Job summary
We have a fantastic opportunity to join our Solihull Wheelchair Services.
You will lead a small, friendly team, based in a wheelchair clinical centre equipped to assess, prescribe and provide wheelchair and seating equipment to ensure the delivery of safe, effective, and person-centred care. We actively use a multi-disciplinary approach, linking with NHS services, and working in homes, schools and hospitals as well as clinic.
This rewarding role provides a unique opportunity to work with service users of all ages. Experience in posture & mobility services, childrens services, neuro, LD and rehab is an advantage. Knowledge of manual and powered wheelchairs is recommended but training is provided.
You will supervise other members of the team up to Band 6 level, and will yourself receive line management support from our Clinical Lead. Our management structure provides further clinical support from our Regional Therapist, Clinical Quality and Governance Manager and Head of Clinical Services.
The Team have a weekly meeting for information sharing and peer support.
Main duties of the job
Providing clinical leadership and expertise to the wheelchair services, working closely with service users, their families and carers and the multi-disciplinary teams supporting service users.
Supervise other members of the team up to Band 6 level.
Work as an autonomous practitioner, managing a caseload including complex service users.

Spinnaker Ward at St Mary's Hospital, Portsmouth is looking to recruit caring, motivated and enthusiastic Healthcare Support Workers to join our inpatient team.
As a Healthcare Support Worker, you will play an important role in supporting patients and registered nursing staff to deliver safe, effective and person-centred care. Main duties of the job

Late evenings and weekend on call are allocated on a rota at least 6 weeks in advance. Cover for long days and weekends that are inconvenient for any reason is arranged by the person allocated to that night or weekend by swapping with a colleague. They will be responsible for informing the rota organiser and the switchboard of any differences from the published rota.
The operational principle that the directorate aims to achieve whenever possible is for another consultant to re-use patient contact sessions that would otherwise be cancelled or remain uncovered due to leave or other agreed reasons for absence.

The aim of the PCN Heart Health Checks in Surrey Heath Primary Care Network is to provide a quality service that will help people live longer, healthier lives. As a Health Care Assistant with us, you will play a key role working under supervision and delivering our heart health checks which identifies the persons risk of developing cardiovascular disease and diabetes. Main duties of the job
To undertake NHS health checks
Phlebotomy every patient will have a blood test as part of the heart health check
Carry out baseline observations such as pulse oximetry, blood pressure, temperature, pulse rate, recording findings accurately
Accurately measure and record clinical measurements such as height, weight and waist measurements

We are seeking a skilled and compassionate Healthcare Support Worker to join our Crisis Resolution and Home Treatment Team in Portsmouth. This role is vital in supporting individuals experiencing acute mental health crises, enabling them to remain safely in their own homes as an alternative to hospital admission.
Working as part of a multidisciplinary team, you will provide high-quality, recovery focused support, undertake comprehensive assessments, delivering planned interventions and contributing to crisis and risk management plans. You will take a senior role in supporting and guiding any junior or new staff, promoting best practice, and ensuring care is delivered in line with local and national standards.
The successful candidate will have significant experience in mental health support work, strong communication and decision making skills, and the ability to work effectively in fast paced, high pressure environments. You will be confident working autonomously, responding to changing clinical needs, and building therapeutic relationships with service users, carers, and partner agencies.
Based at The Orchards, St James' Hospital, the team work across Portsmouth city, therefore there is a requirement for the post holder to have a full UK driving licence and have access to a suitable vehicle, in order to undertake home visits and respond to urgent situations in a timely manner.

In line with policy to increase the access and availability of mental health and wellbeing support, we are seeking to recruit two graduate level (or equivalent) practitioners to provide CBT based guided self-help and parenting interventions to children and young people. Based in Bakewell, our service promotes a person-centred and non-pharmacological approach to support older adults with functional health , who may be experiencing or approaching a crisis in their mental health.
We cover the High Peak and North Dales area of Derbyshire.
The aim of the service is to assist in maintaining the independence and dignity of those living with dementia, with the ultimate goal of preventing hospital admission where possible.
In addition the IRHTT can support early discharge from hospital, working closely with carers and relatives to facilitate this.
The service operates a service 365 days a year,9am-5pm seven days a week and Bank Holidays.
Main duties of the job
You will:
- Employ a wide range of skills, building on and developing their expertise in managing complex cases as part of a wider multi-disciplinary team, which includes Occupational Therapy, Physiotherapy, Nurses, Specialist Support Workers, Psychology and a Consultant Psychiatrist.
- Be clinically responsible for supporting the delivery of a flexible, responsive local service within the community.
- Be expected to travel independently across the catchment area of the service and occasionally outside of this if required.
- Work a shift pattern facilitating the service, which operates 7 days per week.
- Undertake the Duty role; triaging referrals, allocating visits, preparing and delivering handover, ensuring colleagues are "safe and well".
- Work as part of a MDT; liaising with a multitude of different professionals and agencies.
- Complete initial assessments of patients.
- Apply knowledge of the Mental Capacity Act and Mental Health Act when required.
- Assess risk and formulating safety plans accordingly.
- Formulate person-centred care plans focusing on non-pharmacological interventions.
- In-reach to inpatient wards, attending 117 meetings.
- Visit patients in their own home and those in 24 care.
- Utilise the EPR to ensure excellent documentation of patient assessment and treatment.

Main duties of the job
