Job summary
The Same Day Emergency Care units at Charing Cross and St Mary's Hospital are looking for motivated and enthusiastic healthcare assistants to join the team.
You need to be passionate about providing high quality, emergency same day care in this fast paced, evolving and exciting area of acute emergency care. You must have excellent communication skills and be excited about being part of an innovative nursing team.
This is an exciting time to join Same Day Emergency Care as we increase the numbers of patients attending the service. We see approximately 60-70 patients a day on each site, with varying presentations including renal colic, DVT, cellulitis, low risk chest pain and many more. We work closely with the Emergency Department, taking many patients from the Urgent Care Centre and ED triage, as well as patients from General Practice, London Ambulance Service and 111.
This post consists of a mixture of long days (0800-2030) and long day B shifts (0930-2200) and includes weekends, but no nights.You will be supported by a Deputy matron and the Matron of SDEC.
Main duties of the job
- Assist patients with activities of daily living when required maintaining their privacy and dignity at all times.
- Measure and record routine observations of weight, blood pressure, temperature, pulse, oxygen saturations and respiratory rate. Report any abnormalities and concerns to a Registered Nurse.
- Have an understanding of the policies and procedures within the ward and Imperial College Healthcare NHS Trust.
- Have an understanding of the needs for universal precautions at all times and be aware of the infection control policy when equipment is used.
- Have an understanding of any equipment used, safe storage, health and safety recommendations and report any breakages to the Registered Nurse.
- Ensure effective and appropriate communication with staff, patients, relatives and visitors, maintaining confidentiality at all times.
- Assist with ordering and checking of ward supplies and their storage.
- Participate in appraisal and the development of a personal development plan in agreement with the RN.
- Ensure that knowledge and skills are updated to maintain safe standards of care and treatment for patients.

Job summary
We are looking for a compassionate, motivated and dedicated Registered Nurse to join our Medicine for the Elderly ward. As a Band 5 Staff Nurse, you will provide high-quality, person-centred care to older patients with complex medical needs, promoting dignity, independence and wellbeing. You will work collaboratively with the multidisciplinary team, supporting assessment, care planning, medication administration and discharge planning.
The successful candidate will demonstrate excellent communication, organisation and clinical skills, with a commitment to continuous professional development. Previous experience in elderly care is essential or a previous placement in elderly care during your training. We offer comprehensive support, training and career development opportunities.
Main duties of the job
- Carry out planned care for a group of patients without direct supervision.
- Ensure the assessment, planning, implementation and evaluation of evidence based.
- Organise one's own workload ensuring most effective use of time.
- Assist in the orientation of new staff and students.
- Assess the needs of patients, relatives and carers and provide them with information as requested.
- Assist in the teaching and development of nursingassistants and students helping them to achieve their learning outcomes.
- Act as a role model and mentor to nursing assistants, pre-registration students and nurses undertaking supervised practice and return to nursing programmes.
- To keep up to date with current developments in nursing and ensure evidence-based practice.

The ward consists in 8 single rooms, providing specialist facilities for the care of oncologic patients. Our Mental Health Team provide specialist interventions at primary and secondary care level including; referral management, screening assessment, triage, evidence-based interventions, care planning and risk assessing, plus one-to-one and group-work facilitation.
Our Mental Health Practitioners manage a mixed and challenging caseload and are required to perform robust assessment, screening and interventions to offenders with learning disabilities and mental health conditions. You will contribute to alternatives to inpatient admission and assisting with early discharge through the implementation of high intensity interventions and complex case management arrangements.
One of our key aims is to ensure continuity of care for an offender on ordinary location and reduce the length of stay, both in relation to prison inpatient services and external NHS/independent in-patient services. You will work closely with community mental health teams to ensure appropriate sharing of information, continuity of care and the Care Programme Approach for all offenders as necessary.

About the service:
Marie Curie is launching the Solihull Identification and Support Service, a targeted, place-based model providing proactive palliative and end-of-life care across six Solihull Primary Care Networks. The service will support earlier identification, stronger end-of-life register reviews, advance care planning, treatment escalation planning, and improved coordination across primary, community and specialist care pathways. By moving from reactive crisis response to earlier community-based support, the service aims to improve outcomes and experiences for patients, families and carers, while contributing to system priorities such as better hospital flow and reduced avoidable urgent care use.
About the role:
We are looking for experienced and compassionate Clinical Nurse Specialists to help deliver this service, supporting earlier identification of people who may be approaching the last year of life and improving access to timely, coordinated, person-centred care.
Main duties of the job
Working as part of a multidisciplinary and multi-agency model, you will be embedded in primary care MDTs and support GP practices, community services and specialist palliative care partners to identify people earlier, review end-of-life registers, undertake holistic assessment, and support advance care planning, treatment escalation conversations and recording of patient preferences. The role is central to helping people, families and carers receive more planned, coordinated and responsive support closer to home.
This role offers the opportunity to help develop a new place-based service with a clear strategic focus: reaching people earlier, improving the quality and consistency of end-of-life care, strengthening partnership working with Solihull PCNs and local system partners, and using learning from the service to shape future development across Birmingham and Solihull.

DWM is a well-established, high-performing partnership between Dorset Healthcare Trust and Dorset Mental Health Forum. We are seeking motivated specialists to support people with mental health conditions into sustainable employment, working closely with NHS teams, employers and partner organisations. You will build trusting relationships and provide practical, tailored support to help individuals achieve their employment goals, with much of the work being delivered in the community.

To undertake weekly face-to-face engagement with local employers to proactively generate tailored job opportunities, including job carving, in line with IPS principles.

Provide welfare benefits advice and calculations to help clients make informed employment decisions.

The Clinical Beds team provides holistic assessments to patients who require a period of rehabilitation and assessment prior going home after being discharged from hospital.
We work closely as a multidisciplinary team alongside our partner services such as Adult social care and continuing health care to aid patients and families make long term decisions about their health and social care needs.

Main duties of the job

You will carry out profession specific, and holistic, assessments and evaluations as required to promote independence and prevent unnecessary hospital admissions. You will be part of the multi-disciplinary team; planning and developing goal focused rehabilitation/reablement programme for adults with a wide variety of complex physical and mental health needs in community beds.

Main duties of the job
The post holder will be working in a Complex Psychosis Team within the Assertive Outreach Pathway, taking responsibility to assess, plan and coordinate the care plan and treatment of a defined caseload of service users with complex needs related to psychotic illnesses.
The Complex Psychosis Teams are a multidisciplinary, community-based service responsible for providing on-going holistic assessments, treatment and support to clients with severe and enduring mental illness living in the community. The service will enable patients with severe mental health problems to be supported and cared for in the community in the least restrictive setting through an assertive, personalised and integrated approach to care and treatment.
The service will provide an intensive community-based mental health service delivered in service users' home environments and in supported living accommodation or other locations agreed with the service user utilising "A whole system approach" to recovery from mental ill health. The approach maximizes an individual's quality of life and social inclusion by supporting engagement, encouraging their skills, promoting independence and autonomy in order to give them hope for the future which leads to successful community living through appropriate support

We are looking to recruit an enthusiastic, dedicated, and reliable Mental Health Support Worker at Band 3, who wants to work with our dynamic and supportive team. MHSW's are an important component of the Glendinning team and provide a valuable source of support to both the patients and qualified staff. The role provides the opportunity and time to use your skills to develop rapport and trusting relationships with patients, which is at the core of the team's approach.
Glendinning is a 9 bedded inpatient rehabilitation unit, that supports both male and female patients requiring ongoing support at this point in their mental health recovery journey.
This post is a part-time role of 18.75 hours. With the following shift patterns:
- Early - 07:30 - 15:30
- Late - 13:00 - 21:00
- Long Day - 07:30 - 21:00
- Nights - 20.30 - 07.30 (enhanced pay)

Main duties of the job
You will carry out assigned tasks involving direct care in support of and supervised by a Registered Nurse.
You will be an active member of the multi-disciplinary team in assisting in organised activities for patients; helping to create a Therapeutic Environment for patients, monitoring patients' personal hygiene standards and ensuring these are maintained.

If so, come and join our Mental Health advisor team! We are launching the national 111, option 2 for mental health and the role of Mental Health Advisor may be the perfect fit for you!
Our new 111 press 2 for Mental Health service will support patients in crisis across the Durham Tees Valley area to be able to access mental health care by calling the NHS 111 number then pressing option two. Patients will then be able to talk to a mental health advisor and if needed with the Mental Health Crisis Line, Community mental health teams or mental health hubs throughout the area.
As an NHS 111 press 2, Mental Health Advisor, you will be the first point of contact for people who are struggling with their mental health. You will provide reassurance and comfort. You will provide basic advice and health information to patients calling 111, supporting callers to access the appropriate level of care.
You must have the Right to Work in the UK in order to be successfully appointed to this role. Please note, this role does not meet the required eligibility criteria for sponsorship under the Skilled Worker visa and therefore the Trust is unable to offer sponsorship for this particular role.
Main duties of the job

The Occupational Therapy Assistant role involves planning and implementing therapeutic activities appropriate to the needs, choices and interests of individuals in our care, under the supervision and guidance of a qualified Occupational Therapist. The Occupational Therapy Assistant will also be working with the in-house MDT which is designed to support the individual in our care throughout their journey with us.
Main duties of the job
In this role you will:
- Deliver planned activities for patients under the direction of the ward Occupational Therapist (OT) and Multidisciplinary Team
- Assist the OT in the day to day duties and running of the department.
- Undertake assessment and intervention programmes with clients as delegated by a senior occupational therapist underpinned by appropriate experience / knowledge.
- Contribute ideas, create and facilitate events and activities safely and support people to structure their time through meaningful activity.
- Have involvement in the delivery of one-to-one and group interventions.
- Implement individual care plans during activity sessions and take into account specific interests and opportunities to promote choice and independence.
