Main duties of the job
As a Neuro Physiotherapist you will be part of our valued team in our Surrey Inpatient Stroke Rehabilitation and Neuro Service, receiving access to exclusive rewards and benefits. You will be working with our multidisciplinary team to deliver high quality evidence-based treatment and rehabilitation for a range of neurological conditions. The role comprises of 2 x 9-month rotations, involving inpatient Stroke Rehabilitation and a mix of neuro outpatients and community rehabilitation. You will maintain accurate records, communicate effectively with the team, and appreciate cultural and religious differences in patient care. Candidates should have CSP and HCPC memberships, a minimum of 1-year post registration experience, NHS experience, and a full UK driving licence. The service, based at Farnham Hospital, operates Monday to Friday with occasional weekend work. This role requires a DBS check and professional registration.

Job Introduction
We are looking for a highly motivated Band 6 Neuro physiotherapist to support the rehabilitation service at Farnham hospital.
The role is comprised of 2 x 9-month rotations.
- Inpatient Stroke Rehabilitation on Runfold ward, with some additional care of the elderly/neuro rehabilitation
- Split 60:40 neuro outpatients and community rehabilitation.
You will be working as part of the multidisciplinary team working to provide rehabilitation for a wide range of neurological conditions. These patients may have acute or chronic neurological impairments often with complex presentations. In the inpatient stroke rehabilitation setting there will also be some input into Older peoples Rehabilitation .
You will besupportedin the role by team leads who will steer the development of your role and clinical skills.
We actively participate in clinical supervision, in-service training and have an appraisal system to help develop you personally and your CPD. We also have a wide range of on-line training programmes tosupportyour personal development. WesupportBand 5 physiotherapists on rotation and students from London and more local universities.

The service operates Monday to Friday between 8:30am and 4:30pm with the occasional weekend and bank holiday working requirement.
Main Responsibilities
As a Band 6 Neuro Physiotherapist you will:
- To deliver high quality evidence-based treatment and rehabilitation to support the management of patients with a range of conditions.
- To maintain accurate records in line with the Chartered Society of Physiotherapy standards and Trust and service standards and policies.
- To communicate effectively with all disciplines involved in patient care, both within the organisation and those outside to ensure effective multidisciplinary working. This will include discussion of patient care, patient progress and involvement in discharge planning.
- To be able to communicate complex and sensitive information to patients, carers and other staff which may include imparting unwelcome news.
- Maintain confidentiality at all times
- Assess patients understanding of treatment proposals, gain valid consent and have the capacity to work within a legal framework with patients who lack capacity to consent to treatment.
- To have knowledge of and ability to use a range of verbal and non-verbal communication tools to communicate effectively with patients. This may include patients who will have difficulties in understanding or communicating, e.g. patients who may be deaf, blind, or be unable to accept diagnosis or presenting condition or who have cognitive and behavioural problems.
- To appreciate cultural & religious differences and take these into account when agreeing treatment plans with patients.

Main duties of the job

We are seeking a qualified clinical psychologist to work across the Bexley Older People's Community Mental Health Team (CMHT) and Bexley Memory Service to provide specialist psychological and neuropsychological assessments, and individual and group-based psychological interventions to our client group and their carers.

To provide specialist mental healthcare to offenders and to work as part of the Mental Health In-Reach Team and wider mental health services in HMP Wandsworth
To deliver a range of specialist interventions at primary and secondary care level. to include:
- Referral management, screening assessment, triage, and evidence based intervention
- One-to-one and group-work facilitation
- assertive outreach interventions
- managing a mixed and challenging caseload

Main duties of the job
- To contribute to alternatives to inpatient admission and assisting with early discharge, through the implementation of high intensity specialist interventions and complex case management arrangements resulting in continuity of care for an offender on ordinary location, reduced length of stay (both in relation to prison inpatient services and external NHS or independent in-patient services).
- To perform robust assessment, screening, and interventions to offenders with a learning disability
- Ensure that all waiting time, assessment and interventions are delivered in a timely manner, and positively contribute to the achievement of the services. performance targets.
- Ensure integrated and collaborative working with other specialist services, including continuity of care and through the gate arrangements and liaison.
- Work closely with community mental health teams (CMHTs) to ensure appropriate sharing of information, continuity of care and the Care Programme Approach (CPA) for all offenders as necessary.
