We are looking for a highly experienced and hard-working Administrator to complement our Business Support team supporting the Mental Health Social Care Pathway.
The postholder will work to support all three practice Hubs, East, West and North and work predominantly but not wholly on performance support.
Main duties of the job
We are looking for an individual who has a keen eye for detail and knowledge and experience around clinical systems supporting our client care. The postholder will be proactive in identifying data requirements and be experienced in data manipulation, extraction and inputting in a timely manner and to business requirements.
If you have experience of working within this environment and are able to meet the demands and challenges of this important role we look forward to hearing from you.
This is a demanding post and the ability to multi-task, be flexible in your approach and learn quickly is essential.

We are excited to welcome an enthusiastic Business Support Officer to join our Mental Health Administration Service. This role is specifically to support our Community Mental Health Teams based in South Staffordshire.
This key role within the team will be to provide comprehensive business support to the mental health teams, as we seek to provide an effective mental health service across South Staffordshire.
We are a supportive team, who are passionate about delivering excellent care as well as promoting staff wellbeing.
As a person, you will need to be self-motivated, with the ability to prioritise your daily workload whilst also managing your working time effectively. You will be able to work independently as well as being a good team player, with excellent communication and accuracy skills. This role will require travel across the South East Staffs area (Lichfield/Tamworth/Burton) with the occasional need to travel to other parts of Staffordshire.
Main duties of the job
- To analyse reports and extract data from all report formats for all Key Performance Indicators (Quantity & Quality) and feed this back to Managers in a format that can be used within Services/Teams for planning and supervision purposes.
- The post holder will establish and maintain a business support infrastructure necessary to assist the Mental Health Leads and senior clinicians in meeting the Trusts objectives and targets. This will include the development of systems and processes to monitor performance against the Trust's Key Performance Indicators.
- Required to have the authority/ability to approach individual clinicians and highlight performance issues and request action. To escalate poor performance/compliance as necessary via the Team and Quality Leads with a requirement to further escalate if deemed necessary via the Service Lead.
- To work with Corporate Services to identify areas of under-recording of clinical activity and work within the Services/Teams to improve recording and data quality.

Main duties of the job
3. To attend Multi-Disciplinary/ Best Interest meetings to track and update patient journeys which may involve some travelling across D2A footprint to assist with the flow of patients through the service.
4. To work pro-actively offering professional challenge to expedite discharges - reporting any delays or issues to Senior Flow Co-ordinator and Service leads.
5. Practical and technical problem solving skills to maintain the flow throughout the service by using own initiative and persuasion at times using relationships and written or verbal communication to reduce length of stay and throughout, helping colleagues achieve the same by sharing knowledge.
6. Able to work under pressure and to deadlines
7. Liaison with families and professionals including external agencies to provide timely updates and discharges with the booking of transport.

We are offering an exciting opportunity for a Band 5 NMC Registered Nurse to join our Out of Hours Community Nursing team, with vacancies across Stoke-on-Trent, Stafford and Norton Canes.
Our service operates overnight between 16:00 and 08:15, seven days a week, providing urgent nursing care to patients in their own homes. The Children's Community Learning Disability team in South Staffordshire are expanding and havean exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. Our model places our young people's needs at the heart of what we offer to ensure that they are getting the best care possible. The Children's Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission.
You will be responsible for the management of a complex caseload, which
includes, specialist assessment (within service specification), care planning in
conjunction with service user and carer, evaluation of individual treatment
plans for CYP with learning disabilities.
Main duties of the job
- Be responsible for the assessment of mental health and/or medical risk factors relating to children and young people, including co-morbidity (e.g. suicidality, self-harming behaviours and medical risk associated with mental health issues)
- To undertake physical health monitoring as appropriate i.e height, weight, blood pressure etc and be responsible for identifying appropriate action to be taken
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
- Complete and contribute robust and dynamic risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve and offer optimal quality of life and health outcomes
- Maintain an effective reporting system by observing and reporting verbally and in writing on patient conditions.
- Utilise agreed outcome measures to review the effectiveness of interventions, as part of the care planning and evaluation process. Assist in ensuring that the aims and objectives of the service are fulfilled and to identify factors which may inhibit these from being achieved.

An exciting opportunity has arisen for a full-time Senior Care Act assessor within a well established and dynamic initial response team.

We are looking to appoint a highly motivated, enthusiastic and proactive Track Administrator to join our dynamic and friendly team based at Great Wyrley Health Centre.
You will be part of a team of administrators that receive referrals from several hospital Trusts in South West and South East Staffordshire, for patients with complex needs who require ongoing support following hospital discharge.
The role includes processing referrals, tracking patients' progress and arranging the discharge of patients. You will liaise with hospital staff, families and members of the multi-disciplinary team.
We need an energetic and driven person who can support the team and liaise as appropriate with staff in acute and community settings to ensure safe and timely discharges. You will be able to work autonomously and as part of the team and should be able to effectively time manage and prioritise multiple tasks. You will need to possess excellent administrative, communication skills and organisational skills and have a positive and professional approach.
The service currently operates a 7 day service through the hours of 8am to 8pm. The hours worked will include weekends and Bank Holidays on a rota basis. The full-time role consists of working 3 various days each week, 8am until 8pm, plus one weekend within a 4 week rota (8am-4pm). Part time hours are open to negotiation.
Main duties of the job
As a Track Administrator, you will be expected to effectively provide administrative support to assist in the delivery of safe and effective patient transfer and tracking within Discharge to Assess Service (D2A).
As part of this role, you will be expected to:
Be responsible for information flow to the local health economy and for the Track function of the patient journey.
To prioritise to ensure effectiveness and efficiency in delivering objectives whilst working with minimal supervision.

Closely work with health and social care staff within the triage function and have an understanding of community Home First service.

We are offering an exciting opportunity to join our Integrated Care Coordination for Urgent Care Team as a Band 6 Clinical Triage Nurse / Paramedic based in Burslem, Stoke on Trent.
You will either be a NMC registered Triage Nurse or HCPC registered Paramedic and you will provide triage assessment, clinical support, assistance and professional expertise to all health and social care registered professionals requiring assistance and support with Admission Avoidance.
The Service is the access point to take referrals from professionals requiring Health or Social Care Service and you will work as a key member of the Integrated Care Coordination Team to ensure professionals are able to make referrals through one access point andwork independently, utilising assessment and evaluation skills to full ability in order to avoid inappropriate hospital admissions, adjust care programmes as professionally judged to be necessary, and instigate changes in care that are evidence based and in the patient's best interests.
Due to the nature of this role, you will be required to work on a shift Rota basis over 365 days, covering weekends and out of hours working.

We are offering an exciting opportunity for a Band 5 NMC Registered Nurse to join our Out of Hours Community Nursing team in the South and Central Hubs which are based in Norton Canes and Stafford.
You will be committed to providing exceptional care in patients own homes demonstrating a wide-range of clinical skills. Community services are admitting patients with a more complex acuity than ever before with increasingly specialist treatment requiring care from highly skilled staff.
You will also have an opportunity to undertake training and development to ensure you are well equipped for the role, with opportunity to advance further. Extensive in house support is available from the teams own Lead Practitioner and Community Educator.
The shifts are 18.00-06.15 and 20.00-08.15. The shifts cover 7 days per week evenings and nights to fully embrace the ethos and values of patient centred care. The team are friendly and supportive, with a key focus on staff wellbeing and safety.
Due to the nature of the role you will be expected to travel independently over a large geographical area and will ideally require a valid driving licence and access to a car.

An exciting opportunity has arisen within East Staffs Community Hospitals for a Level 1 Band 5 registered Social Worker.

We are offering an exciting opportunity for a NMC Registered Band 6 Senior Nurse Co-ordinator to join our Discharge to Access (D2A), Home First Service based in Stafford and surrounding areas of West Staffordshire.
The Home First Team provides clinical support for patients in the community to either avoid potential hospital admissions or to support people to return home through the Discharge to Assess Pathway following a period of acute care in hospital.
We pride ourselves on the delivery of quality care to community patients, many with complex needs and we actively promote the philosophy of self-care and the promotion of independence whilst offering the facilities of rehabilitation and assessment. The ethos of the team is to facilitate safe hospital discharges for patients leaving acute care and to offer them nursing support in their own home environment so they can remain in their own home and avoid unnecessary hospital admissions.
As an efficient and effective team member you will work collaboratively in a multi-disciplinary team alongside the Operational Leadership to ensure high quality care delivery to service users and provide specialist assessment and care planning in conjunction with service users and carers.
Due to the nature of this role, you will be required to work shift patterns covering a 24/7 365-day period. You will also be required to travel independently over a large geographical area.
Main duties of the job
As a Senior Nurse Co-ordinator you will assess, manage, plan and deliver care whilst providing support to your peers and team leader.
As part of this role, you will also so be expected to:
Triage referrals for the discharge to assess pathway in line with the referral criteria.
Provide holistic complex assessment, planning, implementation and evaluation of care. For example, Chronic Disease Management, Tissue Viability, Palliative Care.

We are looking to recruit a band 2 activities co-ordinator who has a passion for enabling engagement through activities. The post involves working with service users within an adult mental health ward.
We are looking for an individual who has knowledge, experience, and motivation for engaging people who require mental health inpatient care. The role will be focussed on providing a broad range of activities on a group and 1:1 basis to enable service users to engage in opportunities which actively support their recovery and transfer into a less restrictive setting. You will contribute to the delivery of an evidenced based service by working with the Multidisciplinary Team, other service providers, service users, carers and families, to provide an effective client centred service.
Main duties of the job
You will provide formalised support and practical assistance to service users in order for them to regain control over their lives and their own unique recovery process. The aim is to engage and provide activities to service users on the ward that promotes recovery as part of the therapeutic day. Activities will focus on delivering social, recreational and leisure opportunities. The post holder will work as part of the ward based team and work collaboratively with members of the multi-disciplinary team.
Activities will take place within the ward environment, but may where appropriate take place in the community or hospital grounds. You will liaise closely with registered staff on service user issues and will receive clinical supervision from the ward based occupational therapist.

We are seeking a Business Support Officer (30 hours) to provide high-quality administrative and business support, helping our teams deliver effective services to children, young people, and their families.
