Job summary
We are seeking a highly motivated and compassionate Band 5 Community Staff Nurse to join our dynamic and supportive District Nursing team. This is an exciting opportunity to provide high-quality, patient-centred nursing care within the homes of individuals across RCT and the surrounding areas.
As a key member of our team, you will be responsible for assessing, planning, implementing, and evaluating holistic nursing care for a diverse caseload of patients, promoting independence and well-being. You will work autonomously, managing your own caseload, whilst also collaborating closely with a multidisciplinary team including GPs, social workers, therapists, and other healthcare professionals.
This role requires excellent clinical skills, strong communication abilities, and a commitment to delivering empathetic care within the community setting. If you are a dedicated and adaptable registered nurse looking to make a significant difference to patients' lives outside of a hospital environment, we encourage you to apply.
Please note: We welcome applications from registered nurses; however, this role is not suitable for student nurses who have not yet obtained NMC registration. Candidates must be registered with the NMC at the point of application.
Main duties of the job
A Band 5 Community Staff Nurse provides holistic, patient-centred care in homes. Duties include: comprehensive assessment and individualised care planning; diverse clinical interventions like wound care, medication administration, catheter care, and palliative support; autonomous caseload management, prioritising visits and documenting accurately. They communicate effectively with patients, families, and multidisciplinary teams (GPs, OTs, Social Workers) for integrated care. The role demands strong problem-solving, health promotion, safeguarding awareness, and continuous professional development, ensuring safe, effective, and compassionate nursing care in the community.
Welsh Skills Desirable: This post is advertised as Welsh Desirable. This doesn't mean essential; whilst the candidate doesn't need to have skills in Welsh, we'll consider it an advantage when short-listing and selecting candidates. This isn't 'fluency', just Speaking & Listening skills at Level 3 (equivalent to CEFR B2) or above. Level 3 means basic conversations with patients about their everyday health. For more information, see 'Welsh Language Guidance' in the documents right at the bottom.
