Job summary
We are looking to appoint a TB Clinical Nurse Specialist to join our team.
Along with your NMC registration and post registration experience within the speciality of TB or respiratory, the successful applicant will form part of the Tuberculosis Prevention and Control Nursing Service. Serving SaTH and the communities of Shropshire, Telford & Wrekin and Powys.
The post holder will work with the TB MDT to deliver a service to reflect the needs of patients and their families affected by TB.
Main duties of the job
oActing as a clinical expert in TB management and infection control.oManaging a caseload of patients with complex needs and associated incidents/outbreaks.oLeading on contact tracing clinics.oLeading on specialist investigations such as cluster and whole genome sequencing.oSupporting and delivering clinical audits, service evaluation, and quality improvement.oDeveloping and implementing clinical policies, protocols, and guidelines.oProviding education, training, and mentorship to healthcare staff and students.oEnsuring efficient use of resources and contributing to governance, risk management, and incident review.oEnhancing patient experience through advocacy, communication, and service-user engagement.
The post holder is accountable for safe, effective clinical practice and supports the ongoing development of staff, service delivery, and patient outcomes across Shropshire, Telford & Wrekin and Powys.

Main duties of the job
Essential requirements of a Support Worker
- Flexibility - Your role will involve working shifts including weekends and Bank Holidays
- Full UK Manual Driving Licence - Not Automatic

At Care in Mind our homes are always a hub of activity and energy where the young people can feel safe, and our staff enjoy coming to work. Your role will involve working with our young people to promote independence through support with education, household tasks and general life skills. You will also update online care records and complete medication administration. In addition, you will be responsible for the general upkeep and tidiness of the home ensuring the young people live in homely environment.
At our home in Bury, you will be working with young people who have a variety of mental health diagnosis including personality disorder, those who have difficult relationships with food and those who use self-harm as a strategy to cope with their emotions. As such, you will be required to be patient and resilient as the support you will be providing to our young people will include supporting them to improve their relationship with food and managing incidents of self-harm. When our young people are preparing for moving on positively from the home into the community, you will be supporting this transition through things such as: supporting them to self-administer their own medication, re-engaging with education, accessing public transport and appointments independently. The young people enjoy in house activities such as arts/crafts, baking, cooking, board games and quiz/movie/theme nights. They also like going out in the community, on day trips to the beach or similar, shopping, eating out, bowling, walks, concerts and the cinema.
The home is a 6-bed service. You will be working with an established, experienced and diverse staff team who all have a passion for supporting the young people to achieve their goals and help them move on to full independence.
Previous care experience is not essentialand full training will be provided. You will receive ongoing support and mentoring from our friendly residential and clinical team.

As such, you will be required to be patient and resilient as the support you will be providing to our young people will include supporting them to improve their relationship with food and managing incidents of self-harm. When our young people are preparing for moving on positively from the home into the community, you will be supporting this transition through things such as: supporting them to self-administer their own medication, re-engaging with education, accessing public transport and appointments independently. The young people enjoy in house activities such as arts/crafts, baking, cooking, board games and quiz/movie/theme nights. They also like going out in the community, on day trips to the beach or similar, shopping, eating out, bowling, walks, concerts and the cinema.
The home is a 6-bed service. You will be working with an established, experienced and diverse staff team who all have a passion for supporting the young people to achieve their goals and help them move on to full independence.

We have an exciting opportunity for a Mental Health Practitioner to join our Mental Health Liaison Service.
The team is based in the Leicester Royal Infirmary and provide assessments for over 18's presenting in the acute medical setting with primary or co-morbid mental health problems. We offer an urgent assessment service to people attending the Emergency Departments in times of mental health crisis, and an assessment and liaison service to the medical wards across the 3 UHL sites. The service operates 24/7, days and nights. The Mental Health Liaison Service experiences a high number of referrals which can be complex and challenging. This provides an excellent opportunity to enhance your practice and develop your nursing skills.
We would require an enthusiastic, motivated and experienced RMN, OT or social worker with relevant post registration experience. You would work as part of the established team and would need to demonstrate flexibility and a commitment to high standards of patient care. Main duties of the job
The successful candidate will be able to work autonomously and as part of a wider team, review and triage of referrals whilst completing comprehensive patient centred assessments and completing risk assessments.

The post requires excellent communication skills both written and verbal and the ability to work in a dynamic and busy environment. Job summary
To undertake a full range of advanced (specialist and highly specialist) cardio logical diagnostic procedures.
To work independently in all aspects of cardiology. Invasive and non Invasive.
To support the trainees and assist with their training.
Support the day-to-day running of the pacemaker service alongside the pacing team, you will be expected to ensure optimum delivery of the pacing service. You will be required maintain awareness of the status of the service as a whole and provide support and, where necessary, personal participation in the work of other sections to ensure the highest possible standards of quality and efficiency are maintained.
Working to Trust and National Guidelines and in cooperation with the Procurement Service, you and the pacing team will also be responsible for ensuring the infrastructure and equipment of the implantation service is maintained in good working order and is adequate for purpose.
You and the pacing team will ensure the service is at the forefront of modern practice by the development, implementation and maintenance of education and monitoring programmes in the field of implantation technology. This will require a substantial personal involvement in the training, mentoring, supervision and assessment of cardiac physiology, nursing and medical staff.
Main duties of the job
Clinical- Generic
1. Support the pacing team with the day-to-day delivery of the pacing service and Implantable devices Service at Manor Hospital, in co-operation with the Cardiologists and Head of Technical Services, Cardiology.
2. Provision to support and oversee Domiciliary patients. Organise pacing staff to attend to these patients as well as occasional home visits.
5. Independently analyses complex data, performs complex calculations and compiles an accurate written technical report.
6 Performs a wide range of non-invasive Cardiac investigations for example ECGs, BP, Holter Monitoring, Exercise Tolerance Testing, Device downloads and device re-programming.
7. Works as part of a multi-disciplinary team within the Cath Labs during Angiograms, PCI, Device implantation and Cardioversions.
8. Promote Physiologist ILR implants - maintain competencies to gain formal written consent from patients.

Job summary
We are looking for a Salaried GP to provide 4-6 general surgeries per week on a Monday, Tuesday and/or Friday, although there will be flexibility for the right candidate.
Main duties of the job
The clinical sessions are delivered over our two sites, in Kenton and Benwell respectively, depending on the needs of the Rota. The work timetable includes participation in the on-call Rota (usually 1 full day per week) and the Care Home Ward Round Rota (one session every 2-3 weeks.)
The surgeries will include clinical admin time as well as protected time for supervision of the medical students if necessary. The role will also include up to 2 lunchtime house calls that will have been triaged by the On-call GP. The start and finish times of the surgeries can be flexible to suit individual needs.
The On-call GP covers both sites from 08:30 am-18 pm as the single point of Access and Triage of all house call requests, Urgent queries from the practice team, community and Hospital teams as well as complex medication queries.
We offer a competitive salary (both experience and qualifications will be considered), indemnity paid (in addition to salary) and 6 weeks annual leave. Betts Avenue is a long-established GP Practice operating over two sites (Kenton and Benwell) in Newcastle upon Tyne.
We are a team of 3 GP Partners supported by 3 salaried GPs, 2 long-term Locum GPs, 1 Advanced Care Practitioner, 1 Lead practice Nurse/Advanced Nurse Practitioner, 3 Practice Nurses, 1 HCA as well as an experienced managerial and admin team. We deliver a range of GMS services to a population of approximately 10,500 patients. In accordance with our practice rota, as agreed the post holder will make themselves available to undertake a variety of duties: including clinical audits, F2F surgery consultations, a triage system, use of AccuRx, video & telephone consultations, home visits, checking/signing repeat prescriptions and issuing acute scripts. They will deal with paperwork and correspondence in a timely manner.
We have a strong culture of training, teaching/staff development and will actively support and encourage development in any areas of special interest. We are seeking a caring and committed doctor who is able to demonstrate high standards of clinical care and is willing to work as part of a highly motivated team.

Join the Pharmacy Team at King George Hospital and help deliver high-quality patient-centred care across Barking, Havering and Redbridge University Hospitals NHS Trust.
We are looking for a motivated and enthusiastic Pharmacy Technician registered with the General Pharmaceutical Council (GPhC) to provide technical support across dispensary and ward-based services. Working as an Accredited Checking Pharmacy Technician, you will undertake final accuracy checking, support medicines optimisation, complete medicines reconciliation and drug histories, assess patients' own medicines, support discharge processes, and provide medication counselling.
You will work closely with pharmacists, nurses and other healthcare professionals, while contributing to service improvement projects, audits and staff development. This role offers the opportunity to use and develop a broad range of clinical and technical skills within a supportive and forward-thinking pharmacy department.
Applicants must hold recognised pharmacy technician qualifications, be an Accredited Checking Pharmacy Technician, and demonstrate excellent communication, organisational and teamwork skills.

Main duties of the job

Job summary
We require Care Navigators to join our team, are you who we are looking for?
We are a forward thinking, single practice PCN, based in Plympton and Ivybridge (Devon). We have a list size of 43,000 patients. This has allowed us to recognise the skills and knowledge within our Group and expand our team to offer our patients the best care. To ensure job security, the practice employs all members of the PCN.

We are currently recruiting to the following shift pattern:
26.25 hour post
Monday 08:00 - 13:15, Tuesday 13:00 - 18:15, Wednesday 8:00 -13:15, Thursday 13:00 - 18:15, Friday 8:00 -13:15

Main duties of the job
The post holder will work as part of an MDT supporting the triage and signposting function for all consultation requests. Working the clinical team, you will act as care navigator for our patients, making basic need assessments to identify appropriate services and support to meet their care needs.

Key tasks
Process and effectively signpost patients to the appropriate healthcare services and to appropriate local services: this requires awareness and knowledge of services/ providers and how to direct people toward them
Process patient requests for appointments, performing an assessment of patients needs
Initiate contact with and respond to requests from patients, team members and external agencies
Enter appropriate coding data on SystmOne where required
Input data into the patients healthcare records as necessary
Manage all queries as necessary in an efficient manner
Carry out system searches as requested
Support sifting clinical staff with general tasks as requested

Maintain and monitor all online requests are action in a timely manner
Support Care Navigators, providing cover during staff absences

Process online consultations requests for appointments, visits, medication support and telephone consultations.
Identify the care and support needs of the patients and ensure there are signposted correctly
Support patients to utilise support links and other services when required
Provide coordination and navigation through relevant pathways, liaising with all stakeholders as appropriate, accessing & auditing IT systems to ensure relevant information is available.
Access up-to-date, accurate information for a range of services to provide support for an individual and where appropriate carers
Initiating contact with and responding to, requests from patients, team members and external agencies
Communicate clearly and sensitively with patients, family members and other professionals
Adapt communication style and method to best support patient preference and need
Other tasks:
Undertake a variety of System One tasks to assist in the smooth running of the service.
Responding to system wide alerts.
Contributing to the development and improvement of services to patients.
To assist your colleagues in learning new procedures and offering support across the team when required.
Assist in devising and getting agreement on multi-agency guidance to support the MDT function.

The postholder will participate in any training programme implemented by the practice as part of this employment, such training to include:
- Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
- Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work.
- To partake in and help build on bespoke training for the Care Navigation service alongside Clinical Support
