An exciting opportunity has arisen within the Community Teams Adult Social Care in South staffs for a Band 4 Care Act Assessor. This is a permanent position and is an ideal opportunity for someone at the beginning of their social care career who wants to experience a wide range of opportunities in adult social services. You will be fully supported to develop your skills and experience. The team successful applicant will be joining is configured of social workers, social care assessors and Care act Assessors, therefore it would be suited to someone who has an interest in the welfare of those in the community of South Staffordshire and surrounding areas.
The role involves being part of a busy community assessment and review team and involves working closely with service-users, carers, partner agencies and other professionals providing assessment of need, signposting, information, care planning and reviewing in line with The Care Act (2014) and Mental Capacity Act (2005).
Main duties of the job
A good knowledge of the services available to support and promote independence in the community is a benefit but not essential as an induction and full training will be provided. The Trust is committed to professional development and will provide opportunity for training and routes for career progression.
We are looking for an enthusiastic, motivated and innovative individual who possesses excellent communication, time management and organisational skills. Proficient computer skills are essential. The successful candidate will have a flexible approach and be a team player who is able to forge excellent working relationships with our partner agencies.
The successful applicant will need to travel across Staffordshire and when required out of county. You will require a clean driving licence and a vehicle for business use.

We are recruiting a Fixed Term Children and Young People's Neurodevelopmental Practitioner to join our multidisciplinary team within the NHS. This new exciting role working within our established autism service and our new developing neurodevelopmental pathway and service. This offers the opportunity to work with children, young people and families to assess, support, and guide development in a range of neurodevelopmental conditions including autism spectrum disorders and ADHD.
We are looking for anyone with relevant professional registration in either: Nursing, Speech and Language Therapy, Occupational Therapy, or Psychology
You will have experience working with children with neurodevelopmental conditions. Possess strong communication skills with children, families, and professionals
You will work independently within an established multi-disciplinary team working alongside a Consultant Psychiatrist, Psychologists, Nurses and AHP professionals. You will have access to ongoing professional development and training opportunities. Supporting our staff is very important to us and we offer staff supervision both clinically and managerially on a regular basis, promoting staff wellbeing

The post holder will be responsible for working in an identified adult community mental health service, with the flexibility of working across pathways, dependent on skill and service need. They will be responsible for key working of an identified caseload.
The post holder will be responsible for undertaking clinical assessments, in accordance with the clinical service, to establish the person's needs, goals, risks and strengths in order to generate a formulation based care plan agreed with all involved. Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
Administer medication, complying with relevant trust procedures and NMC Guidance.
Assess patient's individual care needs, develop, implement and evaluate episodes of care.
Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance including; pharmacological and psychologically informed interventions.
Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.

In the Community Complex Care Team pride ourselves on our delivery of excellent, holistic patient and family centred care within the home, school and a variety of community settings. All of our client group (Adults and Children) have highly complex physical health needs, from needing support with their breathing to managing challenging seizure activity and everything in between!
We are a large, expanding team covering the whole of Staffordshire, with office bases in Stafford, Stoke-On-Trent and Lichfield. We operate in a Hub system, which is, based on geographical location allowing us to ensure a supportive, small team feel across all hubs. We provide a 24/7 nurse on call service, ensuring a constant point of contact where support can be accessed.
We are looking for Complex Care Support Workers to join our team. We do cover a 24-hour period so there is an expectation that you would be able to work days and nights and the shift times are individual to each package of care. The whole of your shift will be spent with one client, so you will get the opportunity to really make a difference.
During interview you will be able discuss which geographical location you would be able tocover. You must be able to travel to areas and for shift times where public transport is not available.
Main duties of the job
To work in patient's homes or other designated settings to deliver high quality care to patients with complex physical health care needs, including but not limited to management of tracheostomies, ventilators, enteral feeding, complex seizures and medication administration.
To assess patient care needs on a daily basis, implementing and evaluating care plans produced by qualified nurses with patients and their families.
To assess care plans, risk assessments and contingencies, in conjunction with the qualified nurses, on a regular basis to ensure consistency and reflect up to date care needs.
To ensure the delivery and quality of care given complies with local and Trust agreed policies and procedures and is of the highest standard.

We would like to welcome additional Practitioner Psychologists to be part of our CAMHS multi-disciplinary teams, working within established services.
We have 2 fixed-term 12-month vacancies for Clinical/Counselling Psychologists based in our Tamworth and Lichfield CAMHS teams, working alongside the multi-disciplinary teams and supported by a Principal Clinical Psychologist and Consultant Clinical Psychologist. Main duties of the job
- You will be responsible for the provision of a therapeutic psychological / mental health service to children & young people presenting with psychological distress and mental health difficulties. The role will incorporate complex assessments leading to psychological formulations and the delivery of effective interventions to this population.
- All clinical staff in the team assist with the duty rota (9-5 on working days), contribute to initial assessments, manage risk and act as case managers for their general caseload.
- CAMHS staff need to be able to work in a range of settings, most usually clinic settings but also including home visits, residential settings, and schools as well as attending meetings at different sites. As such, the ability to travel across South Staffordshire is required.

Do you have lived experience of a recovery journey relating to mental ill health and accessing community mental health services? Would you like to support other people who are currently experiencing mental health issues? If so, we would like to hear from you as we have an exciting opportunity to join our Integrated Mental Health Team based in Codsall as a Peer Support Worker.
The Integrated Mental Health teams work as part of a 'neighbourhood' offering place-based provision of mental health services in partnership with other local organisations. Clinicians within the IMHT will offer interventions and treatments which are concordant with NICE guidelines, and allocation within the team will be based on the needs of the service user, matching clinician skills to service user needs.
Main duties of the job
A key feature of this post is to promote individual person-centred recovery by drawing on the peer support workers own personal lived experience of mental health challenges and of accessing mental health services.
As a pivotal and highly valued member of the team, peer support workers will have a case load of identified patients who they will work with on both an individual basis and by delivering group interventions, working in collaboration with the wider multi-disciplinary team.
This formalised peer support will allow the post holder to provide a safe space for individuals to talk and make sense of their experiences whilst sharing wisdom from their own recovery to inspire hope and belief that recovery is possible.

We are seeking a compassionate and motivated Support Worker to join our specialist Children and Young People's Eating Disorders Service. Our CAMHS team in Tamworth is looking to recruit a CAMHS Practioner who will deliver a high quality, comprehensive mental health service for children and young people with serious and enduring mental health difficulties. The role involves managing a defined caseload with a focus on working with children and young people and their families who experience a complex range of needs and difficulties.
Acting as a key member of the multi-disciplinary team, the post holder will provide assessment, psychosocial, evidence-based interventions using a collaborative approach and will be skilled at risk assessment and management. They will complement the multi-disciplinary working of the team, providing supervision for other team members, as agreed with the Team Lead.
In addition you will participate in team and service development and evaluation, and liaise, advise and consult with a wide range of agencies.
Main duties of the job
- Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), and care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with mental health needs.
- Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and
- Assess patient's individual care needs, develop, implement and evaluate programmes of care.
- Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.

An exciting opportunity has opened up within the Inclusion Substance Misuse Team at HMP Gartree.
As a band 4 Recovery Support Worker you will be delivering evidence based interventions in a group and 1:1 setting with our service users.

The Speech & Language Therapy Service in Staffordshire and Stoke-on-Trent provides assessment and treatment for adults with communication and/or swallowing difficulties in community settings, including patients' own homes. We offer dysphagia, communication, and voice assessment and intervention, supported by strong leadership and compassionate management.
We are excited to offer a permanent opportunity for a Speech & Language Therapist Advanced Practitioner. This full-time role will allow you to make a real difference across the North Staffordshire and Stoke-on-Trent area, with a base in Bentilee. You will to work with our colleagues in North Staffordshire Combined Healthcare NHS Trust with a specific caseload, contribute to service development and take a leading role in shaping the delivery of speech and language therapy within the community.
The caseload which this role supports is a mixture of people with Learning Disability in the community, people with severe mental health needs in a specialist hospital setting and people with Huntington's Disease. The clinical work is diverse and interesting with MDT support in place.

We are seeking a Telephony/Receptionist to undertake a wide range of duties including call handling and reception duties for a Secure Forensic Unit and corporate Trust HQ, along with administrative support.
As the first point of contact for the Trust, you will be responsible for providing a positive customer service experience; directing callers, visitors, colleagues to the most appropriate service for ongoing care, support and advice, of which some calls can be highly distressing.
You will be part of the Out of Hours District Nurses call handling service, acting as the first point of contact for all messages; this will include urgent calls, queries and referrals for the community District Nursing service in North and South Staffordshire. The post holder will be required to identify and forward urgent calls and enquiries, some of which may be complex and sensitive, direct to the triage nursing team in line with established processes - this includes RAG rating the calls as they come through and assigning to the wait lists.
This is role is working routine shifts, however you must be prepared to cover other receptionists when they are on annual leave/ sick leave, by taking on their complete duty at short notice including night duty.

Main duties of the job
All Telephonists/Receptionists will be required to have a general understanding and ability to carry out all tasks below however, some tasks may not be required at all locations:

Main duties of the job
Working in a skilled and motivated adult community mental health team that values a range of interventions and support to help people in their recovery journey.
Delivering a high-quality service to service users and their families as part of a collaborative and supportive team.
Key-working of an identified caseload with access to regular and robust supervision.
Delivering assessment, formulation and psychologically informed interventions, in accordance with an agreed care plan, to service users experiencing a wide range of mental health difficulties. Interventions are individual and group.
You will be responsible for the assessment of care needs, including risk, and contributing to; the formulation of the person's needs and the development of a relevant plan based upon the person's negotiated goals and relevant evidence-based interventions. We are delighted to offer an exciting opportunity for an enthusiastic and motivated Band 5 HCPC Registered Physiotherapist to join our Musculoskeletal (MSK) Physiotherapy Service in the Midlands Partnership University NHS Foundation Trust based in Lichfield.
You will be working as part of a friendly team of Physiotherapists and Advanced Practice Physiotherapists. You will work independently to manage your own case load and contribute to departmental and service needs. You will provide initial assessments to service users and provide programmes of treatment, care and support required for the individual and their family or carers.
An understanding of MSK rehabilitation is desirable along with varied clinical placements as a student. We offer comprehensive and robust support and you will be supported by our highly skilled and experienced physiotherapists and the wider multidisciplinary team. Continuing Professional Development is encouraged and identified through annual appraisal. Clinical supervision is offered through groups or individual sessions as appropriate.
You should hold excellent communication skills, show initiative and team spirit as well as be committed to patient care.
You will be expected to be flexible and have the ability to travel in a timely manner to work in a variety of locations across Lichfield and surrounding regions, as the service requires.
Main duties of the job
You will join our friendly team as a Physiotherapist and will be expected to assess new service user referrals and care/support needs. Assess service users and formulate, develop, implement and evaluate programmes of treatment /care/support for individuals, families and carers. - Working on an identified adult mental health ward, with the flexibility of working across wards, dependent on skill and service need.
- Delivering high quality clinical service-to-service users and their families.

Main duties of the job
The School Nursing Targeted Intervention Service prides itself in providing health advice and support to children, young people and their families within Stoke on Trent.
The key responsibilities of the role include: working closely with our special and mainstream schools in the Stoke on Trent area, supporting teacher training and care planning for children with specific health needs, delivering nocturnal enuresis clinics, toileting/continence issues, advice and support to parents regarding physical and emotional health needs of their child and completing review health assessments with looked after children. The post holder will need to have excellent communication skills and be confident in the roles of safeguarding children.
The post is a permanent post for 30 hours per week, the service is delivered between Monday to Friday, 9.00am to 5.00 pm. There will be flexibility for the hours to be delivered within these days and times.

An exciting opportunity has arisen to apply for the post of part-time Mental Health Act Administrator within the Midlands Partnership University NHS Foundation Trust. We are looking for a bright, enthusiastic and flexible individual prepared to acquire a working knowledge of the operation and application of the Mental Health Act 1983 (MHA) and related legislation to work as part of a small, close knit team in this extremely busy department, dealing with a complex specialised area of work.
You will report to the Head of MHA and MCA and be a member of the Mental Health Legislation Team. You will be based in The Redwoods Centre, Shrewsbury, however may on occasion be required to work and provide cover at St George's Hospital, Stafford.
On a day to day basis you will deliver, maintain and promote operational compliance with statutory legislation in respect of the administration of the MHA, safeguarding and promoting the rights of some of our most vulnerable patients.
You will possess excellent written and verbal communication skills, a methodical and flexible approach, the ability to work under pressure and a keen eye for detail. You will also be confident in dealing with professionals at all levels both internal and external to the organisation, place a great deal of importance on accuracy and have the ability to work as part of a team.

Staffordshire and Shropshire Health Informatics Service (S&SHIS) is a shared service hosted by Midlands Partnership University NHS Foundation Trust, providing digital and IT services to NHS organisations across Staffordshire and Shropshire.
We are seeking a motivated, customer-focused individual to join our Service Delivery Team based at Lawton House, Trentham. As a Service Delivery Engineer, you will provide high-quality IT support to staff across our customer organisations, helping to maintain the digital services that support frontline healthcare.
You will investigate, diagnose and resolve a wide range of IT incidents and service requests across desktop, laptop, application and peripheral technologies, delivering support remotely and on-site. You will take ownership of issues, build positive relationships with users and provide excellent customer service.
Working with colleagues across Service Delivery and specialist teams, you will help ensure NHS staff can access the systems and digital tools they rely on every day. This role offers the opportunity to develop your technical knowledge, contribute to service improvements and support the delivery of safe and effective patient care across the local health economy.
Main duties of the job
- Provide high-quality, customer-focused IT support to staff across S&SHIS customer organisations, ensuring an excellent user experience.
- Investigate, diagnose and resolve a wide range of IT incidents and service requests relating to desktop devices, laptops, software applications, peripherals and user access.
- Deliver support through a combination of telephone, remote and on-site assistance, taking ownership of issues and seeing them through to resolution.
- Install, configure, maintain and deploy end-user devices and associated software in line with agreed standards and procedures.
- Participate in duty-based working arrangements, supporting frontline customer enquiries, technical resolution activities, equipment deployments and service request fulfilment.
- Work collaboratively with colleagues, suppliers and specialist teams to resolve complex technical issues and support service continuity.
- Maintain accurate records of incidents, requests, asset information and technical activity within the service management system.
- Contribute to service improvement by identifying recurring issues, sharing knowledge and supporting the implementation of new technologies, tools and ways of working.
- Build positive working relationships with customers and colleagues, communicating technical information clearly and professionally to users of varying technical ability.
- Support healthcare services across Staffordshire and Shropshire through regular travel to customer sites as required.
