to help drive developing and implementing high quality stroke rehabilitation care pathways and multidisciplinary training plans.

You will actively contribute to working in partnership with our patients, care partners and building a culture of research and development. Our service is transformational with the post holder requiring a level of NHS and stroke rehabilitation experience, high emotional intelligence and confidence to support with helping us drive development of our ICSS model.
We are based at the Haywood Hospital with our community service working across a 7 day shift pattern. Due to the nature of this role, you will be required to travel independently and efficiently over a large geographical area .
Main duties of the job
As the Specialist Occupational Therapist you will be responsible for the management of a complex stroke caseload, which includes specialist assessment and evaluation (within service specification), goal setting and care planning in conjunction with patient and care partner.
As part of this role you will be expected to,
Deliver a range of therapeutic interventions following specific occupational therapy and interdisciplinary assessments including goal planning and evaluation within a biopsychosocial approach to maximise an individual's abilities.
Develop effective communication and delegation to team members and wider professionals within service specified processes to meet the needs of the individual.
Complete and contribute to robust risk assessments & management plans promoting positive risk taking to improve quality of life and maximise occupational and functional performance/independence.
Maintain effective service and clinical governance reporting systems including contemporaneous paperwork, safeguarding and incident reporting mechanisms.
Support in the provision of formal and informal training of pre/post registration students, staff members, carers and service users.
Responsible for participating in quality driven processes, research and service development opportunities.

Job summary
The post holder will be a specialist in implantable cardiac rhythm management (CRM) devices, including CRT, conduction system pacing and leadless pacemakers, working primarily in CRM follow-up clinics and implant laboratories. They will work at an advanced Healthcare Scientist level, supporting the Principal Cardiac Physiologist for CRM in complex device implantation, follow-up and remote monitoring, and providing cross-cover for the CRM service.
The post holder will support development of the CRM MRI service, including referral triage and staff competency; lead the training and development of junior staff, including PTP/STP students; and contribute to advanced clinical practice such as Healthcare Scientist/Physiologist-led ILR implantation. They will participate in CRM/EP multidisciplinary meetings, support service management, quality improvement and patient support initiatives, and undertake delegated managerial responsibilities.
The role also requires flexibility to work across Cardiac Physiology, including non-invasive diagnostics and interventional catheter laboratories for urgent coronary and structural heart procedures. Participation in the on-call rota for primary angioplasty and the emergency arrhythmia pathway is required, providing specialist support for urgent and potentially life-threatening cardiac conditions.
Main duties of the job
- Provide cross-cover and support to the Lead Physiologist(s) for CRM in the day-to-day management and delivery of the CRM service.
- Contribute to the smooth, efficient and effective running of a high-quality clinical service and undertake additional managerial responsibilities as delegated by the Principal Physiologists.
- Work collaboratively within the multidisciplinary team, including Electrophysiology/CRM Consultants, Arrhythmia Nurse Specialists and the Heart Failure team, to support the safe and effective management of patients with cardiac implantable electronic devices.
- Demonstrate and maintain clinical proficiency and competency across a wide range of cardiac physiology techniques, working across interventional catheter laboratories and non-invasive diagnostic services as required.
- Work with the Principal Cardiac Physiologists to develop and implement advanced clinical practices within the department, including Healthcare Scientist/Physiologist-led ILR implantation services.
- Participate in the on-call service for primary angioplasty and the emergency arrhythmia pathway, providing specialist support for urgent and potentially life-threatening cardiac conditions
