Job summary
We are looking for an experienced senior registered practitioner to be part of our friendly and enthusiastic team in the Day Surgical Unit which covers both Prince Philip and Amman Valley sites. There will be an expectation to work across both these sites.
As a senior registered practitioner and team leader, you will be accountable for the delivery of a high standard of cost effective care without direct supervision, adopting current evidenced based practice whilst adhering to legal requirements, statutory rules and Health Board policies.
The successful individual will be able to provide expert clinical leadership, whilst being accountable for one's own professional actions and be responsible for the effective running of the service.
The post is a full time vacancy and will require participation in a variety of shift patterns which could include but not limited to days, evenings and weekends.

Main duties of the job
To have the responsibility in providing expert levels of practice and knowledge, promoting evidence based care within the Peri-Operative Practice and Day Surgical ward environment, working within personal, professional and corporate boundaries.
To act as a professional role model across all teams, working in accordance to the Health Board values.
Ensure that staff participate in a developing culture where openness and a positive approach to learning, change management and continuous professional development are attained.
Demonstrate the ability to deal with complex physical and psychological needs and use one's judgement in negotiating and adapting patient care to provide solutions.
Takes the lead in planning, organising and initiating developments in services.
Participate and support the Senior Nurse/ Clinical Manager in multidisciplinary clinical audit programmes including relevant standards relating to the speciality.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

- To help with patient Radiology bookings at various communitydiagnostic sites, monitoring their pathways, referrals, requests,and all coordination required to safely treat patients.
- To monitor modality booking lists and diaries, ensuring patientswho require more specific procedures/scans are booked, alsoto provide administrative support to the admin and clerical team.
Main duties of the job
Main Duties/Key Results Areas:
- To act as point of contact for patients and their representatives who contact the modalities.
- To help book and arrange for patients to be seen at sites under the Community Diagnostic Community hub and in St George's Radiology.
- To ensure that the electronic appointment work list is updated and monitored, that all appointments are made within the specific waiting time requirements and that all relevant paperwork is correctly filed and retrieved from all sites where the patients are imaged.
- To ensure images are on our PACS system when scanned at other sites.
- To ensure that relevant patient data is recorded accurately in a complete and confidential manner in accordance with Trust policy.
- The post holder will work closely with the St George's trust Superintendent Radiographers who are liaising with other diagnostic sites to assist with patient pathways and imaging wait times.

Main duties of the job
This role is focussed on supporting people in prison with a range of acute mental health needs. These needs will need to be met in line with personal recovery goals and facilitating engagement with mainstream services.
Always under the overarching framework of CPA, the postholder will contribute to the ongoing assessment, planning, delivery and review of activities and interventions against identified health, social care and wellbeing needs. You will be acting as the care/recovery coordinator for an allocated group of service users. The role will also require undertaking and delivering defined activities and interventions for a wider group of identified service users, in accordance with the agreed personal recovery plan. The job is a 8PAs a week over 4 days and you will be working alongside the multidisciplinary team, including resident doctors, and wider clinical teams, you will be the clinical expert from a medical perspective to ensure best practice care is delivered and support the team.
The post holder will offer medical input to the Step up hub team South Tyneside. The Step up Hub team Provides an enhanced intensity of interventions to patients with high complex needs , usually in stepped care or supported accommodation.
Main duties of the job
The post-holders will provide Consultant clinical leadership to the service and will assume consultant psychiatrist responsibility for their proportion of patients being looked after by the team. This includes assuming Responsible Clinician status for patients under their care, including patients subject to Community Treatment Orders, taking a lead on compliance with the Mental Health Act and CQC requirements. They will participate, directly and indirectly, in the assessment of patients referred to the service and in drawing up care plans which will address the patients' psychiatric, physical, and social needs. They will carry out detailed psychiatric assessments of patients as clinically indicated to allow accurate diagnoses and contribute to their treatment plans, including pharmacological management of psychotropic medication.
The post-holder will forge good working relationships with other psychiatrists across the Directorate in order to encourage appropriate referrals and assure a cooperative approach to the care of patients. They will be linked in with other Consultants and supported to join peer groups. They will also provide consultation and advice to members of the MDT and primary care colleagues.

Job summary
As a Support, Time and Recovery (STR) Worker within our integrated Home Treatment and Rapid Response Services, you will play a vital role in providing a safe, compassionate alternative to inpatient admission. You will work directly with individuals experiencing acute mental health crises, supporting them and their families within their own homes when they need it most.
This is a dynamic, fast-paced role operating on a 24/7 roster. You will balance providing evidence-based, recovery-oriented care with responding rapidly to urgent referrals, helping patients navigate significant distress with dignity and respect.
Key Responsibilities-
Home Treatment Function: Deliver client-centred, recovery-focused support to patients and their carers in their home environment, utilizing general mental health knowledge to promote stability and wellbeing.

Rapid Response & Assessment: Undertake crucial assessments for referrals received via the Urgent Mental Health Helpline, as well as completing gatekeeping assessments to ensure patients receive the most appropriate level of care.

Crisis Management: Provide a safe, effective alternative to hospital admission by supporting individuals experiencing acute distress in a familiar environment.

Collaborative Care: Work seamlessly within the Care Programme Approach (CPA) framework and in strict accordance with Trust and Crisis Resolution Home Treatment Team policie
Main duties of the job
- To provide support to clients of Home Treatment and Rapid Response and their carers in relation to personal care, daily living and social activities, and help maintain the social network by the use of a range of home treatment interventions.
- To carry out specified interventions, alone on occasion, as per crisis plan and update as appropriate
- To monitor progress of Home Treatment and Rapid Response clients against the crisis plans and communicate findings, recommend changes to senior HOME TREATMENT AND RAPID RESPONSE staff in order for crisis plan to be reviewed.
- To report any changes in client presentation and safety to Senior Home Treatment and Rapid Response staff
- Work flexibly in order to respond to crisis work and adjust allocated work and prioritise at short notice as required by Shift Co-Ordinator.
- Engage clients and carers with Home Treatment and Rapid Response crisis plans and negotiate options available whilst promoting crisis prevention recovery and resilience.
- Support carers to enable clients to remain at home. On occasions assist client with administration of medication as per crisis plan.

We are looking for an Administrative Assistant to join our Adult Mental Health Services based in Severn Fields Health Village, Shrewsbury. The Service offers mental health care and treatment across a variety of different age ranges and presentation. The role is fixed term for 10 months and would require you to work 37.5 hours per week (Monday to Friday 9.00am to 5.00pm). You would predominantly be based in Severn Fields, although you may be required to travel to work in other Trust locations to meet service need.
In the role you will be working as part of a team along with Administrative Assistants and Medical and Clinical Administrators. You will be expected to be able to use your own initiative and judgement, although you will be well supported by other Mental Health Admin staff and through the clinical and professional networks accessible through the community teams. Main duties of the job
- To accurately maintain and keep secure patient health and staff records/systems in line with policy and standard operating procedure, to ensure data quality, confidentiality and compliance with the current Data Protection Law.
- Maintain strict confidentiality regarding information concerning patients, staff and sensitive Trust Business at all times.
- To carry out reception duties including meeting and greeting service users, carers, relatives and visitors, as requested by the Admin Support Supervisor and/or the Admin Team Leader.
- Handle incoming and outgoing telephone calls in a polite and efficient manner. Receive and deal with telephone enquiries referring telephone queries to a more senior colleague where appropriate. Taking accurate messages and passing them to the appropriate person in a timely manner.
- Type a range of documents including emailed requests, copy typing and audio transcription (via T-Pro digital dictation system) generated by the mental health pathway teams.
- Prioritise own workload and meet deadlines in order to meet the requirements of the post.

Job summary
The Community Children's Nursing Specialist and Family Services team has an opportunity for a Band 6Community Children's Nurse (CCN) to their team in Fenton.
The Community Children's Nurse (CCN) will manage a defined caseload of CYP with a range of acute and complex health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
The service aims to support families in caring for their sick children at home, in order to avoid hospital admission and/or facilitate early discharge. This is done by the assessment of children and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.
In the role you will have responsibility for the management of a defined caseload and responsibility for the team caseload including out of hours/on call when required. The team provides a 7-day week service to include 24 hours on call for children receiving palliative care.
You will work in partnership with health colleagues and other agencies to provide potentially complex packages of care.
Main duties of the job
- Plan and organise own caseload in order to meet the needs of the children and families.
- Develop, assess, implement and evaluate plans of care working in partnership with children and families
- Provide specialist knowledge and skilled nursing care to children and families in their own home or community setting
- Be able to work flexiblyand participate in on call rota to provide 24 hour cover if needed.
- Establish and maintain links with professionals and agencies involved in the child's care, possibly taking on responsibility of key worker when appropriate.
- Work in partnership with all concerned to ensure smooth transition between hospital and home to include discharge planning.
- Identify and report any cases of suspected child abuse in accordance with Staffordshire Safeguarding Children Board (SSCB) guidelines and 2004 Children Act, and attend core group and case conferences, preparing statements and reports as appropriate.
- Assist in planning and organisation of individual therapeutic programmes of care for children and young people by attending assessment, discharge planning and safeguarding meetings, ensuring reports are submitted on the child's progress if unable to attend.

We are offering an exciting opportunity for a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) in East Staffordshire to cover Maternity Leave.The Team is an integrated clinical and care and support service providing short term reablement intervention to enable people to maximise their independence and meet their health and social care needs.
You will be responsible for scheduling and allocating care calls on the Complex Discharge Tracker System and will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales andwill ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
We are looking for a team member who is enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills. There will be a requirement to deliver face to face care/support to our community patients on an adhoc basis. The post holder is expected to commute to the office bases across South Staffordshire, if required. Shift patterns are on a rotational basis over 7 days, working early and lates shifts therefore flexibility is required for this role.
Main duties of the job
As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
As part of this role, you will be expected to:
- Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
- Attend all CDT training and briefing sessions as the system evolves.
- Ensure that all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
- Provide care/support to community patients in their own homes on a adhoc basis.

An exciting opportunity has arisen within our Pay Services Team for a Payroll Team Leader to join our friendly, supportive and customer-focused team.
Working closely with the Payroll Manager and Senior Payroll Team Leaders, you will support the delivery of an accurate and timely payroll service while helping to develop and lead a team of Payroll Assistants and Payroll Advisors.
This hybrid role combines home and office-based working, with attendance required for training, meetings and service needs. The Housekeeping staff provide a clean and safe environment to our patients, visitors and staff ensuring that a high standard of cleanliness in line with National Cleaning Standards, infection control and health and safety are met and maintained.
The post holder will be expected to work as part of the wider Housekeeping team, providing an integrated and flexible service, and as such may sometimes be required to undertake duties within other areas of the Hotel Services Department should the needs of the service demand it.
Main duties of the job
- Maintain the required standard of cleaning services following the correct procedure and frequencies
- Undertake a variety of cleaning duties in line with the ward/area/location cleaning schedules
- Operate mechanical cleaning equipment
- Replenish disposables and consumables
- Safely and correctly store cleaning chemicals and equipment
- Carry out specialist cleaning and one-off cleaning duties in line with service requirements
- Assist with laundry duties as and when required
- Where applicable - ward based duties as required:o Ward based catering duties to ensure in conjunction with nursing staff that service users are provided with appropriate mealso Regenerate food, probe, record temperature of food and serveo Ensure that meals are served in a presentable and timely manner and appropriate cutlery and condiments are suppliedo Assist to ensure dining room area is ready for service and cleared afterwardso Record waste levels at end of serviceo Clean regeneration trolley
