Are you a Registered Nurse with a passion for long-term condition management and a desire to develop specialist skills in a rewarding and growing area of practice? This is an exciting developmental opportunity to join our Kent-wide Epilepsy Service as a Band 5 Development Nurse, working alongside experienced Specialist Epilepsy Nurses to build the knowledge, confidence and clinical expertise needed to become an autonomous Band 6 practitioner.

You will be supported to develop specialist skills in the assessment, treatment and ongoing management of people living with epilepsy. Working under the guidance of a Specialist Epilepsy Nurse, you will gain hands-on experience delivering holistic assessments, creating personalised care plans and supporting patients to manage their condition with confidence.

This is a unique opportunity to be part of a highly valued service that supports people throughout their epilepsy journey, including those planning a pregnancy, newly diagnosed patients and those with complex needs.

Main duties of the job

This role has been specifically designed to support your progression into a Specialist Epilepsy Nurse position. From the outset, you will have a structured professional development plan, created jointly with a Specialist Epilepsy Nurse, with clear objectives, milestones and dedicated support to help you achieve specialist competencies within 12 to 18 months.

Throughout your development, you will gradually build autonomy while managing your own caseload under supervision. You'll receive tailored learning opportunities, specialist training and ongoing mentorship to develop the clinical, communication and leadership skills required to thrive in this specialist field. A key part of the role will be empowering patients to self-manage their lifelong condition and helping them achieve the best possible quality of life.

Join our Community Nursing Team supporting housebound patients across Thanet, including Margate, Ramsgate and surrounding coastal villages. Working within Primary Care Networks, you will deliver personalised care to patients with complex and long-term conditions, helping them remain safe and independent at home. Community Nursing Teams provide personalised nursing care to housebound patients aged 18+ with complex and long-term conditions, helping them remain safe, independent and well in their chosen environment. The community chronic pain service supports people with chronic pain to achieve long-term self-management, empowering them to live well.

Main duties of the job

This patient facing clinical role includes direct patient care in community clinics and secure settings and supporting others within the service.

The post holder provides clinical leadership to the Integrated Musculoskeletal service team working within the secure environment.

We are looking for an experienced and motivated Band 6 Sexual Health Outreach Nurse to join our Sexual Health Service.

This is an opportunity to take sexual healthcare beyond traditional clinic settings and work directly with people in the communities where they live, learn and access support. Provide advice and support on contraception, emergency contraception, sexual health, healthy relationships and risk reduction. Develop trusted relationships with communities and partner organisations to improve engagement and access to services. • Provide expert advice, guidance and training to members of the multidisciplinary team to enhance knowledge and improve patient outcomes.

• Support and empower rehabilitation assistants, therapy support workers and nursing colleagues to maximise therapy opportunities throughout the patient journey.

As part of a team of Clinical Pharmacists and technicians you will be responsible for providing safe and effective medicines optimisation services to patients referred into the Integrated Pharmacy Team (IPT) and agreed community hospital sites. The Community Cardiology Team provides a specialist service for adults with complex cardiac conditions across Medway, with a particular focus on heart failure. Working within a multidisciplinary team, you will support patients with chronic cardiac conditions requiring ongoing specialist management. You will build effective relationships with patients and their families, promoting self-management, independence and understanding of their condition.

There may also be opportunities to visit patients in their own homes when clinically indicated, ensuring care is tailored to individual needs. Our inpatient care and management is overseen by a matron, along with a team of registered nurses, healthcare assistants, physiotherapists, occupational therapists' rehabilitation assistants, care management staff, a pharmacist and our visiting medical officers. Qualifications, training and experience

Essential

• A Qualifications and Credit Framework (QCF) Level 3. This could be an NVQ Level 3 apprenticeship, BTEC or equivalent international qualification in healthcare.
• In addition you will also have Functional Skills at Level 2 (Literacy and numeracy/ GCSE grade A-C/ 4 9 or equivalent in English and Maths) and be registered with the NMC as a Nursing Associate registrant.
• You'll need to have recent experience of healthcare/ working as a health care assistant gained through education, work experience or previous employment.

Desirable

• Awaiting NMC registration

Knowledge and skills

Essential

• You'll need have sound communication skills in order to understand and communicate both verbally and in writing. both verbally and in writing being mindful of potential language and cultural difficulties.
• You will need to be able to be able to use IT systems and databases.
• You will need to be able to remain professional, work well under pressure and organise your own work in order to prioritise care delivery in line with trust policies and procedures.
• You will have knowledge of up to date clinical practice by using up- to- date skills and adhering to evidence based procedures and clinical effectiveness frameworks. You will fully understand the role and remit of a Nursing Associate.
