The Regional Hyper-acute Rehabilitation Unit (RHRU) is a 24 bed in-patient specialist neuro-rehabilitation service. It is part of a specialist rehabilitation network and acts as a central focus for research and training of all professions in rehabilitation. The unit offers intensive multi-disciplinary neuro-rehabilitation to adults with severe and complex cognitive, communicative, physical and psychosocial difficulties. The post holder is one of the key members of the ED/UTC team; help maintain flow across the planned and emergency care pathways to achieve National Access Targets relevant to both areas. You will be mainly based at Ealing hospital but you may be required to work cross site if needed.

Job summary

St Mark's Hospital is looking to recruit a stoma and pouch care lead nurse to join their established team of stoma and pouch care team. The post holder will have responsibility for the daily operational management of the stoma and pouch care services alongside the nurse consultant, they will provide long term management and support to patients with stomas and an ileoanal pouch, and any related stoma care concerns the patient may encounter. Main duties of the job

The post holder will work as a Specialist Speech and Language Therapist within the adult acute and stroke inpatient services at Northwick Park Hospital. The post is rotational with approximately 8 months within each team. They will independently assess, diagnose and manage adults with complex acquired communication and swallowing disorders, including patients with acute medical, neurological and critical care needs, with access to senior clinical support for highly complex presentations. This is an excellent opportunity to develop experience in both inpatient and outpatient dermatology services. Clinical commitments include regular outpatient clinics, weekly minor procedures list and inpatient (referral) emergency care. There will be opportunity to present interesting cases at local meetings. The successful candidates would be expected to be actively involved in departmental audits, journal club and medical student teaching.

Main duties of the job

The post holders will be responsible for managing inpatient referrals in a timely fashion including liaison with various medical and nursing staff on wards.

ACUTE MEDICINE DEPARTMENT

The Acute Medicine service at Northwick Park University Healthcare NHSTrust has seen an exponential rise in demand over the last two decades,including increase in the medical take and Same Day Emergency Care(SDEC) services.

The Acute Medicine service is an integral part of the Urgent & EmergencyMedicine Division. The service cares for a range of patients across SDEC,the Acute Medical Unit and the Medical High Dependency Unit.

Medical take

Northwick Park is a major acute hospital with a busy medical take and anestablished reputation for patients presenting with a very wide range ormedical conditions with a high degree of acuity. The successful candidateswill work as part of the admissions team, supported by 24/7 registrarsupervision and 12 hour a day, seven day a week, consultant oversight.

Acute Medical Unit

Currently there is a 56-bedded Acute Medical Unit based over two wards(Crick and Darwin) next to the Emergency Department, which has beenextensively developed within the last five years to provide a state of the artenvironment. In early 2024 there will be a further 32 bedded AMU opening.

The appointees will work as a member of the Acute Medical team with F1trainees, IMTs, F2s, GPVTSs and SpRs. There is seven-day consultantcover across the acute floor.

Same Day Emergency Care Unit (SDEC)

Northwick Park SDEC currently reviews up to 500 patients per week (40-50% of the acute medical take). A variety of conditions are treated on theunit, which is manned by acute physicians as per rota, supervisinga dedicated team of advanced nurse practitioners, medical registrarsand core trainees, seven days per week. The SDEC is open seven days aweek and is comprehensively equipped with diagnosticsincluding ultrasound, ECHO and access to CT with specialist teaminput. The unit is continuing to grow and develop novel means of treatingacute medical disease.

As part of this development there is a Single Point of Access (SPA) thatreceives all external medical referrals to acute medicine. Staffed by trainedANPs, they triage the call to SDEC, medical take for admission or to otherservices within the Trust.

Medical High Dependency Unit

The Medical HDU is located within Dryden ward. Patients with critical careneeds including nasal high flow oxygen, CPAP, NIV and vasosuppressorsupport can be cared for in the unit, which has up to 10 level 2 beds, andan additional 12 level 1 beds. mHDU is run by a combination of respiratoryand acute physicians.

ACUTE MEDICINE DEPARTMENT The Acute Medicine service at Northwick Park University Healthcare NHSTrust has seen an exponential rise in demand over the last two decades,including increase in the medical take and Same Day Emergency Care(SDEC) services. The Acute Medicine service is an integral part of the Urgent & EmergencyMedicine Division. The service cares for a range of patients across SDEC,the Acute Medical Unit and the Medical High Dependency Unit. Medical take Northwick Park is a major acute hospital with a busy medical take and anestablished reputation for patients presenting with a very wide range ormedical conditions with a high degree of acuity. The successful candidateswill work as part of the admissions team, supported by 24/7 registrarsupervision and 12 hour a day, seven day a week, consultant oversight. Acute Medical Unit Currently there is a 56-bedded Acute Medical Unit based over two wards(Crick and Darwin) next to the Emergency Department, which has beenextensively developed within the last five years to provide a state of the artenvironment. In early 2024 there will be a further 32 bedded AMU opening. The appointees will work as a member of the Acute Medical team with F1trainees, IMTs, F2s, GPVTSs and SpRs. There is seven-day consultantcover across the acute floor. Same Day Emergency Care Unit (SDEC) Northwick Park SDEC currently reviews up to 500 patients per week (40-50% of the acute medical take). A variety of conditions are treated on theunit, which is manned by acute physicians as per rota, supervisinga dedicated team of advanced nurse practitioners, medical registrarsand core trainees, seven days per week. The SDEC is open seven days aweek and is comprehensively equipped with diagnosticsincluding ultrasound, ECHO and access to CT with specialist teaminput. The unit is continuing to grow and develop novel means of treatingacute medical disease. As part of this development there is a Single Point of Access (SPA) thatreceives all external medical referrals to acute medicine. Staffed by trainedANPs, they triage the call to SDEC, medical take for admission or to otherservices within the Trust. Medical High Dependency Unit The Medical HDU is located within Dryden ward. SHO Training Rotations in Acute, Specialty & Intensive Care Medicine Fixed Term contract - until Aug 27

The Trust is recruiting to its award winning Trust Trainee rotations including emergency medicine, acute medicine, medical specialties (including respiratory, gastroenterology, endocrinology, rheumatology, haematology, infectious diseases, oncology) and intensive care. Each rotation will be 12 months (unless you start after Aug 2026, in which case it will be less than 12 months) and include 2-3 specialties & acute medicine.

While not London Deanery funded rotations, these will all be training posts with full educational and clinical supervision. All trainees will be enrolled into the RCP-portfolio (Foundation or IMT) and entitled to study leave. PACES teaching, medical SHO procedures course and Reg Ready sim will be available.

Trainees have the opportunity to achieve sign off for Alternative Certificate for Foundation Competence* (F2) or Alternative Certificate to Enter Higher Specialty Training (IMT) depending on their stage of training and performance

All trainees will undergo a mid-year GMC approved SARD appraisal and end of year ARCP with training programme directors.

Main duties of the job

Foundation trainee already in a UK Foundation School, permission from this school is required before you transfer your training to us.

These posts will be attractive to both those post F2 (F3), pre IMT, ACCS or Core Anaesthetic training. All trainees will be expected to progress through MRCP (or FRCA) examinationsand/or apply for core medical, anaesthetic or specialty training during these posts. For those not applying for deanery posts there will be the option of renewing the contract for another year. Such an arrangement would be made before the next recruitment cycle.
