St Mark's Hospital is looking to recruit a stoma and pouch care lead nurse to join their established team of stoma and pouch care team. The post holder will have responsibility for the daily operational management of the stoma and pouch care services alongside the nurse consultant, they will provide long term management and support to patients with stomas and an ileoanal pouch, and any related stoma care concerns the patient may encounter. The Discharge Coordinator plays a vital role in supporting safe, effective, and timely patient discharges across the Trust. The postholder will work closely with ward teams, multidisciplinary professionals, patients, families, carers, and external agencies to coordinate all aspects of the discharge process and ensure a seamless transition from hospital to community services.

The post holder will be providing direct hearing managementof the adult population. This includes diagnostic assessment, speech testing and rehabilitation.

2. You will be seeing patients for routine hearing aid repair and maintenance , making any necessary adjustments patient hearing aids and impression taking.
3. Ordering and maintaining Audiology stock across all sites.
4. Auditing stock levels.
5. You will carry out various administrative duties, which includes scanning of patient referrals and reports, daily checks of both Adult and Paediatric email inboxes, responding to emails or forwarding to relevant service.
6. A clear speaking voice is essential when working with Deaf and hard of hearing people.
7. Assists with clinical supervision and training of junior staff and students.
8. Updates own professional knowledge, keeping abreast of new digital hearing aid technology, assessment techniques and best clinical practice.

Audiology Service at London North West University Healthcare NHS trust is looking for a qualified audiologist to work within their busy department in Northwick Park and Central Middlesex Hospitals.

The successful individual will be responsible for delivering comprehensive diagnostic and rehabilitative services for adults. The workload will involve working across mainly at Northwick Park Hospital and some cross site working across Central Middlesex Hospital and Ealing Hospital. Main duties of the job

Undertake full Audiological assessments of patients and to assist in complex and non-routine cases such as those with multiple disabilities and severe communication difficulties. This include history taking and carrying out hearing and auditory function tests to include pure tone audiometry, tympanometry, acoustic reflex testing and speech audiometry.

Carry out examination of the ear by Otoscopy in patients including complex cases, to identify the anatomy of an individual's ear, any abnormalities or pathology.

Analyse results to determine the next stage of diagnostics required, and to problem solve inconsistent or erroneous findings.

Take medical histories of patients directly referred from primary care to exclude pathologies requiring a medical opinion

Develop and implement Individual Plans for patients, including complex and routine cases.

Holistically obtain history including the assessment of disability, handicap and expectations.

Take accurate ear impressions using suitable material for patients to include postoperative patients and those with additional needs.

Modify ear moulds using the range of laboratory equipment available.

Job summary

The post holder will be responsible for the medical care of patients over the age of 65 admitted with rib fractures, head injuries, all routine traumatic non-femur fractures and select non-trauma patients. Part of the role is to provide routine cover Orthogeriatrics during leave. There is also scope within the job plan for further service development. This will be agreed with the successful candidate though it is anticipated this will involve improving patient care within the surgical directorate.

The post is based at Ealing Hospital and the post holder is expected to assist in the development of innovative Surgical Liaison service.
