The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients.
Main duties of the job
To undertake duties governed by the contract and Direct and Local Enhanced Services comprising a variety of duties, including face to face surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.
About us
Primary Care Networks (PCNs), supported by Dacorum Healthcare Providers Limited (DHPL), are developing new and exciting additional roles under the 2020 scheme to support GPs and clinical teams at enhancing patient services in the local community.
There is a wide socio-economic and demographic served within this area which will provide an interesting and varied case mix. The role will contribute to improving quality of patient care across the network of practices.
DHPL has been awarded the Herts Valleys Clinical Commissioning Group (HVCCG) Certificate of Achievement in recognition of exceptional contribution to the Primary Care workforce under the category of Supported Retention and Recruitment of Staff. This is an exciting opportunity for a newly qualified Occupational Therapist or an established 5 wanting to gain physical health, inpatient experience, to join our friendly and supportive team at Hillingdon Hospital NHS Trust. To facilitate your successful transition from student to qualified OT band 5s are supported by senior therapists, and peer support, alongside band 5 meetings and in service training and team meetings. Main duties of the job
As part of the multidisciplinary team (MDT), assess the suitability of patients for provision of OT interventions following a referral.
To carry out specific standardised and non-standardised OT assessment(s) within your designated caseload.
To ensure ongoing monitoring and evaluation of intervention programmes.

Job summary
We are seeking an enthusiastic and motivated Highly Specialist Speech and Language Therapist to join our large, friendly and innovative paediatric Speech and Language Therapy team. We can offer the opportunity to work alongside a diverse range of skilled clinicians and support staff across a well-established service. The role will involve leading service development across both areas of the service. The postholder will also provide highly specialist assessment and evidence-based intervention for children accessing the Clinic and Designated Provision services. The role requires excellent clinical reasoning, communication, organisation and leadership skills, alongside the ability to make independent decisions within the scope of the role and recognise when issues should be escalated to Team Leads.

Job summary
We are seeking a motivated, hard-working, professional, and passionate Physiologist to join our Cardio-Respiratory Department.
This is an exciting opportunity to become part of our friendly, supportive team, working across both Warrington and Halton Hospital sites. Our department undertakes a wide range of respiratory investigations, and we are looking for an individual who can work independently and contribute confidently to our service.
The successful candidate will be competent in all aspects of respiratory diagnostics, including:
- Full lung function testing
- Overnight polygraphy
- CPAP initiation and follow-up
We are looking for someone with strong clinical skills, excellent communication abilities, and a genuine enthusiasm for delivering high-quality patient care.
Main duties of the job
- To be competent in all routine investigations in the department including Spirometry +/- reversibility, respiratory muscle strength, full lung function test using nitrogen washout method and/or body box.
- Reviewing and signing off spirometry performed in community clinics and ensuring reports are sent out within deadline targets.
- To undertake fitting and analysis of limited channel sleep studies and overnight oximetry.
- Analyse and produce complex reports on limited channel and respiratory sleep studies to ARTP standards.
- To support the services as they evolve and change for the benefit of our patients.

Main duties of the job
The post holder will provide specialist expertise in the development, configuration, testing and optimisation of electronic prescribing and medicines administration (ePMA) systems.
The role will support the delivery of safe, effective and efficient digital medicines services by undertaking system configuration, validation, testing and user support activities, ensuring systems are aligned to clinical workflows, medicines policy and safety standards.
Working as part of the Digital Medicines Team, the post holder will contribute to system improvements, upgrades and deployments, supporting service development and ensuring compliance with clinical safety standards (DCB0129/DCB0160).
The post holder will act as a subject matter expert for digital medicines systems, providing advice, support and training to clinical and operational staff, and contributing to the continuous improvement of digital medicines services.

As a Donor Carer with NHS Blood and Transplant (NHSBT), youll play a vital role in supporting blood donors throughout their donation journey. Working as part of a skilled and supportive mobile team, youll help create a safe, positive and caring experience for donors while contributing to the collection of lifesaving blood products.
Youll receive comprehensive training to develop practical healthcare skills, including donor screening, venepuncture and donor care. Main duties of the job
Loading, unloading, assembling and dismantling equipment to setup and prepare our blood donation session.
Driving a 7.5 tonne van and 15-seater minibus as and when required (training and assessment will be provided).
Learn and develop practical healthcare skills, including donor health screening, venepuncture and post-donation care.
Work as part of a close-knit multidisciplinary team committed to delivering high-quality donor care and service excellence.
Maintain accurate donor records and support the safe handling, reconciliation and transportation of blood products and samples.
Encourage donors to return and continue making a lifesaving difference through regular donation appointments.
Contribute to maintaining high standards of quality, safety, infection prevention and regulatory compliance.
Support continuous improvement by contributing ideas, embracing new ways of working and participating in training and development opportunities.
Travel to different donation venues and help set up, operate and pack down equipment to ensure sessions run smoothly and efficiently.

Led by the Digital Medicines Programme lead you will be responsible for leading on defined elements of Digital Medicines workstreams, providing expert input into system design, configuration, optimisation, interoperability, and governance. to ensure they are clinically safe, effective, and aligned with real-world workflows.

The role combines leadership with clinical and operational delivery, ensuring digital medicines systems continue to evolve in response to clinical need while maintaining the highest standards of patient safety and governance. The post holder will provide operational leadership and management of the Trust's Digital Medicines service, supporting the delivery, optimisation and governance of electronic prescribing and medicines administration (ePMA) systems.
The role will be responsible for the day-to-day management and continuous improvement of digital medicines services, ensuring systems are safe, effective and aligned with clinical and organisational requirements.

The post holder will be the identified lead in the development of nursing practice in own specialist area, working at an advanced level across the healthcare teams, also as a core member of the metastatic breast cancer Multi-disciplinary Team (MDT). They will be the identified key worker for patients with metastatic breast cancer and their families on the IOW, and will co-ordinate island patients care throughout their cancer pathway, including the onward referral of key worker status. The post holder will also act as a resource to all healthcare professionals caring for patients with metastatic breast cancer, and work in close collaboration with other cancer specialist nurses in mainland cancer centres, especially Queen Alexandra Hospital (PHT) and University Hospital Southampton (UHS). The post-holder will work as part of the wider cancer team and across departmental, divisional and Trust boundaries to support the delivery of a patient-centred seamless service.
Main duties of the job
The day to day work of the Cancer Clinical Nurse specialist role is essential to deliver cancer waiting targets, manage the patients cancer pathway, support with critical clinical nursing interventions, liaise with PHT specialist MDT to facilitate safe effective care within IOW with external speciality consultants, prevent admissions or expediate admission for patients with oncological emergencies, arrange blood transfusions or vital medications and treatment plans, support during breaking bad news, and ensure MDT outcomes are actioned to avoid delay in-patient cancer pathways, support visiting oncologists, provide vital patient information and signposting, and provide psychological support to cancer patients.
