Job summary
Supporting High-Quality Diabetes Care Across the Community
This is an exciting opportunity for an experienced nurse with a passion for diabetes care to join our community-based specialist team. The role focuses on delivering expert assessment, treatment, education, and support to patients with diabetes, their families, and carers.
The successful candidate will work within a multidisciplinary team while exercising a high degree of professional autonomy in the management of patients with diabetes. They will contribute to the delivery of safe, effective, and patient-centred care, support service improvement activities, and work collaboratively with colleagues across Wiltshire to ensure consistently high standards of practice in line with local and national guidance.

Job summary
An exciting opportunity has arisen for a passionate Band 4 Therapy Assistant Practitioner to join our Falls Prevention service, working alongside our wider Rapid Response and Intermediate Care therapy services.
Our service is focused on helping people remain safe and independent within their own homes wherever possible. Youll play an important role in supporting patients following a fall, or those at risk of falling, by providing practical rehabilitation and therapy interventions that can help improve their mobility, confidence, safety and independence.
Working under the direction of our registered therapists, youll undertake delegated assessments, deliver individualized rehabilitation programmes and monitor patients progress. Youll have the opportunity to work independently within your agreed competencies, while being supported by experienced Physiotherapists, Occupational Therapists and the wider multidisciplinary team.
This is a varied and rewarding community role where you can develop your therapy and rehabilitation skills while making a real difference to patients and their families.
Please note: a full UK driving license and access to a car are essential as travelling to patients within the community is a day-to-day requirement of this role.

Main duties of the job
