We are seeking a caring and driven Rehabilitation Assistant to join our welcoming and supportive team within the Community Neurological Rehabilitation Service.

In this role, you will play a key part in delivering excellent patient care while supporting the clinical team in the smooth running of the service.

Working closely with our multidisciplinary team of Occupational Therapists, Physiotherapists, Consultants in Rehabilitation, Therapy Assistant Practitioners, Dietitians and Nurses, you will perform a variety of clinical duties.

This position is ideal for candidates with prior therapy-based experience, particularly in primary care or community health, who are enthusiastic, organised, and committed to making a positive impact on patients lives.

This is a permanent role for 30 hours per week.

Main duties of the job

The post holder will be responsible for supporting the Community Neurological Rehabilitation Team in all aspects of their clinical and administrative activities, both at St Marys Hospital and during patient home visits.

Working alongside the administrative team, the post holder should ensure that all interactions between patients, visitors, and the Community Neurological Rehabilitation Team are conducted to the highest possible standards.

The Community Neurological Rehabilitation Service provides support to people, to manage long-term neurological conditions and to live as independently as possible.

We aim to empower people to develop, recover or maintain their meaningful activities, independence and quality of life. The community Neurological Rehabilitation Service is available to adults (16+ and no longer at high school) who are registered with a Leeds GP. The team offer goal focused rehabilitation for adults with neurological conditions within a defined period of therapy

This may include advice, therapy and treatment for a patient, and advice and support for their family, carers and other professionals. We are a multidisciplinary team made up of a consultant in rehabilitation medicine, dietitians, nurses, occupational therapists, physiotherapists, psychologists and therapy assistants

• The post holder will work as part of the Community Neurological Rehabilitation team in the community and LCH based clinics city wide.

Are you a diabetes specialist nurse looking for a new role in the community, then we have the role for you.

We are currently recruiting to two full-time band 6 diabetes specialist nurse roles. To be eligible for this role you must currently work or have worked previously as a diabetes specialist nurse. The role can however be offered as a band 5-6 development role for anyone with diabetes experience, but who has not worked as a diabetes specialist nurse before.

We have an exciting opportunity for a diabetes specialist nurse to join our multi-disciplinary team, made up of nurses, dietitians, GPsi's, diabetes facilitators and pharmacists. If successful you would be working across Leeds to provide face to face and telephone clinics, carrying out home visits and providing education groups for patients with type 2 diabetes.

An exciting opportunity has arisen for a motivated and enthusiastic Dietetic Assistant to join our Adult Community Dietetic Team.

Applicants should have experience working within a UK health or social care setting supporting people with complex health needs. Previous experience within a dietetic service is desirable.

Main duties of the job

The Dietetic Assistant will support Dietitians in the assessment, monitoring and review of patients receiving dietetic intervention, helping patients, carers and professionals implement nutritional care plans effectively.

The role includes telephone reviews, face-to-face contacts and administrative duties for clinically stable patients. Responsibilities may include monitoring nutritional intake, weight and feeding tolerance; supporting the ordering of enteral feeds, feeding equipment and oral nutritional supplements; and maintaining accurate patient records.

The post holder will work across a range of settings including patients' homes, care homes, clinics and telephone consultations. They will liaise with healthcare professionals, carers and other agencies to support patient care and escalate concerns appropriately to the supervising Dietitian.

Training and ongoing support will be provided, including development in home enteral feeding, nutritional support, transition from Children's Services and supporting patients with neurological conditions such as MND.

This is a part-time position of 22.5 hours per week (0.6 WTE), worked within standard office hours.

This is an exciting opportunity to work within the MindMate Support Team (MMST) who aspire to improve mental health and wellbeing across the city. We are looking to recruit a number of Trainee Education Mental Health Practitioners (TEMHPs).

This has been driven from the Green Paper (2017) to transform Young Peoples Mental Health by focussing on early intervention. Candidates will be offered a place to train at the University of Sheffield, in delivering Low Intensity Cognitive Behaviour Therapy (LI- CBT) evidence-based early interventions, and Whole Setting Approach initiatives (WSA) within education settings.

Job summary

An exciting opportunity has arisen to join the Community Intravenous Administration Service (CIVAS) in Leeds as a Band 5 Community Staff Nurse. This rewarding role involves delivering IV therapies, including antimicrobials, diuretics and antivirals, to patients in their own homes and clinic settings, supporting early discharge and reducing hospital admissions. You will work as part of a small, experienced and supportive team, managing your own caseload while maintaining regular communication with colleagues. Band 5 Staff Nurse Short Term Care Beds Working as a Staff Nurse for Leeds Community Healthcare NHS Trust within the Short Term Care Beds service is very different from many traditional nursing roles. We work as a community within a community, providing person-centred care and rehabilitation to support patients on their recovery journey. The Short Term Care Beds service provides care for patients who require a short-term stay following discharge from hospital. While returning home is the preferred outcome for many patients, some require a temporary stay in a supportive, homely environment where 24-hour care is available. The Leeds Sexual Health Service are looking for a forward thinking, innovative enthusiastic Senior Healthcare Assistant to join our lively and friendly team. The post holder will be required to hold, be in the process of attaining, or demonstrate a willingness to obtain the Care Certificate.

You will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants. The role is based from Hunslet Health Centre, though it is expected that you could travel across the city of Leeds during your shift to deliver patient care. The contract is a min of 10 hpw (2 evenings) up to a max of 27.5 hpw (6, 5 evenings a fortnight).

Main duties of the job

You will be a registered staff nurse and have excellent assessment, planning and evaluation skills to deliver clinical care to patients and support our Nursing Assistants. You will coordinate and manage care ensuring that all calls are effectively assessed, triaged, allocated and recorded. You will have an overview of the service as a whole to ensure that patients and staff remain safe and care is appropriate and effective overnight. E.g. re-allocating work to cover unexpected staff absence. At times you will be expected to work clinically (5-10pm) and deliver direct patient care, though the majority of your role will be clinical triage via telephone. You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients. We recruiting a Band 5 Physiotherapist to join the Active Recovery service, a part of the ABU Neighbourhood and Active Recovery Team. Active Recovery deliver high-quality, person-centred rehabilitation, helping adults regain independence and remain safely in their own homes and communities. As an Physiotherapist, you will support adults following illness, injury, hospital admission, functional decline or changes in health and wellbeing. You will complete comprehensive assessments, develop personalised rehabilitation plans and deliver evidence-based interventions that promote recovery, independence and quality of life. The Active Recovery Service is an innovative partnership between Leeds Community Healthcare and Leeds City Council, providing integrated intermediate care. The service delivers short-term, goal-focused rehabilitation for people recovering from trauma, orthopaedic surgery, neurological conditions, frailty, deconditioning and long-term health conditions, using a strengths-based approach to reduce ongoing care needs. You will manage a varied caseload, undertake holistic assessments and deliver interventions including rehabilitation, falls prevention, hospital discharge support, environmental assessments and equipment provision. Working closely with multidisciplinary colleagues, you will contribute to coordinated care planning and positive patient outcomes.

Main duties of the job

In this role, you'll have the opportunity to make a real difference in your patients lives while developing a broad range of clinical skills. You will work closely with a wider multidisciplinary team, enhancing your knowledge and practice through regular clinical supervision, peer support, and continuing professional development. While you will be fully supported by senior therapists, you will also take responsibility for managing your own caseload, delivering holistic rehabilitation or appropriately delegating care. The role also includes supervision of students, participation in service improvement and quality initiatives, and access to ongoing professional development, clinical supervision and opportunities to help shape future community rehabilitation services in Leeds. This role is part of a 7-day service, with working hours between 8am and 6pm. Weekend and unsocial hours attract enhanced pay.
