Job summary
This team will provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that the service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided. Our service will deliver home treatment 24/7 to an identified group of children and young people in Crisis by responding quickly to those referred with urgent and high-risk mental health needs, and provide collaborative, effective, evidence based interventions at the right time with the right person.
There will be scope for creativity in this team, to develop evidence based interventions, to think systemically and work collaboratively with children, young people and families, informed by the local Transformation Plan, Future in Mind and the THRIVE framework. It really is time to think differently and creatively about how we can help more children and young people with their emotional wellbeing and mental health. You will be supported by some very lovely people in all parts of our service and the wider partnership
If you are currently working at an AfC Band 5 position, you are encouraged to apply for this role as part of a development programme.
You must be a fully registered Nurse, Social Worker or Allied Health Care Professional with appropriate mental health experience to apply. Working 37.5 per week you will be required to cover a 24 hr rota on a shift basis.
Main duties of the job
- Provide specialist assessment and care planning in conjunction with service users and carers, evaluation of individual treatment plans and discharge planning
- Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team..
- To provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.
- To contribute to the clinical service development as part of the 0-25 Emotional Health and Wellbeing Service.
- Triage and assess referrals received by service in liaison with multi-disciplinary colleagues as appropriate. Formulate and action appropriate response based on need, risk and urgency.

Job summary
The Community Children's Nursing Specialist and Family Services team has an opportunity for a Band 6Community Children's Nurse (CCN) to their team in Fenton.
The Community Children's Nurse (CCN) will manage a defined caseload of CYP with a range of acute and complex health care needs, including life limiting and life-threatening conditions requiring palliative and terminal care.
The service aims to support families in caring for their sick children at home, in order to avoid hospital admission and/or facilitate early discharge. This is done by the assessment of children and young people's healthcare needs, and the planning, development, implementation and evaluation of programmes of care which facilitate care at home and in other community settings.
In the role you will have responsibility for the management of a defined caseload and responsibility for the team caseload including out of hours/on call when required. The team provides a 7-day week service to include 24 hours on call for children receiving palliative care.
You will work in partnership with health colleagues and other agencies to provide potentially complex packages of care.
Main duties of the job
- Plan and organise own caseload in order to meet the needs of the children and families.
- Develop, assess, implement and evaluate plans of care working in partnership with children and families
- Provide specialist knowledge and skilled nursing care to children and families in their own home or community setting
- Be able to work flexiblyand participate in on call rota to provide 24 hour cover if needed.
- Establish and maintain links with professionals and agencies involved in the child's care, possibly taking on responsibility of key worker when appropriate.
- Work in partnership with all concerned to ensure smooth transition between hospital and home to include discharge planning.
- Identify and report any cases of suspected child abuse in accordance with Staffordshire Safeguarding Children Board (SSCB) guidelines and 2004 Children Act, and attend core group and case conferences, preparing statements and reports as appropriate.

Main duties of the job
The Adult Community Nursing Service provides care to patients in their own homes, covering care homes and working closely with our GP practices and integrated services such as Physio, OT and Podiatry. As a Community Staff Nurse you will deliver a high quality service, working closely with GP's and multi-disciplinary teams in addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions. We provide wound care, palliative and end of life care, leg ulcer management, continence and catheter care, management and administration of injections such as insulin and B12.
You will work independently in a community setting, participating in the ongoing assessment, planning, implementation and evaluation of programmes of care, adhering to policies and procedures.
You must be a qualified nursing practitioner with current NMC registration. Previous experience of working in the community is not an essential requirement, however, you do need to be able to demonstrate a commitment to delivering high quality targeted personalised care.
Post holders will be required to work flexibly on a rota system across a 7 day working week.

The role of the band 4 outreach support worker is to support vulnerable people who have entered the criminal justice system. The service has a focus on supporting adults who have been released from police custody and may have additional health needs. We are offering an exciting opportunity for a Band 3 Complex Discharge Tracker for the Home First (Discharge to Assess) in East Staffordshire to cover Maternity Leave.The Team is an integrated clinical and care and support service providing short term reablement intervention to enable people to maximise their independence and meet their health and social care needs.
You will be responsible for scheduling and allocating care calls on the Complex Discharge Tracker System and will be working in partnership with the Home First Locality Teams to ensure that all referrals are allocated care calls within the discharge to assess timescales andwill ensure that the Complex Discharge Tracker system is validated and updated within each locality teams.
We are looking for a team member who is enthusiastic, passionate and a dynamic individual. You will need to be motivated, flexible and an effective team player, in order to prioritise and implement effective negotiation and communication skills. There will be a requirement to deliver face to face care/support to our community patients on an adhoc basis. The post holder is expected to commute to the office bases across South Staffordshire, if required. Shift patterns are on a rotational basis over 7 days, working early and lates shifts therefore flexibility is required for this role.
Main duties of the job
As a Complex Discharge Tracker, you will review all daily Home First referrals with the Locality Teams and ensure that all patients are allocated the appropriate care calls within the discharge to assess appropriate referral timescale.
As part of this role, you will be expected to:
- Ensure that all Home First referrals from the acute portals are immediately sourced and prioritised throughout the working day.
- Attend all CDT training and briefing sessions as the system evolves.
- Ensure that all updates recorded in CDT are accurate before entering them. Ensure that any identified mistakes are corrected immediately.
- Provide care/support to community patients in their own homes on a adhoc basis.

Children and Adolescent Mental Health Services Shropshire, Telford & Wrekin (CAMHS STW) are excited to offer a full-time permanent post (09:00-17:00 Monday to Friday)
Band 6 Mental Health Practitioner role with our CAMHS Assessment Team.
This role includes screening referrals within the team's operational framework during a shift co-ordinator role that will be expected one day per week. For the rest of the working week, it will include completing initial mental health assessments for children and young people. Following assessments, you will be attending the multidisciplinary team meetings to identify the most suited outcome/ intervention for the young person based on their needs.
The role will require a good understanding of mental health, assessing and managing risks whilst working within a supportive multidisciplinary team.
Main duties of the job
You will be involved in the delivery of a high standard of Mental Health Care to Children and Young People (CYP's) who have been referred to the service.
- Your role as a shift coordinator will entail screening referrals to establish if they are urgent, crisis, routine or for signposting.
- Taking calls from professionals and parents/ guardians when there are possible concerns around mental health and risks, including safeguarding concerns.
- Work within the guidance of the service wide operational framework alongside the Thrive Model to identify stages of care needs
- You will be required to complete face to face urgent and Routine Mental Health assessments and then discuss the outcome within the daily MDT outcome meeting and identify the most appropriate treatment/ intervention and feed this back to the CYP's and families/ Guardians and professionals involved.
- The documentation that is required within the role is expected to be of a high standard and completed within the correct timeframe as shown in the service's operational framework.

Main duties of the job
Good communication skills and informed clinical judgement are vital to the role. It is imperative that you can provide evidence of working with people who have experienced a mental health problem and are able to demonstrate an understanding of how anxiety and depression present in Primary Care. You should have a real interest in helping people with anxiety and depression to achieve a level of recovery which will improve their overall wellbeing and day to day life.
You will join a team of CBT Therapists, Counsellors and Psychological Wellbeing Practitioners. You will be responsible for assessing patient suitability and delivering evidenced based CBT interventions. The post is managed and supervised within the IAPT service with some specialist supervision for Long Term Conditions.
Applicants should be BABCP accredited/working towards registration and have experience of working in Primary care mental health services for adults.
The service works core 9-5 hours but offers patient therapy between the hours of 8am-8pm Monday to Friday, and in future on Saturday mornings. Candidates should be prepared to work flexibly to meet service needs within these hours and to work a late shift on site one day per week. It is also expected that 60% of the working week will be office based and 40% worked remotely.

Job summary
We are excited to offer a developmental Band 5-6 opportunity for an ambitious and motivated Podiatrist looking to progress into a Specialist Podiatrist role. Job summary
An exciting position has arisen within our East Staffordshire Mental Health Support Team supporting schools in and around Uttoxeter. We are looking for an experienced & enthusiastic Education Mental Health Practitioner.
Youwill continue to build on the fantastic work carried out within your allocated schools. The role is varied & no two days are the same, you may be setting up and delivering group work & assemblies one day to delivering assessment & interventions the next, as well assupportingschools to develop their whole school approaches to mental health & wellbeing.
You will be skilled & experienced in conducting mental health assessments of children and young people (CYP) presenting with common mental health difficulties such as anxiety and low mood. You will conduct risk assessments & identify the right level of support for CYP you come into contact with.
Alongside the clinical aspect of your role, you will play an integral part in supporting schools with their whole school approaches. There is scope for creativity and opportunities to work alongside CYP & parents/carers to drive the whole school approach agenda.
This is an opportunity to be involved in an established team within the wider CAMHS structure, as a joint initiative between health & education. You will work across the Staffordshire locality as well as supporting other MHSTs across South Staffordshire. You will work deliver parent, staff well-being, 1:1 & group work sessions.
Main duties of the job
- Assess and deliver outcome focused, evidence-based interventions in educational settings for children and young people experiencing mild to moderate mental health difficulties, working at all times in collaboration with and giving respect to the education function of the setting in which the post-holder is deployed.
- Work in effective, evidence-based partnership with children, young people, their families and their educators in the development of plans for the intervention and agreed outcomes.

Job summary
We are looking for a compassionate, motivated and enthusiastic Community Healthcare Support Worker to join our Children and Young People (CYP) Autism Service.
This is a new, exciting role and opportunity to work as part of a multidisciplinary team supporting autistic children and young people and their families across South Staffordshire. You will be working predominantly alongside our nurses but also with our Occupational Therapists, Psychologists, Speech and Language Therapists and other professionals to deliver person-centred, evidence-based care that promotes independence, wellbeing and positive outcomes for our CYP and their families.

Brockington Parent and Baby Unit are recruiting 2 x Band 3 mental health support workers.In the role you will work on a Perinatal Mental Health Inpatient unit, delivering a high-quality clinical service to patients and their families. You will be working within a specialist multi-disciplinary team and will have access to regular supervision.
You will be delivering supportive interventions, in accordance with an agreed care plan, to an identified group of people experiencing mental health related difficulties in the Perinatal period.
You will work in conjunction with the multi-disciplinary team and will provide interventions to support the person to meet their agreed goals and coordinate and / or participate in the review of progress towards those goals, utilising recommended outcome measures, involving the service user and any identified carer throughout the clinical pathway.
