Main duties of the job
The postholder will work as part of a community nursing service working across the Fylde coast, undertaking nursing duties in patients' homes or clinic settings.
To provide safe, effective, individualised care to patients and to give support to families and service users requiring scheduled and unscheduled care interventions.
The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.
To monitor the condition and health needs of patients on a continual basis in partnership with colleagues, families and carers, referring to others for reassessment, when required.

Main duties of the job
As an Assistant Technical Officer, your responsibilities are as follows:
Pharmacy Stores Responsibilities:
- Topping up stock medicines for use in wards and clinical areas across the Trust using a predefined stock list.
- Receiving medicines into the Trust, ensuring accurate documentation and organisation, and appropriate storage and handling.
- Handling the recycling process for medicines no longer required by patients or ward areas.
- Following guidance and instructions carefully whilst working in a fast-paced environment.
- Working across both Leighton Hospital and Victoria Infirmary Northwich sites is required. Travel between the sites is required using own transport or public transport, with travel expenses reimbursed in line with the Trust Expenses Policy.
Dispensary Responsibilities:
- Dispensing medicines for both inpatients and outpatients against prescriptions.
- Preparing medicines for discharge.
- Ensuring you are careful and follow guidance and instructions carefully throughout.
In both settings, your role is vital. Your attention to detail, commitment to accuracy, and dedication to patient wellbeing will contribute significantly to the high standards of service we uphold in the Pharmacy Department. An exciting opportunity has arrived to join the domestic team at Victoria Infirmary, Northwich incorporating the new Cheshire & Merseyside Surgical Centre.
Being a Hospital Domestic can be a very demanding, fast paced role. You will be expected to clean all high clinical areas within our new Surgical Centre which will include the two theatres, consultation rooms, clinical and non clinical - within the Main Hospital site to an extremely high standard. You may occasionally be asked to assist with domestic duties within other areas of the hospital with full training provided. You will need to have pride in your work and an attention to detail.
This 25 hour position is Monday to Friday only working from 4pm to 9pm.

Main duties of the job

You may need to be flexible as may occasionally be asked to travel at short notice between other sites belonging to the Trust to assist with the changing demands of the service.

Elmhurst is a 30 bedded unit providing round the clock nursing support to adult patients who are in transition from hospital to community.

It is expected that the location of this post will be within Elmhurst but with an assumption of supporting our colleagues across the CCICP footprint when needed.
The successful candidate will have Assistant Practitioner qualification (ST0215 Healthcare Assistant Practitioner Level 5)or equivalent.
Main duties of the job
We are looking for an enthusiastic, motivated and flexible Band 4 Therapy Assistant to join our friendly, supportive, dynamic team in Elmhurst.
The Therapy Assistant will take responsibility for the management of a defined workload of patients within Elmhurst following triage by the qualified therapist. This will include independent assessment & formulation and progression of a person-centred rehabilitation plan. To work as part of the team providing a comprehensive rehabilitation service under the guidance of qualified staff for patients in the Discharge to Assess Unit.There will be associated administrative duties and you will need to be computer literate with a good knowledge of Microsoft packages and have accurate keyboard skills.Our team utilises digital technology to support remote working and accommodate a flexible working approach where possible and is supported to do this with access to a laptop and mobile phone.

Job summary
This role is to provide expertise in the specialist clinical area of Pathway 2 rehabilitation and assessment community bed base, as lead clinician and co-ordinator within CCICP. It will involve high level clinical triage and knowledge alongside strategic planning and project management to ensure all strands of Pathway 2 rehabilitation, across the CCICP footprint, is provided in a responsive, timely and equitable service delivery.
The post holder will work in conjunction with key partners across CCICP and MCHT,in the planning, development and delivery of Pathway 2 and act as a central liaison and resource for referrals requiring a timely and pathway appropriate response.
The role holder will work collaboratively with Care Community Therapy Team Leads to ensure a consistent rehab therapy led model is embedded across multiple sites. The role holder's base will be IPOCH Hub in BIU Leighton Hospital, Crewe however they will be expected to travel to D2A bedded sites across the Central Cheshire Integrated Care Partnership (CCICP) footprint.

The post holder will support the workload coordinator in managing the administrative team and support in ensuring effective service delivery including monitoring performance targets. Main duties of the job
The provision of first and second line support functions in all aspects of IT within the Trust. Specifically, for the Local Area Network (LAN) and Wide Area Network (WAN) including the Service Management of all desktop, laptop and mobile devices over a multiple sites. Typically, this is achieved via the Service Desk function within the Trust, which adheres to pre-defined SLA's and also Governmental recognized National Standards (ITIL).
You will have good experience of supporting both software and hardware, with good knowledge of Windows Server 2016/2019, Windows 10/11 desktops, Active Directory, networking and PC, Laptop and peripheral support.
A flexible approach to the work ethos will help considerably, including flexibility to working times based on service needs.
A logical and objective approach in dealing with problems will be essential.
Whilst this post participates in the 24-hour shift pattern, the successful candidate will not be expected to participate in this rota for the first 2 months of service.
The standard technician shift times are 8am - 4pm, 9am - 5pm, 2pm-10pm, 10pm - 6am, 6am-2pm, 7 days per week.
As this post will be cover the 24 hours support rota, the post holder will also receive additional enhanced pay rates in line with agenda for change terms and conditions.
The successful candidate will have the ability to travel to all sites that we cover.

The role includes planning, implementing and evaluating programmes of care, initiating changes to care and escalation, if required, to the senior community nurse and the wider team.
