Main duties of the job
Main Duties
As a Practice Nurse, you will work as part of the nursing and wider clinical team to provide safe, effective and patient-centred care. Your main duties will include:
Providing a range of general practice nursing services to patients.
Undertaking chronic disease reviews and supporting the ongoing management of long-term conditions.
Carrying out immunisations, vaccinations and health promotion activities.
Providing treatment-room services, including wound care, dressings and other appropriate clinical procedures.
Supporting the delivery of practice clinical targets and quality improvement initiatives.
Working collaboratively with GPs, nurses, healthcare assistants and other members of the multidisciplinary team.
Maintaining accurate, timely and confidential patient records in accordance with practice procedures and professional standards.
Recognising and responding appropriately to changes in a patient's condition, escalating concerns where necessary.
Supporting the development and improvement of nursing services within the practice.
Maintaining professional knowledge and skills through appropriate training, learning and continuing professional development.
Working within the NMC Code of Professional Conduct and all relevant practice policies, procedures and clinical guidelines.

Assist GPs with minor surgery and coil fittings
Ensure accurate details of treatments (where applicable) are recorded in the patients notes.
Ensure accurate data entries are made on the computer record of any patient contact such as consultations, telephone advice and clinical facts e.g. blood pressures, peak flows, cervical status, vaccinations, and immunisations.
Clean and Sterilise equipment as necessary.
Maintaining stock and ordering if necessary

Meon Health Practice is looking for a Salaried GP to join our Practice across 3 sites. We are a long established, fully merged practice with around 35,000 patients. We have a diverse primary care team of social prescribers, pharmacists, physiotherapists, nurses and healthcare assistants, advanced nurse practitioners, physician associates, care coordinators, salaried GPs, retainer GPs and GP partners.

Fixed Term Salaried GP is required from November 2026 to January 2027 with potential to extend or become permanent Meon Health Practice 4 sessions Tuesday and Wednesday
Meon Health Practice is looking for a Salaried GP to join our Practice across 3 sites. We are a long established, fully merged practice with around 35,000 patients. We have a diverse primary care team of social prescribers, pharmacists, physiotherapists, nurses and healthcare assistants, advanced nurse practitioners, physician associates, care coordinators, salaried GPs, retainer GPs and GP partners.
Meon Health Practice is innovative and high achieving, but more importantly, friendly, and welcoming practice continually seeking to improve our patient care services. We have a Triage team who deal with our patients same day health concerns
We have a Complex Care team who manage the needs of our housebound patients those in residential or nursing care, or those with increasing frailty
We are a well-established training practice.

Main duties of the job
- The post-holder will make themselves available to undertake a variety of duties, including surgery consultations, triaging telephone consultations, queries, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion
- Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
- Assessing the health care needs of patients with undifferentiated and undiagnosed problems
- Screening patients for disease risk factors and early signs of illness
- Developing care plans for health in consultation with patients and in line with current practice disease management protocols
- Providing counselling and health education
- Admitting or discharging patients to and from the caseload and referring to other care providers as appropriate
- Recording clear and contemporaneous consultation notes to agreed standards
